A roof fall occurred in the #5 entry of the green ridge #2 mine.
8O Section #8Break #2 Entry had been mined 05/04/2004 7,663 Feet from 8Q Face. Fall approximately 18' wide, 40' long and 10' high.
An old roof fall was discovered just inby the bottom of the slope. The fall is located in the old #1 heading and extends over in to the #1 to #3 crosscut and appears to have happened several years ago. This area is not traveled on a regular basis.

Injured was drawing a rail together with come-alongs to narrow the gauge on the track. He was between the two rails when one of them broke striking him on top of the foot.

Injured was bolting top in #6 entry when foreign materials fell, falling in behind his glasses and into his eyes. Scratched eye.

Injured was removing belt clamp from #24 belt. The counter weight was suspended and held by 2 sets of 6 ton chain hoist. Chains broke, dropping the counter weight tensioning the belt. This threw the injured upwards into the structure causing injury on left side; compression fracture to the 10th vertabra and he was fitted for a back brace.

Employee was removing bolts from equipment. While removing, he was using an impact gun in one hand and with the other hand was applying pressure to the bolt head. Bolt came free from the employee's grip and cut the knuckle of the ring finger on the left hand, requiring stitches.
