Accident type, without injuries

Continuous mining machine had at least 5 bits worn down into the shank with no metal carbide tip present. Ventilation plan not being followed. Fire suppression system not in secure location.

Contracted worker was exiting the mine on a bus driven by mine personnel escort when it collided with a motor hauling pipe traveling into the mine.

injured was reinstalling a guard the had been removed to allow cleaning under the overland belt line. injured lifted the guard up and felt tear in his right shoulder. belt was off and locked out.

While attaching come-a-longs to a chain the hook slipped off hitting the employee on the head requiring stitches.

Employee was loading bad rollers onto the slope belt. Employee slipped and fell while carrying a roller. The roller hit the belt structure and then struck the employee resulting in a contusion to the left knee and shin.

Employee was moving mantrip into a side track and went to throw the switch back to the straight. When throwing the switch, the throw sprung back and struck the employee in the knee. The employee suffered a contusion to the right knee.

Employee was carrying a catwalk platform through a mandoor when the platform fell and landed on the left foot causing a contusion.

Employee was traveling with an MSHA inspector. Employee was attempting to bend a sander bar when the bar slipped causing the employee to slip and fall. The lower left foot was became caught as the injured fell resulting in a broken leg.

Stepped of the mine ride and twisted his ankle. Went to the hospital and determined that it was a sprain. Later found out that some ligaments were torn and required surgery.

Employee was hauling a scoop on a flat car. The flat car struck the motor he was operating causing jarring injuries. The injured suffered a strain to the lower back.
