F-Mine elevator was down due to intermittent faults. No employees were on the elevator. This incident was immediately reported.

F-Mine Elevator broke down with 7 employees on it. All employees were able to get off with no injuries reported. This incident was immediately called in to the hotline.

An unplanned and reportable roof fall was discovered in the #4 entry at 172 break F-Mains intake/track. There were no injuries, no property damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

Elevator was not called in due to troubleshooting. MSHA inspector said that this needed to be called in, so we did.

Struck by powered moving object

The victim was fatally injured when ejected from the operator's compartment of a diesel scoop after the scoop was struck by a locomotive that had lost control of a trip of supplies along the 41 Butt track haulage.

Over-exertion (Not Elsewhere Classified)

Employee was walking on the beltline and slipped. When the employee slipped, EE tried to catch self on the belt rail and heard a pop in EE's right arm/tricep area. Employee is awaiting an MRI.

Struck against a moving object

The employee was operating the #2 Brookville locomotive on 41-butt intake along with another operator in #1 Brookville and a trip of cars. The locomotives lost control of the trip at approximately 47 break on the track going inby. At 58 break, the #2 Brookville struck the #1 913 scoop in the entry.

Fall from machine

Employee was climbing down off the Longwall tailgate onto the gob wing when EE's foot slipped off the gob wing. Employee landed awkwardly on EE's feet between the support pontoon and the gob wing twisting EE's right knee. Employee was taken off work on 10/3/25 pending an MRI.

Fall onto or against objects

Employee was walking around skids of dust in a crosscut. Employee slipped/tripped falling on a block landing on their left arm. Employee was seen at family doctor and referred to an orthopedic. On 7/30/25 it was determined they had a torn left triceps tendon and would need surgery.

Struck by falling object

EE walked beside the bolter in the bolted entry to throw cable out of the way.A rock fell from the rib 8' high measuring 16"" long by 2.5"" wide.The rock hit the EE on the left forearm then the left boot.When it struck EEs boot, the rock went through the top of the LaCross Metatarsal guard mining boot causing a laceration to the top of EEs foot around the toes.EE got 2 pins/sutures.

Struck by flying object

Employee was helping re-rail a motor. While the motor was being jacked up, a 3""x4"" piece of blocking kicked out striking the employee in the left cheek.

Employee was climbing over the section tailpiece to assist the employee on the other side that was moving the tailpiece jill poke to move belt. When doing so, the employee was throwing the jill poke up onto the tailpiece which landed on the employee's right ring finger that was coming across the tailpiece, resulting in a fracture to the tip of the finger.
