A roof fall occurred on the 8O section at the intersection of #57 break in the #1 heading. The fall is approximately 20 feet wide by 30 feet long and 10 feet high.
A roof fall occurred on mmu-002 while extracting a lift on a pillar section.

A roof fall occurred one break outby the slope bottom near the belt shack.
Brakes on the elevator slipped and allowed the elvevator to drift. The brake coil and shoes were replaced.
A roof fall occurred on the 8O section. The roof fall occurred in the #1 heading just inby the #27 crosscut and extended inby for 60 feet. The fall was 10 to 12 feet high and 22 to 23 feet wide. The area had been bolted with 5 foot install B bolts.
A fall located at #41 crosscut between the #2 to #3 heading was discovered by a fireboss on his weekly walk. The fall was 8 feet wide by 30 feet long and ranged in thickness from 0" to 48". The fall will be breakered off.
Injured was scooping in the #2 entry, rt. rib. The scoop blade was back under the curtain on the rt. side of the scoop. When he backed up, the curtain hooked on the scoop, he was attempting to free the curtain while backing out and caught his hand between the scoop and the rib.
Injured was walking from the track entry over to the belt starter at slope bottom. Injured struck his head against a truss bolt resulting in a stiff neck.
Injured was changing a 3/4 high pressure hose on 108 shield. Injured had gotten under 109 shield and was going to function 108. When the tunnel lock was removed in the area where the injured was working the shield automatically pulled up pinning him between a shield power supply and the pan line.
Injured was bolting when a piece of rock pushed the rock shield in and caught him between the machine and rock shield.
Injured was struck in the head by a clevis pin which broke while trying to extract a continuous miner from a pillar fall. Injured was unconscious for about 30 seconds.
Injured was removing material from a stopped up air pump. He had turned the pump off and stuck his hand down inside of the pump; as he was doing this his knee struck the cut off valve for the air and turned the air on to the pump. This caused the pump piston to move and strike his hand and pinch it between the pump housing and the piston.
Injured was cleaning an area around a belt junction when his shovel got caught between a roller and the belt, twisting his wrist.
Running away from roof fall, employee slipped and fell straining his groin.
Injured struck his head against a crib block that was set over a timber for roof support. The area was about 60 inches high. Injured began missing work as a result of this injury on 9/18/06
Injured was lifting 5 gallon buckets of oil from a flat car when he felt a pain in his chest.
Injured was knocking a roll pin out of a splice link and removed his glasses to wipe the sweat off of his face. When he started striking the splice link with the hammer again he had forgot to put his glasses back on. A piece of metal struck him in the eye.
Injures was changing an oil filter when the wrench he was using slipped causing him to bump his right elbow.
Injured was walking up #2 entry from the face and stepped in a hole twisting his right knee.
Injured was cleaning coalfines in front of shields when he felt pain in his lower back.
Injured stated that he was on a step ladder and fell off injuring his left arm.
EE was stripping wire with a razor knife, knife slipped and cut into upper thigh on right leg. Treatment consisted of 24 stitches.
Injured was going to the section parts car to get parts. When he stepped off of the ride, he stepped in a water hole and twisted his right ankle.
Injured was using a air hose to blow wet coal off of the dryer bed. When he turned on the air he lost his balance causing him to turn his ankle.
Injured was moving a gob switch when a piece of rock 1 foot by 1.5 feet by 1 inch thick struck the top of his right hand resulting in a laceration which required 25 stitches.
Injured was installing monorail bolts. After scaling some small pieces of rock from the top, he began drilling a hole. After the hole was drilled about a foot, a piece of rock appproximately 12"x 12" fell out and struck him between his right index finger and his middle finger. Injured was wearing gloves. He received seven stitches.
Injured was getting out of a shuttle car when his legs split causing him to strain his groin.
Injured was installing a degas packer in a degas hole. The packer was pressurized but continued to leak water. Injured looked into the hole and the end of the packer blew out, striking the injured in the face. (Fractures and lacerations to lower and upper jaw.)
Injured walked up beside of a tramming scoop without letting the operator know that he was there. As he turn to flag the operator his left foot was ran over by the scoop tire.
Injured was tramming a scoop up to pick up some rock dust. When he went around a corner of a block he put his left hand outside of the scoop and in between the hinge point of the scoop just outside the operators deck. As he turned the two halves of the scoop came together catching his finger.
Injured was grinding on a piece of metal when a metal partical flew up and struck the injured in the eye.
Injured was standing in a crosscut when a piece of rock measuring 41"x27"x1" thick fell from between the roof bolts and rib and struck him on the back of the neck.
Injured was helping to load crib blocks when one rolled off of the top of the pile and mashed his left thumb.
Injured was removing a rock from a belt line when the rock slid over and caught his finger between the rock and a piece of belt structure.
Injured was removing a bent bolt from a hole. Injured accidently hit the wrong lever on the roof bolter causing the bolt to spin which struck the injured resulting in a broken bone in his arm.
Injured was cutting a piece of belt with a utility knife. The kniffe passed through the belt faster than what he anticipated and entered his right leg just above the knee. Eight stitches were required.
Injured was cutting a piece of cable out of a belt line. The cable was under tension and when he cut it the cable wrapped around his left index finger and left little finger. This resulted in a cut to his left index finger which required stitches and a fracture to his left little finger.
Injured was knocking a block out of a stopping when a piece of roof rock fell and struck him on the right forearm. This resulted in a laceration requiring 4 stitches.
A trespasser cut a hole in the chain-link fence surrounding the 8 haulage substation compound and entered illegally upon the mine property in an apparent attempt to steal copper ground wire. The trespasser was apparently electrocuted while cutting the wire. A Pinnacle Mine employee discovered the trespasser at approximately 7:00 AM on August 23, 2006.
