Over-exertion (Not Elsewhere Classified)

Emp allegedly reached with left hand to grasp handle to pull himself into drill cab, felt pain in left shoulder, pain continued down into left side. Upon arising Fri AM noticed bulge above naval area with pain in abdominal area & left shoulder.

Caught in, under or between a moving and a stationary object

Emplyee was helping move a 55 gal. drum from the service truck, when the drum rolled on emplyee's foot. Employee was taken to emergency room and X-ray revealed a hairline fracture.

Struck against a moving object

An employee was driving a water truck to water the lower part of the haul road. He lost control of the truck and ran the truck up on the berm on the drivers side turning the truck over on the passenter side where it came to rest. The truck was inspected and it was found an airline had ruptured during the operation of the truck. The employee was not wearing his seatbelt.

Struck against stationary object

EE had begun walking from his company service vehicle on ground level within the secondary plant of the surface portion of the mine when he landed on a 1"" size rock causing his left knee to twist. He experienced discomfort immediately, went to the company doctor and ultimately was diagnosed with torn cartilage in the knee. He is on light duty until completing surgery.

Struck by rolling or sliding object

Employee was welding a safety plate above a feeder, when a rock slid down surge pile, striking employee in lower back. Employee was taken for medical treatment; no xrays required. Doctor told employee to remain off til Thursday, June 7th, when employee will return to doctor.

Over-exertion in pulling or pushing objects

Hose was very short & stiff, causing employee to be in a straining position when pulling, twisting his lower back.

Chain broke on dewatering unit; he grabbed hose to keep from falling, pulled low back.

On July 31, employee was washing up around the wash plant when he slipped on wet dirt. Went to doctor diagnosed with bruised left knee. On August 7 the employee was walking to the water truck and said his knee gave out. Nothing was obstructing his path. He was taken to the doctor as a follow up from the previous week and was given a prescription, a knee brace, and light duty.

Jarred in pit loader. Did not hurt day of accident; went to Dr. 5/23/2007. Restricted duty.

The ground man was attempting to dislodge rock with a 5' pry bar. When the rock disloged, employee was unable to stop momentum of the bar and struck his left hand pinky finger. Employee was taken to emergency room for x-ray, which revealed a small-angulated fracture on the little finger of his left hand. Employee returned to work without restrictions.

Replacing blowbars in primary impact crusher. EE's were using a sledge hammer in the process, loosening blow bars prior to removal. A piece of the blow bar came loose at it was hit with hammer. The fragment cut through blue jeans into EE's upper right thigh cutting muscle & nicking the artery.

Working on bearings on walking beam changing bearings, a chisel popped out of split housing and struck employee in safety glasses, pushing them into his left eye. Required 2 stitches.

Fall onto or against objects

Employee performing maintenance work (welding) while standing on a base made of aggregate material. Lost balance, attempted to catch himself as he fell, pushing his left hand out against wall. In contacting wall, he fractured a bone in his wrist.
