All incidents in 2006
the 10 west belt burned into due to slippage on the head drive roller. the tailpiece had been bumped during the loading process causing the tail roller to become gobbed. this in turn caused the belt to begin to slip on the head drive roller.
a methane gas ignition has occurred in the 3 main north between spad 10+40 and 11+6. no injuries or damage to equipment was involved. a scoop bucket was being used as a welding platform. the hinge on a door that was to be installed at a latter time was being welded when the ignition occurred.
A METHANE GAS IGNITION OCCURRED ON THE 9 WEST LONGWALL FACE. THE SHEARER WAS MINING ON A HEAD TO TAIL PASS WHEN THE HEADGATE DRUM BITS CONTACTED THE ROCK FLOOR. SPARKS OR FRICTION IGNITED A POCKET OF METHANE GAS WHICH BURNED FOR APPROXIMATELY TWO SECONDS OR LESS.
An unplanned roof fall occurred on the main north #1 beltline at 58 crosscut. There were no injuries or entrapment. The fall was 20' wide x 25' long x 6' high. The fall was above the anchorage zone.
The safety switch on the elevator tripped while making trip to the surface causing the elevator to go into stop mode. One person was trapped on the elevator for approximately 50 minutes. The elevator was brought to the surface and removed from service immediately until repaired.
INJURED EMPLOYEE WAS CHANGING A DRIVE BELT ON #8 WEMCO DRYER. INJURED WAS ATTEMPTING TO ROLL THE DRIVE BELT ONTO THE PULLEY. THE PULLEY WAS BEING TURNED BY HAND. THE PULLY TURNED, CATCHING INJURED LEFT LITTLE FINGER IN A PINCH POINT BETWEEN THE HOUSING AND THE PULLEY RESULTING IN AMPUTATION OF END OF LITLE FINGER.
injured was geting off the elevator at the elevator bottom when he slipped and fell. Injured struck his left hand on the elevator door when he fell. This resulted in a fractured left hand.
INJURED RIGHT FOOT BECAME STUCK IN DEEP MUD. INJURED TRIED TO FREE HIS FOOT AND TWISTED HIS RIGHT ANKLE.
Injured was working on longwall recovery. Injured was recovering a drill steel from a ""z"" drill. As injured turned to lay drill steel onto the pan line, he lost his footing. Injured fell and struck his head against a shield resulting in a neck strain.
injured was walking on a board in a wet area when his foot slipped causing him to fall.
INJURED WAS HELPING HANG HIGH VOLTAGE CABLE. THE CABLE SLIPPED FROM THE HANGERS AS IT WAS BEING RAISED. THE WEIGHT OF THE CABLE PUSHED INJURED RIGHT HAND BACKWARD RESULTING IN A SPRAINED WRIST.
Injured was using a 16# sledge hammer to break rock at a rock fall. injured was swinging sledge hammer and felt pain in his arm. (Injured did not request medical treatment until 04/28/06)
INJURED WAS HELPING TO TAKE CHAIN OFF A SHIELD CARRIER. HIS FOOT SLIPPED OFF THE CARRIER CAUSING HIM TO TWIST HIS RIGHT ANKLE.
injured and co-worker had loaded a batam rock duster into the bucket of a scoop. injured was pulling the rock duster toward the back of the bucket. the rock duster moved suddenly catching his left thumb between the frame of the rock duster and the push ram in the scoop bucket resulting in a broken thumb. injured was wearing gloves.
Injured employee was changing bits in tailgate shearer drum. A rock fell from the roof line at the face and struck employee on his index finger & forefinger of his left hand, resulting in a laceration to both fingers.
Injured stepped down from a flat car into a tire rut in wet uneven ground twisting his left knee.
Injured fell thru a hole in the floor on the gate floor of the dump. Injured did not request medical treatment until 01/24/06 and recieved same day.
INJURED WAS OPERATING THE LONGWALL SHEARER. THE BRETBY FELL, AT THE MIDFACE TRANSITION AREA, FROM THE CABLE TROUGH. INJURED WAS PULLING THE BRETBY BACK IN PLACE WHEN A ROCK WAS THROWN BY THE TAILGATE DRUM STRIKING INJURED ON HIS LEFT HAND.
Injured was unloading 8"" steel pipe from car. Co-workers were pushing pipe to end of car. Injured hand was between the end of the pipe and the end of the car. The pipe trapped injured left hand between the pipe and car resulting in a fracture to the back of the hand.
Inj employee and co-worker were stacking monorail. as injured stacked the monorail his glove became caught on the monorail end. the monorail being stacked pulled his hand between it and another piece of monorail resulting in a fractured finger.
INJURED WAS USING A UTILITY KNIFE TO CUT OFF A TORN PIECE OF CONVEYOR BELT FROM THE SECTION BELT. THE KNIFE SLIPPED AND CUT HIS RIGHT LEG REQUIRING 4 STITCHES.
INJURED WAS SETTING A HYDRAULIC POWERED WATER PUMP WHEN THE HOSE BURST STRIKING HIM ON HIS LEFT SIDE OF FACE AND EAR.
Injured was passing a water hose to co-worker. The hose nozzle struck a metal beam and came on suddenly. The sudden water pressure forced the nozzle backward striking injured in the chin.
injured was using a ratchet, socket and bolt tap to rethread bolt hole. Injured removed gloves to start tap in the hole. His left hand was on the ratchet head holding it on the tap and right hand pushing down on ratchet handle. The tap shank broke causing injured's left hand to move suddenly and strike his left forefnger on the broken tap resulting in a cut requiring stitches.
Injured was using a water hose to wash debris to bucket elevator in dump. Injured stepped on wet coal/rock causing his foot to slip. When foot slipped, injured fell backward casuing the water hose to come up and strike him on the left side of his face. The hose lacerated his left cheek requiring three stitches.
injured and co-worker were using a pipe wrench to free a pump sheave wheel. the sheave wheel suddenly free up causing injured to pinch his right ring finger between the sheaved wheel and the pipe wrench.
INURED WAS WORKING ON BELT LINE. AS INJURED TURNED AROUND TO TURN OFF BELT SWITCH, HIS FOOT SLIPPED CAUSING HIM TO STRIKE HIS LEFT ARM AGAINST THE BELT STRUCTURE.
The injured and co-workers were in the processing of removing conveyor belt from the belt takup. The injured was using a knife to cut the excess belt. The knife sliped sutting his left small finger. The cut required 4 stitches. The injured person was wearing gloves at the time of the accident.
Employee claims numbness, swelling and pain in both hands caused by drilling. Employee was seeing a doctor on his own. He started going to a company authorized doctor on March 2, 2007. He is currently being treated and has not missed any time from work.
