Accident type, without injuries

A pulley on the bottom of the elevator failed resulting in damage to the counterbalance chain. Both the pulley and counterbalance chain have been replaced.

The elevator became inoperative when a processor failed. It was down approximately 7:00pm to approximately 10:40pm.

A ROOF FALL OCCURRED IN THE #10 INTERSECTION, #5 HEADING & EXTENDING OUTBY FOR 60'. THE AREA WAS SUPPORTED WITH 5'TENSION RESIN BOLTS. THE FALL WAS 6'THICK, 20'WIDE & 60'IN LENGTH.

A FALL OCCURRED IN THE #4 HEADING ON THE OUTBY EDGE OF THE INTERSECTION IN THE #8 CROSSCUT. FALL IS 4 1/2 TO 5 FT IN THICKNESS AND MEASURES 20 FT LONG AT ITS WIDEST POINT AND TAPERS DOWN TO 17 FT WIDE.

A ROOF FALL OCCURRED IN THE RECOVERY ENTRIES BETWEEN 8D AND 8P. THE FALL IS IN THE #3 ENTRY BETWEEN THE 3RD AND 4TH CROSSCUT. NO INJURIES RESULTED FROM THE OCCURRENCE.

A processor went bad in the elevator used to transport men from the surface underground. The processor was replaced and the elevator inspected.

THE SCOOP WAS BEING USED TO PULL A PIECE OF BELT TOGETHER AT #148 BREAK ON WEST MAINS. SCOOP BACKED INTO A PILE OF ROCK CAUSING THE BATTERIES TO RAISE UP & TWIST. THIS PINCHED 1 OF THE POWER LEADS AGAINST SCOOP FRAME & GROUNDED THE CABLE. THIS CAUGHT THE CABLE INSULATION ON FIRE. FLAMES PRESENT APPROX. 5 MINS. BREAKER WAS KNOCKED & ROCK DUST USED TO EXTINGUISH FLAMES.

A roof fall occurred in the intersection of the #4 heading at #147 break on the west mains. No injuries resulted from this occurence.

A fall of roof rock occured int he 4 to 5 break located in #41 crosscut. no injuries resulted from this occurence.

A roof fall occurred in the track entry outby the intersection of #27 break on the white oak supply track (empty side). no injuries resulted fromt his occurrence.

A FALL OF ROOF ROCK OCCURRED IN THE #8 ENTRY FROM 8 1/2 TO #9 CROSS CUT ON THE 8 SOUTH SECTION. NO INJURIES RESULTED FROM THIS OCCURRENCE.

Caught in, under or between a moving and a stationary object

Injured was trying to step over a rock to get into the convyor when he slipped. He caught his body weight by placing his left hand on the pan. His left foot hit the plow guide door and closed it on his thumb.

Over-exertion in lifting objects

Working on dozer (D-10). Lifting on lift jack and hurt back.

Fall onto or against objects

Injured stood on a bucket to reach the top of a stopping. Injured lost his balance and fell off the bucket into the rib falling on his rescurer injuring his ribs on his right side.

Struck against stationary object

Injured was walking with his head tilted for clearance when he struck the left side of his hard hat against the bolt. This pushed his head back, jamming his neck.

Fall to the walkway or working surface

Injured was traveling from #3 entry at the dump point to the power center. His left foot slipped out from under him causing him to fall and striking his knee against the mine floor.

Struck by falling object

While attempting to drill a 5' hole a piece of rock broke loose over top of the TRS and fell down hitting the drill pot and canopy. The rock struck employee above the back of his right leg cutting through his rubber boot and cutting his leg. Laceration was about 3/4 inches long.

HAD RAIL CAR WRECKED IN BOOSTER DRIVE AREA AT #21 BREAK. HAD RAIL CAR JACKED UP AND WAS GOING TO PULL CAR OVER WITH 3/4 TON COME ALONGS. HOOKED UP TO A SHACKLE ON A RIB BOLT. WHEN PRESSURE FROM THE COME ALONGS LOOSENED PLATE, A PIECE OF RIB ROLLED OUT & STRUCK EE ON THE OUTSIDE OF THE RT. LEG.

EMPLOYEE WAS HAULING MONO-RAIL ON THE OUTBY END OF A PORTAL BUS. MONORAIL CAUGHT A CHAIN HANGING DOWN FROM THE TOP. THIS CAUSED THE RAIL TO COME IN THE DECK OF THE PORTAL BUS STRIKING THE INJURED IN THE LEFT LEG.

EMPLOYEE WAS REMOVING A COVER FROM THE ROOF BOLTER TO WORK ON THE CABLE REEL. AS HE SLID THE COVER OUT OF THE WAY, THE COVER BEGAN TO SLIP OFF OF THE MACHINE. HE GRABBED THE COVER AS IT WAS SLIDING OFF AND TWISTED HIS BACK. SHORTLY AFTER THE INCIDENT HE BEGAN TO HAVE SPASMS IN HIS BACK AND WAS TRANSPORTED OUTSIDE.

INJURED WAS STANDING ON A RAM CAR TO HANG A PIECE OF CABLE. WHEN HE STEPPED DOWN HE TWISTED HIS ANKLE.

Struck against a moving object

AS EE WAS TRAMMING DOWN THE ENTRY, THE BATTERY HAULER HE WAS OPERATING (#2) BROKE IN HALF, THROWING EE AGAINST THE CANOPY, CAUSING HIS NECK TO HURT.

Struck against stationary object

Injured was walking towards the first set of doors on the bottom to access the elevator. Someone called out to him and he turned and raised his head up and jammed it into the top.

Fall onto or against objects

INJURED WAS CROSSING UNDER THE SECTION MONO-RAIL WHEN HE SLIPPED AND FELL STRIKING HIS RIGHT KNEE ON A TRACK RAIL.

Struck by falling object

INJURED WAS TURNING A TROLLEY POLE ON A TRACK PORTA-BUS WHEN A ROCK 6" X 8" X 18" FELL FROM THE ROOF STRIKING INJURED ON HIS RIGHT SHOULDER.

EMPLOYEE WAS USING CHANNELOCKS TO LOOSEN A NUT ON A BRACKET THAT ANCHORED THE END OF THE TROLLEY WIRE. AS HE PULLED, THE CHANNELOCKS SLIPPED CAUSING HIM TO LOSE HIS BALANCE AND TWIST HIS RT. KNEE.

Injured was walking in the entry when he stepped in a "hole" in the mine floor with his right foot. His knee buckled and he fell down.

EE WAS SITTING ON A BAG OF SAND THAT WAS LYING ON THE MINE FLOOR. WHEN HE STARTED TO GET UP HIS LEFT KNEE LOCKED UP. THIS MADE HIM IMBALANCED AND CAUSED HIM TO FURTHER TWIST HIS KNEE. EE HAS CHRONIC PROBLEM WITH KNEE.

Over-exertion (Not Elsewhere Classified)

EMPLOYEE WAS EXITING A 988 LOADER. WHILE CLIMBING DOWN HE MISSED THE LAST STEP. HE FELT A PUL ON THE LEFT HAND SIDE OF HIS LOWER BACK.

Fall to the walkway or working surface

INJURED WAS WALKING DOWN DIRT ROAD. HE SLIPPED ON SNOW & ICE, FEET SLID & FELL BACKWARDS.

Struck by... (Not Elsewhere Classified)

Injured was cutting a piece of plastic on his cap light prior to the start of his shift. As he pulled the knife, the bladed passed across his finger. The laceration required three stitches.
