Weekly examiner found an unintentional roof fall between the #9 and #10 crosscuts in the #3 entry of the 9F section. The fall measure approximately 60' long by 18' wide by 8' high.
Unintentional roof fall on the 8Q longwall from the face to the tailpiece. The fall measured approximately 85' long by 18' wide by 12' high.

Mine examiner found a roof fall in the North Mains Bleeders between 1 Right and 1 North. The fall was approximately 18' L x 18' W x 6' H.
Mine examiner discovered a roof fall at the #98 break on the White Oak Empty track. The fall was approximately 6' high x 14' long x 14' wide. The fall occured over top of an overcast and affected the ventilation in this area.
Switch lead broken off first floor door cam switch at 3:15PM. No one was trapped. Had no effect on the ventilation of the mine. The elevator was repaired at 4:30PM and back in operation.
Roof fall was discovered by Fireboss in the 8N Bleeders betweeen #43 and #44 break in #1 Entry. Fall was approximately 30Lx18'Wx5'T. Travel way was rerouted and no ventilation was affectted.
Elevator would not operate due to a sticking door switch at 7:24AM and was repaired at 8:10AM. No one was trapped.

Weekly examiner found an unintentional roof fall measuring approximately 18'x50'x8'high in the 8N bleeders in the #20 crosscut between the #1 and #2 entries. No travelways or air passage was impeded.
8 Hlg. elevator had a grounded final stop switch and was inoperative for 45 minutes. There were 5 men on the elevator during this time.
Ice build up in elevator shaft prohibited the elevator from running properly causing it to be inoperative for more than 30 minutes.
Door switch on the elevator was frozen due to freezing outside temperatures. The elevator was down for more than 30 minutes.

Employee was helping make a belt splice when he jerked on the splicing tool and felt pain in the middle of his back. He continue to work, the first day he missed work was 12/14/2010.
Employee was carrying a tool bag weighing approximately 65lbs. when he slipped on a piece of belt that was laying on the mine floor and felt pain in his lower back.
Employee lifted pump and set it on ground. Pump then turned over on ee finger causing injury.

Construction crew was dismantling a fan housing when a piece of duct work approximately 8'x 12' x 8' fell on an employee.

Employee was carrying two metal track ties, weighing approximately 5lbs. apiece, when he lost his grip and tried to catch the ties, injurying his lower back.
Employee slipped and fell on mine floor as he was attempting to enter the deck of a parked scoop.
Employee was exiting vehicle in the mine parking lot, when he stepped on a patch of ice and slipped and fell and fractured his right ankle.
Employee was working in the bottom of the elevator shaft up under the elevator when a huge piece of ice fell off the wall of the shaft from an unknown distance and struck the bottom of the shaft. When the ice landed it shattered and a piece struck the employee on the right side and foot.
Employee was prying a rock loose from the 9E section feeder when he lost his footing and fell on the slate bar fracturing a right rib.

Employee was placing a bottom belt roller in a hanger when the roller fell striking him on top of the foot. The employee worked until 11/29/2010 at which time he was unable to continue work.
EE was using his hands to pull on a section of handrail to move it enough for a come-a-long chain to clear. The handrail broke loose and he fell backwards about 5 to 6 ft, EE completed the shift. No work was schedule for Monday 9/6/10 due to Labor Day Holiday. When EE returned to work on Tuesday 9/7/10 his knee was swollen and was taken to the doc and was put on light duty.
Employee was repairing a mandoor when she reached through the opening and fell through the door injuring her right shoulder. She didn't miss any work due to this incident until 6/23/2010 at which time she stopped working and is under a doctor's care.
Employee was using a pair of channel locks to help other employees pull on a piece of 60" belt. The employee stated he felt a sharp pain in his right forearm. The employee did not report this to management until the following day. He has not missed any work and his first day of restricted duty was 12/7/2010.
EE was tigtening bolts with a cresent spud wrench, the wrench slipped off of the bolt and hit the left side of EE's chin causing a small laceration. EE completed shift and RTW the next day.
Installing a set of stairs, had to trim sheeting and steel with a cutting torch. A piece of slag popped and got behind cutting glasses and into right eye. Completed shift, sought medical attention after work hours, and returned to regular duties 11/21/10.
Employee holding ladder - Worker on ladder dropped piece of metal - struck employee on nose.
Employee was reaching for a timber off of a stack when he slipped and fell striking his chin on the stack of timbers. He received 3 stitches.
