a methance gass ignition occurred on the 12 east continous miner section. no injuries or damage to equipment entailed.

An eletrical glitch in the system caused the elevator to stop asending. Two persons were on the elevator at the time and were confined to it for approximately 2 hours. The elevator service company found the glitch and reset the system. The elevator was tested and found to be operating properly by the elevator company.

An unplanned roof fall occurred in the old works area behind 17 kickback in the mno 2nd east. The fall was above the anchorage zone. There was no one trapped or ventilation blockage or disruption.

An unplanned roof fall has occurred in the 10 east #3 tailgate entry 4xc behind the working face. This fall was not discovered until 04/06/09 during the weekly old works fireboss examination. No entrapment of miners, ventilation interruption or impeding of passage occurred.

injured and coworkers were recovering old belt take up unit when a beam struck injured in the head. this resulted in a concussion and stitches being required.

Struck by flying object

Employee was holding cable, cable broke away employee was unable to hold cable any longer. While jumping out of the way employee was struck by cable.

Injured was closing the outer door of the elevator when he got his left hand caught between the doors. this resluted in a fractured left hand.

Injured was climbing down a front end loader ladder when he felt pain in his right shoulder.

Injured was pumping cement mix in water hole. He got legs wet with water and mix, causing burns to right and left legs.

EE injured was bending a 8 foot roof bolt.The bolt slipped and injured strained his back.

A hydraulic hose burst striking injured on the side of his left resluting in blood accumulating within the eye ball.

Injured was helping pull pipe into area behind the longwall. The pipe became hung. When freed the pipe sprang back hitting injured and knocking him down. This resulted in a back strain.

injured was carrying t2 5 gallon bucket of oil when his foot slipped causing him to twist his back.

injured was changing a shuttle car tire. as injured rolled the tire out of the way it fell over striking injured on his right leg resulting in a contusion. (injured did not request medical attention until 04/13/09 and recieved same.)

Employee got powder on wrist causing an allergic reaction, burn on right wrist.

Injured and coworker were moving an ""I"" beam across the track. The beam flange caught on the rail and swung around and struck injured on his right shoulder and arm.

Fall onto or against objects

EE was carrying crib bag, tripped on rock and fell, striking right knee on rock.

Employee suffered two fractures to the right foot as a result of a slip and fall incident.

Flash burns (electric)

Employee was connecting the scoop battery charging cable into the recepticle on the scoop's batteries, when arcing occurred.

Employee was walking in an entry to set a pump and slipped and caught his fall.

Fall down stairs

Employee was traveling down steps leading from an overcast when the steps fell. Employee refused medical treatment on 2/11/09 and sought medical treatment on 2/12/09 claiming an injured shoulder.

Injured was cutting a belt when the knife slipped cutting his left arm.

injured was shoveling along the belt line when he felt pain in his back.

Employee tripped over an object that was used to keep open a service door and claims to have injured the foot.

Injured was helping move power center. Injured stepped into deep tire track in the roadway. Injured's foot slipped causing him to fall forward exerting pressure on his left ankle and leg. This resulted in a fracture to the leg.

Injured had washed out engine compartment of mantrip. In closing the engine hood she caught her hand in a fold of the hood. Injured pulled her hand out causing a laceration requiring stitches to the palm.

Injured was installing a roof bolt when the roof bolt wrench broke. The wrench struck injured on his upper lip resulting in 4 stitches.

Injured was in process of installing a d-5 pump at the south dams. A screwdriver he was using spun around and caught his left hand resulting in a puncture wound. No stitches were required and injured was released for work. (This injury was not determined to be reportable until subsequent evaluations as non medical issues were discovered and are still pending.)

Injured caught his fingers between the man trip engine cover and housing resulting in a laceration to the fingers required 3 stiches.

Injured was using a pry bar to remove a stuck bottom roller from the belt line. The bar slipped catching injured's thumb between the bar and roller frame, resulting in a fracture.

Injured was replacing a bucket elevator motor and caught his finger between the motor and the speed reducer coupling.

Contact with heat

Injured suffered heat exhaustion while working on a pump.
