Accident type, without injuries

Roof fall occurred at the headgate area of the longwall 16-ft wide, 12 ft high, 12 ft long. Fall occurred in front of the longwall supports in an area of the roof where a sandstone roll was present. Fall was reported and investigated by MSHA and State roof control specialist. No injury or equipment damage to report from this occurrence.

Accident type, without injuries

Roof fall was discovered in the West Mains #5 entry at #48 cross-cut. Fall measured 45 feet long x 14 ft wide x 6 ft high. Area had been bolted to plan. Area was re-supported with wooden cribs and pack setter posts. No injury's and no equipment damage and did not block escape ways. Roof fall was immediately called in the MSHA and WV hotline.

Over-exertion in lifting objects

EE was assisting another EE in carrying a piece of 20"" ventilation tubing to the #3 face. As he was doing this, he felt a pain in his left shoulder. EE continued to work until 4/2/10, when he sought medical attention for his injury. EE was told by doctor to stay off work until seen by an orthopedic surgeon for further eval. 1st day off work was 4/5/10.

Struck by falling object

Unloding a face drill out of bed of a hummer by hand, EE pulled on drill, lost his balance and the drill struck his right foot.

Over-exertion (Not Elsewhere Classified)

EE was walking in the 8-Butt conveyor belt entry at the #30 crosscut when he stepped into a hole twisting his right knee. EE cont. to work until 11/09/09 when he sought medical treatment and was put off work at that time. First day off for this injury is 11/09/09. The hole was caused in the mine floor where water was coming up out of the bottom.

EE was unhanging bolter cable from the roof when his foot got stuck in fire-clay bottom. As he was pulling his foot free he twisted his left knee. Later during the same shift he again got his foot stuck while he was hanging the bolter cable and again twisted his left knee. EE is not off work as of 10/7/09 pending results from MRI.

Fall to the walkway or working surface

EE was working on the longwall face when his foot caught the edge of an anti-skid plate mounted on the support pontoon causing EE to trip and fall. He used his left arm catching his self to break the fall and afterwards he started feeling pain in left shoulder. EE continued to work until 10/6/09 when he went off work for injury and is off work pending MRI results.

Caught in, under or between a moving and a stationary object

He was carrying a 40"" water prop from inby end to the outby end of the longwall tailpiece. As he was doing so,he stepped up onto a 30"" crib block that was positioned between the tailpiece alignment jack & coal rib to cross over the jack. The crib block was not tight against the rib and when he applied his weight to the crib it gave way allowing his left leg to drop down between

EE stepped into rut in mine floor while walking causing injury to left knee.

Employee was bent over removing parts from underneath shelt when he felt a pain in his lower back. Did not seek medical treatment until 5/11/09.

EE stepped into a hole while walking down the track entry, twisting his right knee. EE continued to work until June 18th, when he reported off work for his injury.

Employee slipped and fell onto the concrete floor while walking from the entrance door of the portal building to the check in/out board to remove his tag.

Struck by flying object

Employee was standing on longwall support drum to mark and pull adjacent support when a piece of rock deflected off the shear cutter drum landing on top of right foot striking foot behind metatarsal gaurd causing injury.

EE was walking from building to building when he stepped into a rut in the roadway hyperextending his left knee. EE continued to work until 09/08/2009 before missing time for his injury and is now off work for surgery.

Person was walking around the mantrip when he stepped into a rut in the roadway causing him to slip injurying his knee.

Person was stepping off the rear platform of the roof bolter and when doing so he stepped into a hole in the pavement twisting his left knee causing injury.

Struck by falling object

While pulling longwall supports at shear a rock fell off shear body and struck on top of left foot fracturing big toe. Person was wearing metatarsal boots at the time of the accident.

Person was pulling rib steel from rib when he felt pain in shoulder. Went to doctor, told not to return to work pending MRI results. First day off for this injury 1/20/09.

Struck against a moving object

While operating shuttle car, operator felt pain in right arm after shuttle car hit a hole in roadway. Operator continued to work until 1/30/09 when he sought medical treatment for the pain. Pain was diagnosed as a repetitive strain injury. The operator uses his right arm to operate the steering on the shuttle car. First day off the job 2/2/09.

Struck by flying object

EE was struck in the mouth while installing a roof bolt when the wrench he was using broke. A piece broke off of the wrench and struck the EE in the upper lip resulting in a laceration which required sutures to close.

EE was removing materials from belt discharge area and while doing this he used a knife to cut a piece of scrap welding cable to remove. While cutting the jacket off this cable, the blade slipped and came in contact with his left thumb, resulting in a laceration to the left thumb. EE was wearing work gloves at the time of the accident.

Struck by falling object

Person was advancing longwall shields and when crossing from #50 to #49 shield a rock fell from between the two shields and struck the person on the left ear lacerating the ear.
