Lime bin stopped up, employee was using a hammer and steel rod to try and get it unplugged when had a pain in left upper arm
This report stems from an incident on 08-10-2016 not reported until later - first doctor visit 09/07/2016. Haul truck backed into the loader. Truck avoiding spillage contacted loader dipper.

EE was washing and cleaning up a primary impactor to paint on 6/5/14. Removed a wedge from side access door, set it on frame to do more cleaning, wedge dislodged falling onto main frame then glancing off onto his foot and ankle. He had pain but continued to work. 6th he came into work with foot swollen and having a hard time walking. Went dr 6/9/14, major contusion, off 3 weeks.

Helping do PM service on a D8R Cat dozer employee was climbing down after shutting the door on the right side of the engine and was stepping from the blade arm step to the ground when his left knee popped that was on the step and cause him to fall to the ground employee went for x rays and show no broke bones but a MRI later showed a torn tendon.

On 8/28/04 at approx. 9:00 AM; two Industrial Machine employees were performing welding repairs on the frame of primary crusher; at this time one of the workers doing repairs inside crusher suddenly collapsed; EMT's were called and transported unconscious employee to Southern TN Medical Center, where employee was pronounced dead. (Investigation pending and on-going)
