The tracking system for the mine was down do to a software issue. The system was rebooted and the tracking system was inspected and found in working order.

A roof fall occurred at #42 - #43 crosscut on 8 Haulage Extension/#1 Belt
The main service hoist located at the 8 Haulage shaft had a trip lever in the governor broken. The part was replaced and the 8 Haulage hoist was back in operation at 10:08pm 06/25/2017.
Employee felt a pain in left shoulder after bending a roof bolt while trying to install it into the mine roof.
Employee was unloading a bundle of belt stands when the belt stands fell off of the flat car and struck the employee in the right ankle.
Employee was using a slate bar to pry when the slate bar slipped and caused the employee to step backwards onto rock and gob causing injury to left foot.
Employee was using a wrench to connect discharge line when the wrench slipped and the employee struck left elbow against the pump.
Employee was struck in the left hand by a rock when it fell between two longwall shields.
Employee was attempting to turn a water valve on and felt a pain in neck / arm (left). Employee did not report incident until 8/12/2016. Was released to full duty. Then on 8/18/2016 received Dr. slip from family doctor to be off until 8/25/2016 and then released to light duty on 8/25/2016 while awaiting MRI and diagnosis.
Employee was walking down the #2 entry when ee stepped on a broken 3x3 ventilation timber and twisted right ankle and knee.
EMPLOYEE WAS UNLOADING FIBER CRIBS WHEN ONE OF THE FIBER CRIBS SLID AND SMASHED THE EMPLOYEE'S LEFT RING FINGER.
Employee was dismounting a 930 Loader when EE fell from the bottom step and landed on EE's back.
Employee was building a stopping & attempted to raise a cinder block above head and felt a pain in left shoulder.

Employee was standing by tray of roof bolter when a rock fell out of the mine roof, struck the ventilation tubing, and then struck the employee. Employee was released and returned to work on 3/18/2016. Employee sought additional medical attention on 3/21/2016.
EMPLOYEE WAS DRAGGING 100' OF TROLLEY WIRE WITH THE HELP OF THREE OTHER EMPLOYEES WHEN THE TROLLEY WIRE GOT HUNG CAUSING THE EMPLOYEE TO BE PULLED BACKWARDS AND FELT PAIN AND POP IN RIGHT KNEE.
Employee was standing on crib block to reach top of double doors to plaster when the crib employee was standing on rolled out causing the employee to fall and scrape right knee.
While attempting to get into company truck the employee slipped on the running board and fell.
Employee was cleaning belt line using a rake when the rake came into contact with a moving belt and caused the rake to strike the employee in the right side of the employee's face. Initially the CT Scan showed no fractures and employee returned to work on employee's next regular shift & worked until On 2/22/2016 another CT Scan showed a fracture in employee's right jaw.
Employee was walking when EE stepped into a hole and felt pain in EE's right foot.
While using a utility knife to make a splice on a shuttle car, the utility knife slipped off of the cable and struck the employees left index finger causing a laceration.
Employee was struck on right hand by a piece of drill steel while lapping belt onto a flat car. Employee went to Dr. and no fractures were found and EE was released to work full duty. Employee went back to Dr. on 5/5/16 and a fracture was found and employee placed on restricted duty.
Employee was struck in the shoulder by a rock, this caused the employee to fall. The rock came to rest on the employee's left foot
Employees were spraying lime on ponds to settle materials and stopped to adjust linkage on motor used for spraying materials. Employee complained of the fumes from the exhaust and walked away. Another employee saw employee lying on ground and went to check on employee.
