Caught in, under or between a moving and a stationary object

Injured and coworker were replacing a pump. there was not enough clearance for the pump to slide over the pump well hand rail. The new pump became caught on the hand rail. Injured lifted up the pump. The pump slid across the hand rail catching injured' s left thumb between the pump base and hand rail resulting in a severe laceration. Hospital personnel amputated the thumb.

Caught in, under or between a moving and a stationary object

Injured was jacking a wrecked manbus when the left middle finger was caught between the jack and the coupler of the manbus when the manbus shifted. The finger required surgery to repair the damage to the tip.

Employee was roofbolting. While pushing up on roofbolter the roofbolter came out of pot head striking employee on hand. Employee cut thumb and dislocated finger.

Employee was walking on uneven ground. He slipped and fell twisting his ankle and fracturing a toe.

Employee got foot stuck in the mud. When he leaned forward to pull it out he twisted his right ankle.

While taking down high voltage cable the cable dropped pinning right hand between cable and scoop bucket.

Employee was in squatting position and knee gave out when standing up.

Employee got dirt particles in his right eye causing a cornea ulcer. He had his safety glasses on.

Employee caught finger between mine roof and roof bolt plate causing a laceration.

Employee tripped over a stack of blocks and fractured his knee.

Employee was pulling miner cable for a miner move and strained his lower back.

While raising door with rock bar employee felt a pop in lower back causing a strained back.

While Fire bossing the slope, employee stepped on a rock and fell twisting her right knee.

While moving the feeder wood fell and struck employee on the neck.

Fall down stairs

While walking down stairs employee misjudged the distance of the last step and fell. Employee has an Inguinal Hernia. Medical evidence shows this may be a re-occurrence of a non work related previous injury.

While jacking a flat car employee caught hand between Jack and car.

Employee was scooping for belt move when a loose rock fell and hit his right forearm requiring 8 stitches.

While moving shuttle car employee ran over a large rock causing him to bounce causing a cervical strain and strained shoulder.

While roof bolting employee hit the joystick and the bolts sprung out hitting him on the wrist causing a fractured wrist.

Employee was pulling down fly pad and strained his abdominal wall.

While rock dusting employee slipped and fell, striking his right elbow on a pipe. Employee went to the Doctor on June 18,2012. Employee did not lose any time but was put on restricted duty.

Employee said a timber fell across his back. This is being investigated.

Over-exertion in pulling or pushing objects

Employee was pulling CM cable and felt pain in his right forearm. This did not become a lost time until 5/22/2012. At this time he was taken off work until he completed physical therapy.

Employee strained his left shoulder carrying a 15 HP Pump.

Employee stepped down after hanging tubing into a hole and twisted his ankle.

Employee was helping pulling trollies on LW belt line. When the hose on the monorail got hung up the employee pulled on the hose to unhang it. He felt a pain in his left shoulder area.

While bending over and pulling a cable employee strained his lower back causing a herniated disc.

injured bent over to move the continuous miner cable from the roadway when he felt a strain to his lower back.

injured was pulling on a pump unit when he felt a muscle pull in his lower back.

injured was hanging pipe in red hooks. the hooks bent letting the pipe fall against injured left ankle spraining it.

Employee was cleaning(shoveling), when he slipped and twisted his right knee.

While gathering belongings to load on bus employee tripped on a block covered in rock dust twisting his right knee.

While removing old belt employee strained his lower back. He did not go to the doctor at the time. He went the next day but did not inform safety. He was put on restricted duty but did not miss any days.

Employee was shoveling under the belt and the area he had to shovel had an old air hose in the way. When he pulled on it the hose came free quicker then he anticipated and he fell catching himself with his left hand. He landed on his left hand causing fractures. This originally was a report only.

EE was walking out of plant when he tripped and struck door with his left shoulder. EE wanted to wait a few days to see if the pain would go away but it didn't, EE was taking to seek treatment on 3-26-12. EE has been on light duty since 7/11. EE has had an MRI done on his left shoulder and seeing another doctor about his rotator cuff.

While installing grid coupling cover using a hammer the hammer glanced off striking the employees left middle finger.

Shuttle car cable struck him in the face when he was washing out the reel. He had to have a suture in his lip.

Employee ran into a roof bolt that was hanging down causing him to strain his neck.

Employee cut his hand while putting a clamp on a hose. This required stitches. Employee did not go by ambulance, employee went on his own. This didn't show in the initial report, this is why this has been reported late.

Employee was standing next to where his buddy was using a hammer to knock a link apart and a piece of the pin on the master link flew into his left lower jaw.

The employee was working on a belt line and stepped on a nail that when through his boot.

Employee was walking across Sump Walkway and tripped cutting his knee.

Employee cut his left thumb while removing outer jacket of cable. He was wearing cut resistant gloves.

Employee strained his left shoulder while shoveling wet coal and felt pain. He will be in therapy.

Employee was stepping out of a manbus when he lost his footing and fell. He hit his right forearm on a door frame cutting it. This required stitches.

Employee was skinning cable back to splice leads together and cut his thumb requiring stitches.

A come-along was hooked to the cutter head to hold it in place. The come-along broke and the hook hit the employee on the right forearm.

Struck by flying object

injured was striking metal conveyor chain when a chip flew off striking injured in his left forearm. chip was imbedded in arm and removed by physician.

Fall from machine

Employee was changing a skirt rubber when the conveyor started up. Employee jumped off and landed on his right shoulder. This caused an abdominal wall strain.

EE was lifting a 12"" PVC weight about 20 lbs. EE grabbed it with both hands rapidly lifting it, striking the I-beam above his head. EE was taking by truck to seek medical treatment. EE was giving stitches and x-ray of his left ring finger. X-ray showed brake in lower finger next to joint and nail, ee was released wit no restrictions.
