Miner was using a man lift basket to observe feed to cone crusher at a height of 28 feet. The miner was wearing fall protection harness and a retractable lanyard but was not secured/tied off to the man lift basket. The miner was ejected from the man lift basket to the surface below.

Employee suffered from suspected Heat Exhaustion. At some point after exiting the enclosed cab of the excavator, employee fell. Employee was found conscious but disoriented on the ground. After falling, EE struck EE's head on an unknown object(s). Employee doesn't know how the fall occurred.
Crouched down loading lifter holes with anfo. Moving to the left for next hole and right foot slipped, and left knee hit rock lying on ground before putting knee on ground
Employee put hand in pinch point to remove drive gear on grizzly.
Employee struck a swale cut into the haul road while returning to the pit and suffered a sprain to EE's left ribs and middle back.
Employee twisted knee while performing routine groundsman operations. Injury did not become reportable until Sept. 23, 2019.
Employee jumped off man lift instead of using the ladder, sprained right knee.
Employee was descending the steps of the MMU when they lost footing on the bottom step, causing hand to slide down the handrail and them to fall over onto their side. Employee sustained a fractured left shoulder blade and a small cut on the head.
Employee was ascending haul truck and felt a pain in left shoulder. Able to continue normal duties. An MRI was done on 12/13/2019, company received report on 12/18/2019, which made the incident reportable.
Employee was changing screens and a bar dropped in 1/2 bin employee tried to retrieve. Employee fell which resulted in a broken leg.
On 7/23/19 an employee was dismounting a haul truck and twisted their L-knee. Initial X-ray and medical evaluation showed negative findings. An MRI on 9/12/19 showed a hairline fracture and local management later learned on 9/16/19 that employee had been receiving PT since August.
Turning water valve on wash plant and strained shoulder.
Employee was dismounting a volvo track hoe. Employee slipped and struck their face on the track and the back of their head on concrete floor. Resulted in a laceration to the nose requiring 5 stitches, a broken nose, and a contusion to the back of the head.
On 08/19/2019 employee sustained a slightly sprained ankle during normal work maintenance. On 09/27/2019 the employee experience sharp pain in the ankle and reported to EE's supervisor. An x-ray revealed a slight fracture making the incident reportable on 09/27/2019.
Employee received an eye injury when a hydraulic hose fitting came loose/broke and sprayed hydraulic fluid into the eye. EE was wearing safety glasses at the time but they were not preventive. Prescription eye medication was administered.
Using grinder with cutting disc. Disc broke and grinder kicked back cutting left bicep.
