Employee was trying to loosen a 5-1/2' cone crusher after it had blown a hydraulic line going to the clamping system. The employee was using three, 3-ton come-a-longs to turn the bowl and loosen the crusher. As the employee was tightening one of the come-a-longs, it slipped from the rigging and came back and hit the employee in the right leg in the shin breaking the leg.

The employee was laying on their back in a tight section of the crusher removing a nut off a bolt, preparing to change an 1-1/2 bolt for the curtain liner. As the employee was attempting to get out of the tight location, they raised up their head and hit the top of their head and hit it on the bolt. Resulting in a small laceration approx. 3/4"" in length to the top of their head.

Employee was prying a hinge bolt from the clamping section of a Hazemag crusher.
While driving haul truck loaded with rock, miner engaged manual brake retarder which resulted in locking up rear wheels and put truck in uncontrollable slide. Truck came to stop at end of haul road and ran into a pile of material.

A haul truck driver claimed ee hurt back after removing several ""cantaloupe"" size rocks from the haul road. EE did not inform supervisor until Friday the 11th. An investigation was begun involving safety, risk management and insurance providers to determine cause of injury being work related or not.

Employee was in the process of installing the lid on a VSI. Miners were using a telehandle to lower the VSI lid back into place. The lid was slightly misaligned. Miner used a pry bar to move lid into place. As he began to pry, the lid shifted and dropped into place and pinched the bar causing it to flip up and strike the left side of the miners face.

Employee was tightening bolts on a pin cover plate. The employee felt a pain in his shoulder while tightening the bolts. He did not wish to see a dr at the time. He requested to see a doctor on 1/21/10 and taken on that date. He was told he needed an MRI and to see an orthopedic surgeon. The surgeon informed him on 2/24/10 that he had a torn tendon in his shoulder.

Employee was throwing welding leads over a highwall for work that had to be done on the water line that came from the pit pump. When he threw the welding leads, he felt a pain in his shoulder. He did not wish to see a dr at the time. On 3/30/09 he requested to see a dr. On 4/27/09 he was prescribed four rounds of physcial therapy making this a reportable injury.

Employee was carrying 5' long piece of 8"" c-channel with another employee. Employee's boot laces got caught on a piece of expanded metal, causing him to lose his balance. When the employee fell, he landed on his right wrist and right knee. Employee complained of swelling of the wirst the following Monday (10/6/2008).
Claim reported to me on 7/7/08 - Loading a shot, the discharge hose on the auger (8-10 feet long) fell on back, went for TX on 6/30/08.

Working on bearings on walking beam changing bearings, a chisel popped out of split housing and struck employee in safety glasses, pushing them into his left eye. Required 2 stitches.
Jarred in pit loader. Did not hurt day of accident; went to Dr. 5/23/2007. Restricted duty.

EE STEPPED OVER DITCH, GOT FOOT STUCK IN WET MUDAND WHEN PULLING FOOT FROM MUD, BACK POPPED AND STARTED HURTING. NEXT DAY REPORTED INJURY AND HAD DOCTOR VISIT TO INVESTIGATE INJURY. LOWER BACK DISC TYPE INJURY WITH MAJOR RESTRICTIONS. PRESCRIBED PHYSICAL THERAPY TO TREAT AND RETURN TO WORK ASAP.
