A roof fall occurred on the H-2 belt line at #35 crosscut. The fall was approximately three foot high and twelve foot by twelve foot in length located towards the tight side of the belt.

The F-Butt working section had an unplanned roof fall at the face area of the #3 entry, 180' inby station spad #355. The fall continued into crosscut 3-2 and 25' into crosscut 3-4. The fall was above the anchorage and up to 10' above the coal seam.

Roof fall approx 60' inby survey station 501 on 5 Belt. The fall is approx 20' wide x 30' long 8'-10' high.

While on return run discovered roof fall on 2 west main in cross cut 4-5 that measured 20-30 ft. long by 20 ft. wide by 4 ft. high. Federal inspector was traveling with company employee at the time

A roof fall occurred in the last open crosscut 60' inby spad #4874 extending approx 60' crosscut 23 into 3-4. Locally difficult ground condition were attributed to the presence of a drag fold which was disrupted by a sand stone. Area bolted with 12 by 16 cable bolts 6' fully grout rods wire mesh, monster mat. This fall was in the A mains 35' long in #3 entry 20' plus high.

While the maintenance foreman was entering the mine he noticed a fall in the belt line on A main just inside the porthole, he reported to the section and contacted the mine foreman

Roof fall occurred right at the pit mouth at end of canopy where you start underground. Best contributing factors to my knowledge would be years of weathering on roof and lots of rain that week. Fall occurred on intake entry. Still had plenty of air going over fall to ventilate intake travelway.

While pre-shift to the section was in the process, the examiner found a roof fall in #3 entry on the front MBC. area was dangered off and a report was phoned outside of the discovery. Fall was aprox. 30 foot long 25 foot wide and 4 foot thick, fall was located in #3 entry of M butt

I believe a slip feature located above the roof bolt anchorage and not visible to the miners caused the immediate shale roof to fail. Roof fall is about 50' long, 18 1/2' wide and 5' to 8' in height. No men or equipment was involved. Ventilation was not interrupted.

On February 20, 2015, a fatal accident occurred in the #2 slope entry of the A Mains, MMU 001. The roof bolter operator received crushing injuries during the bolting process.

While traveling along C2 drive on his Johnson, the employee struck his left arm on a wiper bar. This resulted in a dislocated elbow and fractured arm.

While loading his scoop with rock dust he felt something pop in his lower back.

Employee was pulling a Johnson buggy into the shop and the tires were muddy. One of the tires picked up the rope that was hooked to the garage door rolling it around the tire. Door was pulled down onto employee's back.

While the employee was running across the coal yard to his hi-lift, he stepped in a hole rolling his right foot breaking both bones in his lower leg.

Rubbed or abraded

While crawling employee twisted his left knee causing a tear in his meniscus went off work 8-21-15 having surgery on 9-21 15.

Miners were loading garbage into a hoisting basket when a piece of guarding fell striking a man walking up the stairs on the back.

EE was drilling bottom for P-pins at the lo drive during a section move when a Johnson personal carrier traveling nearby ran over an obstacle. That point tie rod end on the steering arm broke causing vehicle to veer, striking ee with front of vehicle. Caused contusions & bruising to upper body including torn muscles in both shoulders. Did not get medical treatment until 9/24/15

Over-exertion in wielding or throwing objects

While building a brattice wall employee was using a pole ax to split wedges. He hit the roof causing the ax to hit his right knee with the blunt end causing a contusion. Employee went off work on 5-27-2015.

Fall from machine

As the employee was entering the cab of the D-10 dozer, he slipped and fell off the machine.

While pulling rock off miner employee stepped on uneven bottom twisting knee. Employee worked up until the day of his surgery on 4-28-15

Haulage equipment was tramming back the belt line, stopped to sling up cables and a small piece of rock fell striking the MBC operator in the knee.

While building return wall went to put block on top row & felt pull in his shoulder. Did not miss any work until Aug. 5th. 2015. Then went off to have his shoulder fixed.

While hauling supplies in mine cement block fell out of bucket causing him to run over it.

The man said that his knee had been hurting for a week, when he crawled, or was bending it. He reported this on 2/18/2015. He said that there wasn't any incident where he knows of injuring his knee. His first day away from work was 5/12/15.

While installing mainline structure felt a strain in his lower back while putting stands on ropes. Came to work on 02/02/2015 and worked every day until 03/30/2015 then went off

Area was very muddy. Section was about to start moving to set of rooms outby. Haulage system was being disassembled. Bridge conveyor was removed from RHS and dolly and set on a crib block on the bottom. Mechanic stepped between bridge and RHS when bridge slid toward RHS catching victim's left ankle and fracturing same.

Employee was driving on the haul road into the mine when his truck became stuck. He walked up to the top of the hill to get help. When he walked back down to the truck he bent down to find a place to hook a chain and his feet slipped out from under him because the road was icy. He went to the ER and he has 4 broken ribs.

While walking to his loader, the employee slipped and fell onto his right wrist, which resulted in a fracture.

The man, and a helper were changing a hydraulic cylinder on a roof bolter. They were lifting the cylinder up in the air, and it fell onto his upper leg.

The miner was adding belt to the low structure. The tail was pushed before the miner had his rail out causing the low structure to push towards the rib. The miner was kneeling down and the low structure hit his right knee pushing it and his ankle into the rib.

The man was operating a scoop. the man hit the coal rib with the side of the scoop. A large piece of coal fell from the rib striking the man on his shoulder, and back.

While using a hand help grinder to prep a hitch pin for welding the cut-off wheel broke then caught causing the tool to kick suddenly striking the employee lacerating his forearm. Stiches were required to close the wound.

Repairing L25 yellow mine spray hose, after repairing hose with band pulling excess band material with channel locks with force lacerating left thumb from sharp end.

The man was drilling a hole for a roof bolt. A piece of rock fell onto the drill head. The rock was thrown off the drill head, and struck the man in the face.

While walking around the back of the twin boom to check his cable he hit his head on a bent roof bolt protruding from the roof, causing 7 stitches to his right ear.

As the employee was pulling on a new piece of conveyor belt that had just been threaded through a belt tail he lost his footing on the muddy bottom and as he slipped - strained his left knee. ***Employee did not go off work until 6/10/15***

While bottom dusting EE felt something in his right eye and rubbed eye to try to remove it.

He was putting a piece of angle iron onto another and caught his finger in between, smashing his left ring finger.

On 4/21/2015 two employee's were cleaning up rock with a shovel the one employee was hit by the shovel in the right pointer finger. Resulting in a scratch. On 4/27 2015 employee was admitted to the hospital for an infected finger.

Some rock had fallen on the top of the continuous mining machine while mining. After the miner was backed out of the cut the victim was pushing rock off the miner with his hand. As one of the piece tipped before falling it caught the ring finger of his right hand and lacerating it. It required several stitches.

Rock was stuck between head and pan of miner. Lifted heads to reach in to get rock and it slid pinching arm between rock and pan of miner.

While relaying belt structure for section belt up. Victim attempted to grab a piece of structure and was hit on right hand by another piece that was thrown by another miner.
