Employee claimed back pain next day after using chain falls on 10-28-04.
Injured was using a 3/4 ton come-a-long to couple carrier to a locomotive. The come-a-long spun around catching his finger between the handle and the chain.
injured was using a come-a-long to remove the door from the longwall head drive. the door fell on injured leg.
slack was being taken out of face conveyor chain. the chain was anchored with wedges. pressure from the chain flights on the hold down metal caused the metal to break allowing the flight to strike injured on the left leg.
Injured was riding in the passenger seat on top of the man trip, traveling to the section. Injured had his about 1/2 of his right foot sticking out of the confines of the mantrip body. Injured caught his right foot on a piece of a roll of belt beside the track causing his foot to be bent backward.
Sitting in passenger compartment of mantrip waiting for block signal light to clear. Another mantrip's brakes failed causing it to collide with stopped mantrip. This caused pain in injured's lower back.
The slack in the longwall belt monorail trolleys was being pulled up. A sheave wheel and wire rope was being used. The injured employee walked along he belt placing himself in the ""vee"" of the wire rope. The sheave wheel came loose from it's mounting and flew across the belt striking the injured employee in the jaw. This resulted in a broken jaw.
Tramming empty shuttle car over uneven bottom back to continuous miner. Shuttle car bounced casuing the seat back to come out of back hinges. Injured fell forward striking the panic bar with his left shoulder. (Injured did not request medical treatment until 04/26/2004.)
The injured employee had been washing out a screen from the inside. As the injured climbed from the screen, his foot slipped causing him to twist his left knee.
A SCOOP WAS BEING USED TO PULLED FOUR 6"" STEEL PIPE TO THE LOADING POINT IN #1 ENTRY. THE INJURED EE WAS WALKING BESIDE THE PIPE. ONE OF THE PIPES CAUGHT ON AN OLD TRACK SWITCH RAIL. THIS CAUS ED THE PIPE TO SWING UP AND ACROSS THE ENTRY STRIKING THE INJURED EE IN THE CHEST, HEAD AND NECK AREA. THIS RESULTED IN A CONTUSION TO THE NECK.
THE INJURED EE WAS SHOVELING IN THE BELT TAKE UP AREA WHERE THE HYDRAULIC TAKE UP JACK IS POSITION. A HYDRAULIC HOSE BLEW OFF THE JACK STRIKING THE INJURED IN THE BACK.
THE INJURED EE WAS AT THE TAILPIECE CONTROLS WITH HIS BACK TO THE RIB. THE INJURED WAS APPROX 3-6 FT FROM THE RIB. A PIECE OF COAL FELL FROM THETOP OF THE RIB AND STRUCK INJURED ON HIS BACK AND NECK. THIS PUSHED INJURED INTO THE TAILPIECECONTROLS. THIS RESULTED IN A DISLOCATION OF VERTEBRA IN HIS NECK.
THE INJURED WAS CHECKING BELT STRUCTURE IN A BELT CHANNEL. A ROCK 30"" BY 23"" BY 7"" FELL FROM BETWEEN THE ROWS OF ROOF BOLTS AND STRUCK INJURED ON HIS SHOULDER AND HEAD.
THE INJURED EE WAS CARRYING A HOSE TO #91 SHIELD. THE SHIELDS WERE PULLED UP. A PUSH PRIME WAS PUT IN TO ADVANCE THE PAN LINE OVER. WHEN THE PAN LINE WAS ADVANCED, THE INJURED'S RIGHT FOOT SHI PPED OFF THE PONTOON. #92 SHIELD ADVANCED TO THE FACE ONTO INJURED RIGHT FOOT. THE INJURED PUSHED A STOP BUTTON AND PUSHED THE SHIELD OFF HIS FOOT.
TWO EMPLOYEES WERE LOADING A LARGE VALVE INTO A MAN BASKET. THE VALVE DROPPED INTO THE BASKET, ROLLED OVER AND MASHED ONE EMPLOYEE'S LEFT PINKY FINGER.
foeman walking from tail to head along the longwall walkway. walkway had incline. slippery mine floor at #104 shield caused injured foot to slip. injured attempted to catch himself with left forearm. injured struck his forearm against the spillboard resulting a fractured elbow.
The injured was using a knife to cut a hole in a piece of belt. The knife cut through the belt and into injured left wrist. This resulted in a cut requiring approximately 16 stiches.
A SCOOP WAS BEING USED TO PULL FOUR 6"" STEEL WATER PIPES TO THE LOADING POINT IN #1 ENTRY. ONE OF THE 6"" PIPE S CAUGHT ON A OLD TRACK SWITCH RAIL. THIS CAUSED PIPE TO SWING UP ACROSS THE ENTRY STRIKING THE INJURED EE IN THE HEAD. THE INJURED EE WAS WALKING BESIDE THE PIPES BEING PULLED. THE INJURED EE WAS UNCONSCIOUS BRIEFLY AFTER BEING STRUCK BY THE PIPE.
