Accident type, without injuries

the elevator electrical controler malfunction in mid trip causing the otis elevator to stop. 10 employes were stranded for a period of 2 hours. the levator was lower to allow egress of the men. the elevator was removed from service immediately until repaired.

A bearing on the govenor shaft on the Otis elevator failed causing the elevator to stop in mid trip. The men riding the elevator were stranded for approximately 10 minutes on the elevator. The elevator was removed from service and the bearing was replaced by Kone elevator maintenance personel on 12/11/05.

A ROOF FALL HAS OCCURRED IN THE MNO #2 #10 ENTRY AT SPAD 10+54. THE FALL OCCURRED ABOVE THE ANCHORAGE ZONE. THE ENTRY IS NOT A NORMALLY TRAVELED ENTRY. THE FALL MEASURED APPROXIMATELY 5 FEET HIGH AND 25 FEET LONG AND HAS BEEN DANGERED OFF. THERE WAS NO DISRUPTION OF WORKFORCE, BLOCKAGE OF TRAVELWAYS OR AIR AS A RESULT OF THIS FALL OR DAMAGE TO ANY EQUIPMENT.

a methane gas ignition occured on the longwall face when cutting hard rock on the tail to head pass.

A ROCK FALL HAS OCCURRED IN THE 2ND EAST OUTBY ESCAPEWAY. THE FALL MEASURED 20 FEET WIDE BY 8 FEET HIGH BY 40 FEET LONG. THE ESCAPEWAY WAS REROUTED TO THE ADJACENT ENTRY.

THE OTIS ELEVATOR MALFUNCTIONED, CAUSING IT TO STOP BEFORE REACHING THE BOTTOM OF THE ELEVATOR SHAFT. EMPLOYEES WERE ENTRAPPED FOR MORE THAN 30 MINUTES. THE ELEVATOR SETTINGS WERE RESET AND TESTED AND FUNCTIONED NORMALLY AFTERWARDS.

A rock fall occurred on the main north #2 belt line at approximately 5:00 pm on Friday May 13, 2005. the rock fall was approximately 4-4 1/2 feet high, 20 feet wide and 40 feet long.

The mine slope belt motors stopped due to power failure. The brake on the slope belt failed to engage allowing the slope belt to run backward. This caused the fluid couplings to rotate backward. The fluid couplings failed and blew apart.

injured was advancing a shield manually. injured was sitting on pontoon. when shield was advanced a rock fell from between the shields striking injured employee on his right foot and ankle.

injured was lifting wood crib blocks to build crib in longwall shield recovery stall when he felt pain in stomach muscles.

Injured was removing a rock from the shearer. The rock shifted and cut injured's hand.

Fall from machine

injured was dismounting a 930 loader. his foot slipped causing him to fall to the ground. injured tried to catch himself with his left arm and hand. injured jammed his hand into the ground resulting in a fracture to the left foream bone.

INJURED WAS CLEANING THE CANOPY TOP OF LONGWALL SHIELD. A ROCK FELL WHILE INJURED WAS PULLING ROCK FROM THE CANOPY. THE ROCK STRUCK INJURED ON THE RIGHT HAND RESULTING IN A FRACTURE AND LACERATION.

injured was walking along the belt line when he stepped on a rock and twisted his right knee.

INJURED WAS WALKING TO PUT UP TOOL IN TOOL BOX WHEN HIS FOOT CAUGHT IN SOME SCREEN WIRE LAYING ON THE MINE FLOOR. THIS CAUSED INJURED TO FALL AND BRUISE HIS LEFT LEG.

injured had taken down a scoop charger cable and was was walking with the plug when his left knee gave out. injured fell and twisted his left knee.

INJURED AND CO-WORKER WERE TAKING A CANOPY OFF A 630 SCOOP. THE INJURED EMPLOYEE WAS UNDER THE SUSPENDED CANOPY. WHEN THE SCOOP WAS MOVED, THE CANOPY BECAME CAUGHT ON THE SCOOP TIRE FENDER. CO-WORKER USED A BAR TO LOOSEN THE CANOPY CAUSING IT TO SHIFT AND STRIKE INJURED IN THE HEAD.

INJURED WAS OPERATING A LOCOMOTIVE. HE STEPPED OFF THE LOCOMOTIVE AND HIS FOOT SLIPPED, CAUSING HIM TO SET DOWN ON THE MINE FLOOR. THIS RESULTED IN A CONTUSION TO HIS HIPS.

RIDING IN MANTRIP TO 7 WEST BEHIND LOCOMOTIVE AND ROCKING MACHINE. STOPPED AT A CROSSCUT 200' INBY MNO #2 BELT. LOCOMOTIVE WAS GOING UP A GRADE AND LOST TRACTION SLIDING BACK DOWN THE GRADE AND INTO THE MANTRIP. INJURED WAS EXITING THE MANTRIP WHEN IT WAS STRUCK.

INJURED WAS PASSENGER IN A PORTAL BUS WHICH WAS STOPPED AT BOTTOM OF A HILL GOING INTO 7 WEST. A LOCOMOTIVE AND ROCK DUSTER GOING IN AHEAD OF PORTAL BUS COULD NOT CLIMB THE HILL. IT STARTED SLIDING BACKWARDS AND INTO THE PORTAL BUS.

INJURED WAS A PASSENGER IN A POTAL BUS WHICH WAS STOPPED AT BOTTOM OF A HILL GOING INTO 7 WEST. THE LOCOMOTIVE AND ROCK DUSTER GOING IN AHEAD OF HIS PORTAL BUS/ THE LOCOMOTIVE COULD NOT CLIMP HILL AND STARTED SLIDING BCKWARD INTO PORTAL BUS INJURED WAS A PASSENGER IN.

INJURED WAS WALKING FIREBOSS ROUND. INJURED STEPPED OVER A PIECE OF OLD BELT STRUCTURE. HIS RIGHT FOOT CAUGHT ON THE STRUCTURE. HIS LEFT FOOT SLIPPED AT THE SAME TIME. THE SLIP CAUSED HIS RIGHT LEG TO TWIST AND BREAK A BONE.

EE'S FOOT SLIPPED WHEN CLIMBING OUT OF OLD POWER CENTER BEING WORKED ON. HE GRABBED A COME-ALONG HANGING AT POWER CENTER. HIS RIGHT THUMB WENT INTO THE HOLLOW BACK OF THE HANDLE AND WAS LACERATED.

INJURED WAS STANDING ON PUMP MOTOR ATTEMPTING TO HANG COME-A-LONGS IN TROLLEY. INJURED FOOT SLIPPED CAUSING HIM TO FALL AND STRIKE A PROTRUDING GREASE FITTING ON THE PUMP MOTOR.

INJURED WAS HELPING PULL SLACK UP IN THE MINER CABLE & WATER HOSE. A WATER HOSE COUPLING CAUGHT ON THE HANGER. THE TENSION CAUSED THE ROLLER CABLE HANGER TO BE PULLED DOWN JUST AS THE INJURED EMPLOYEE WALKED INTO THE ""V"" TO CHECK IT. THE COUPLING STRUCK THE INJURED ON HIS HARD HAT AND PUSHED IT INTO HIS SAFETY GLASSES WHICH RESULTED IN A LACERATION TO HIS RIGHT LOWER EYE LID.

INJURED WAS USING A PRY BAR TO SUPPORT A SPROCKET. THE PRY BAR SLIPPED CAUSING INJURED TO CATCH HIS RIGHT THUMB BETWEEN A STAGELOADER CHAIN LINK AND THE PRY BAR. THIS RESULTED IN A LACERATION REQUIREING 5 STITCHES. INJURED WAS WEARING GLOVES AT THE TIME OF THE ACCIDENT.

INJURED WAS INSTALLING A 8 FOOT RESIN ROOF BOLT. INJURED WAS SPINNING THE ROOF BOLT TO MIX THE RESIN. THE PLATE ON THE ROOF BOLT TURNED. INJURED REPOSITIONED THE PLATE WHILE SPINNING THE BOLT. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FINGER. 6 STICHES WERE REQUIRED.

INJURED WAS RETRIEVING A SHOVEL FROM UNDER A PILE OF ROCK. THE SHOVEL HANDLE BROKE AND STRUCK INJURED ON HIS LEFT FOREARM RESULTING IN LACERATION.

Fall from scaffolds, walkways, platforms

A subcontractor's employee was improperly crossing bracing when he lost his grip and fell. He suffered a fracture in his lower left leg.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was at the bottom of the dump area when he experienced breathing problems. (We do not agree or feel this is a reportable injury/illness. This form is being filed pending further review.)

Over-exertion in lifting objects

INJURED WAS PICKING UP A SHIELD CLEVIS THAT HAD FALLEN OFF A PONTOON WHEN HE FELT A PAIN IN HIS BACK. (INJURED DID NOT REPORT ACCIDENT UNTIL NEXT DAY.)
