Accident type, without injuries

An unplanned rock fall occurred in the belt entry #6 xc of the main north #2. The belt was damaged. No injuries occurred. The rock fall was reported to the on site MSHA inspector at the time of occurrence.

The Otis elevator was inoperative for approximately 2 hrs due to the door at the shaft bottom being open. This was reported to on-site MSHA inspector. No entrapment occurred. The doors were closed and normal operations resumed.

ONE LAY IN ONE HOIST ROPE ON THE OTIS ELEVATOR SERPARATED AND BECAME ENTANGLED IN A ROPE GUARD. THIS CAUSED THE ELEVATOR TO STOP BEFORE REACHING THE BOTTOM. NO PERSONS WERE RIDING THE ELEVATOR. ALL ELEVATOR HOIST ROPES WERE REPLACED BY KONE ELEVATOR PERSONNEL. KONE ELEVATOR PERSONNEL PERFORMANCE TESTED THE ELEVATOR AND FOUND IT TO BE OPERATING PROPERLY.

Injured was removing a staple lock fitting staple. Injured was using a hammer and chisel to knock out the staple. A piece of metal from the hammer came off. The metal piece went under injured's safety glasses and struck him on his right cheekbone and right eye.

Caught in, under or between a moving and a stationary object

Injured and co-worker were removing a ""I"" beam from the top of the longwall shield puller. Injured rolled the ""I"" beam from the top of the shield puller and caught his left middle finger between the ""I"" beam and the shield puller frame resulting in a fracture to the middle finger.

Over-exertion in lifting objects

Injured was installing belt conveyor top rollers. He lifted up on the top roller frame to position in under the top belt. He felt pain in his right chest wall. This was from strained ribs.

Struck against stationary object

Injured was walking thru the crosscut toward the slope belt. Injured struck his head against an ""H"" beam in the mine roof. This resulted in a cervical and lumbar strain.

Struck against a moving object

Injured was operating a trailing locomotive for material trip. The lead locomotive stopped to let trailing locomotive get material cars up the grade. Trailing locomtive operator did not notice leading locomotive stopping and ran into the back of the trip.

Struck against stationary object

INJURED WAS IN PROCESS OF MAKING A PREOPERATIONAL INSPECTION OF HER LOCOMOTIVE. WHILE WALKING AROUND THE LOCOMOTIVE CHECKING THE SANDERS SHE STRUCK HER RIGHT ANKLE ON A PIECE OF METAL LAYING ON THE MINE FLOOR RESULTING IN A CONTUSION.

Injured had dismounted from mantrip and was walking to job when his foot slipped off a track tie. This resulted in a pull to his left calf muscle.

Injured was moving a roll of cable. Injured bent over to block the cable roll and felt pain in his back. (Injured did not request any medical treatment until 07/11/07).

Struck by falling object

Injured was removing a 8"" I beam from a flat car. As the beam was being positioned to be taken off the flat car, one end of the I beam fell, striking injured on the top of his right foot. This resuted in a fracture to the metarsal bone in his foot.

Fall to the walkway or working surface

Employee was standing on ladder in bottom of oil change pit removing a hydraulic pump from a locomotive. Pump came loose suddenly striking him and causing him to fall from ladder. This resulted in back strain.

Over-exertion in pulling or pushing objects

Injured was pulling a high pressure hydraulic hose over the stageloader when he felt a pain in his back & leg.

Caught in, under or between a moving and a stationary object

INJURED HAD LIFTED A TOOL BOX LID UP TO GET ROOF DRILL BITS. THE LID FELL, CATCHING INJURED'S RIGHT HAND BETWEEN THE LID AND THE BOX.

Fall to the walkway or working surface

INJURED WAS WALKING THRU THE CUT THRU TO THE SLOPE BELT. INJURED'S FOOT SLIPPED CAUSING HIM TO FALL BACKWARD. THIS RESULTED IN A FRACTURE TO BONE IN THE LEG. (INJURED FINISHED SHIFT AND DID NOT SEEK ANY MEDICAL ATTENTION UNTIL 2/12/07)

injured was using a pair of come-a-longs to pull open the northgate of the sump hopper. Injured felt a pain in his right shoulder.

injured stepped onto longwall shield pontoon. injured's foot slipped off the pontoon and his toe caught the relay bar causing him to trip. this cause injured's left foot to twist resulting in fracture to small bone above the ankle. injured finished shift before seeking medical attention.

Injured was helping take slack out of longwall face chain. The chain flight that was secured to the dogs broke causing the chain to strike injured on his right ankle.

Injured was walking belt slope when he slipped and twisted his foot. (this acident was not reported to mgt until 01/17/07)

INJURED WAS CLIMBING A LADDER TO CLEAN LIGHT FIXTURE IN BATHHOUSE. INJURED FELT PAIN IN HIS LEFT BICEP WHILE CLIMBING.

the injured employee was in the bathhouse changing clothes when another employee engaged in horeseplay. the horeseplay resulted in the in the injured employee being cut on his buttocks. the cut required stitches.

Injured was building new regulator in southwest area of mine. The air pressures sucked injured thru the open regulator causing him to fall and strike his head on the regulator panel. This resulted in a laceration requiring 4 stitches.

Injured was in the belt slope drive house preparing the take a conveyor belt roller down the belt slope. As injured placed the roller end on a stair step in the drive house, he caught his finger between the roller end and the metal step. This resulted in a tip fracture and laceration to the 1st finger on his left hand. Injured was wearing gloves.

A HUNG STAGELOADER CHAIN FLIGHT WAS BEING REMOVED BY CUTTING INTO THE FLIGHT WITH A CUTTING TORCH. WHEN THE FLIGHT WAS CUT INTO A PIECE STRUCK INJURED ON THE LEFT BACK SIDE ON HIS HEAD. INJURED WAS APPARENTLY UNCONSCIOUS FOR APPROX 2 MINUTES. INJURED WAS TRANSPORTED TO HOPSITAL AND RELEASED SAME NIGHT WITH NO INJURIES.
