Accident type, without injuries

A roof fall occurred in the intersection of the #2 heading at break #23. The fall extended over into the adjacent crosscuts. The fall was 40 feet long and 19 feet wide.

A fall occurred in the #3 entry, #7 break on the 8 West section. The fall length was 35 feet long and 18 feet wide.

A fall occurred at the intersection of #38 break in the #2 heading on 8 haulage extension. The fall was 18 feet by 18 feet and was 4 feet thick in the center.

A cam lever bracket broke off of its mount at the bottom of the elevator shaft. This would not allow the elevator to function properly. It took 44 minutes to repair the bracket.

A fall occurred on the 8 South section at the intersection of the #8 crosscut in the #2 heading. The fall is 18' wide X 50' long X 6-8' thick. The area had been bolted with 5 foot install b bolts and was developed around July of 2002.

A roof fall occured in the #10 intersection of the #4 heading on the 8 West section. The fall was approximately 20 feet wide and 20 feet long and ranged from 0 to 9 feet thick. The area was bolted with 6 foot install b bolts.

The operation of the elevator was interrupted when a drive card failed.

A roof fall occurred in the 8O to 8P recovery entries. The fall was in the number 3 headiing between the number 2 and number 3 crosscuts. This fall will be breakered off and does not effect the ventilation in that area.

A roof fall occured on 8 haulage extension at the #41 break. The fall is 12 feet wide by 24 feet long by 5.5 thick in the middle tapering down on the edges. The fall was originally bolted with 5 foot install be bolts and was mined in Jan. of 2001. No equipment was involved and no one was injured.

A fall occured on the 8 west section in the intersection of the #4 heading, #9 crosscut (spad 13+37). This fall extended over into the #4 to #5 crosscut. No one was injured.

Four different falls were discovered on the 4 North section extending over to the 42 panel. This area of the coal mine is worked out.

A roof has occured on the 8 South section between the #11 break and the #12 break in the #1 heading. The fall is 80 feet long by 18 feet wide and 7 feet high.

A fall occurred on the plow block side of the #1 entry. This fall began at the inby corner at #57 break and extended inby for 30 feet. The fall is appr. 10 feet wide and 5 feet thick.

Absorption of radiations, caustics, toxic and noxious substances

Injured was changing out a set of batteries on his hauler. As he was lowering the lid on the batteries 6 cells grounded out. This caused battery acid and other debris to strike the injured in the face.

Injured was sawing a 3x3 timber when the saw jumped out of the groove cut by the blade and cut his left index finger. Laceration was 2 inches long.

Struck by falling object

Injured had just installed a roof bolt and as he was reaching to remove his wrench a piece of rock measuring 18"x6"x4" thick fell and struck injured on both the right hand and right wrist.

Injured and co-worker were installing a set of double door frames. Frame work was stood up and when they lifted up a leg to install some additional blocking the frame fell and pulled the injured to the ground. Injured fell on top of the frame work and her co-worker fell on top of her.

Injured was sitting on the edge of a supply car and the tire rolled up on his left leg resulting in a fracture.

Struck by falling object

Injured had just arrived on the section and walked up to the roofbolter and took a sandwich out of his dinner bucket. As he was getting ready to take bite of the sandwich when a rock fell out of the top and lacerated his right hand. The cut required 15 stitches.

Injured was getting out of a ram car when he stepped on to some loose rock on the bottom causing him to twist his knee.

Over-exertion (Not Elsewhere Classified)

Injured picked up a bag of pull chains in his right hand and swung the bag over his right shoulder. He felt a pain in his lower back while swinging bag over shoulder. He then fell striking his right wrist on pallet lying on floor.

Over-exertion in lifting objects

Injured was cleaning rocks out of the jackline. He lifted a rock to put in the conveyor when he felt a pull in his right groin. The rock was approximately 2 feet by 2 feet by 18 inchs thick. Injured was on his knees at the time of the accident.

Over-exertion in lifting objects

Injured was rehooking a piece of plastic pipe to his motor to pull outside. When he lifted the pipe up to hook it to his motor he felt a pain in his lower back.

Fall to the walkway or working surface

Injured was crossing behind the belt drive area. When he stepped in mud and strained his left knee.

Caught in, under or between a moving and a stationary object

Injured leaned over to try and locate an oil leak under the scoop. Injured instructed the scoop operator to turn on the scoop. When he did he pulled back on the ejector jack which caught the injured's fingers between the scoop blade and the ejector blade.

Over-exertion in lifting objects

Injured was attempting to lift a rock up on the belt line when he felt something in his back slip.

Injured was walking with a new air compressor and stepped across a ditch as he stepped across his left knee gave out causing him to fall forward and landing on his hands and knees.

Over-exertion in lifting objects

Injured had been hauling spillage in a wheel barrow. He said he had hauled 7 or 8 loads with no pain in his lower back . He had loaded the wheel barrow again and sa he was raising the wheel barrow he felt a pain across his lower back.

Injured was shaking a rock off of the blade of the scoop. He had not properly shut the 2 piece door. The back door swung open allowing the other part of the door to open up striking the steering jack which in turn swung around and struck the injured in the shoulder.

Struck against stationary object

Injured was walking from parking lot to the bath house. As he was disposing a piece of gum, the gum landed on his tennis shoe. Injured tried kicking the chewing gum from the top of his shoe by kicking his foot, injured kicked the concrete base for the thickner fracturing his right big toe.

Fall onto or against objects

Injured stepped down off of a dozer. As he stepped down off dozer arm his foot slipped causing him to fall backwards. As he fell he tried to catch himself with his left hand.

Struck against a moving object

Mantrip that injured was riding in was struck in the rear by another mantrip causing injured to strike his head on the back of the mantrip compartment causing him to strain his neck and back.

Injured was dragging a belt roller out of the way. As he was dragging the roller the hooked end caught a track tie causing injured to twist his back.

While attempting to lift out a bottom roller on the slope belt a roller fell out of a hanger and struck the injured on the top of his left foot.

Injured was standing between the handles of a wheel barrow while she was removing dust from it. As the last bag was removed the wheel barrow tipped over causing her to fall and pull a muscle in the back of her left leg.

Struck against stationary object

Injured was helping to pull a joint of 3/4" air line and ran his head into a cable bolt, bolt was approx. 4" out of the top, area was 60" high.

Struck by falling object

Injured was operating miner. He was mining on the off side after extending the ventilation rod 3 to 4 feet inby the last row of bolts. A slip measuring 9'x9' x1' fell on the curtain side, striking the ventilation rod, which in turn struck the injured in the back and the back of the right leg.

Fall onto or against objects

Injured was changing bits out on the head when he climbed back across the plow motor to get out of the way while the operator bumped the plow. He stepped on the motor, slipped and fell on the motor, landing on his tail bone.

Caught in, under or between a moving and a stationary object

Injured and co-worker had been building cribs in the #3 entry on the head of the longwall. Scoop operator was returning with a load of cribs. Employee was sitting on chain pillar rib just inby #53 break. The operator unknowingly trammed by him and ran over both of his feet. Operator heard injured call out and stopped tramming the scoop.

Injured was picking up a bundle of roof bolt plates. The plates were wet and slick. She lost control of the bundle of plates she tried to catch them before they slid out of her hand. Her wrist was bent backwards.

Injured was crossing over a dollie tongue when his boot got caught on the dollie tongue. He fell forward landing on his knee and catching himself with his hands. The fall caused him to twist or jerk his back .

Injured was riding in a jeep when the trolley pole came off of the wire. When he went to put the pole back on the wire he felt a pain in his back.

Struck by falling object

Injured was walking around the tail drive of the plow and had laid his hand down on the tail motor. A piece of rock fell from the roof striking his right middle finger causing a laceration which required a stitch.

Struck by flying object

Employee was drilling shot holes in the face fault on the 8k plow. A brow broke off and struck the injured on the left shoulder, neck, and his right ankle.

Struck by falling object

Employee was pulling a piece of curtain away from a stopping which dislodged a cinder block. The block fell 5 feet striking the injured on the top of his right foot.

Employee was walking from his car to the bath house and slipped on ice in the parking lot. Injured fell to the ground injurying his lower back.

Struck against stationary object

Employee was crossing under a piece of water line and raised up and struck his head on a piece of ventilation tubing. This caused pain in his neck and back.

Struck by flying object

The injured was positioned at the mouth of the 8K fault heading approximately 3 feet inby the outby rib corner. A catamaran jack was position at 56 crosscut in the #1 entry 1/2 crosscut outby the injured's locaton. The catamaran jack chain was tensioned and a chain link broke and ricochet into the break striking the injured.

Struck by falling object

Injured was in the 2x3 crosscut at #27 break on the 8O section when a piece of roof rock measuring 31"x24"x4" fell and struck the injured on the top of his head.

Injured was walking down an entry when he felt something in his knee pop.

Struck by falling object

A piece of roof rock fell and struck the injured under the right eye. This resulted in a laceration that required 5 stitches.

Employee had installed a leg for a truss bolt. As he was pushing the end of the leg up against the roof with a piece of drill steel in order to secure it against the top, out of the walkway the leg slipped off, striking him across the nose.
