A roof fall was discovered in the north main (002) working section during the preshift exam on Monday, July 18 in the last open x-cut of #22 room and extended outby for 200'. The fall was 5' to 8' high and 20' wide. As a result we moved to next room set.
A roof fall was found during the preshift for midnight shift. This fall was 3 x-cuts out by the working face of #1 entry of 3 south belt at station 5073. The area was bolted with 3' resin bolts and no separations were detected during mining. Also no breaks were detected while rebolting immediately adjacent to the fall with 6' resin bolts. Area cribbed and dangered off.
A roof fall was found approximately 55' outby the face of the belt entry in the northeast section during the preshift exam for the dayshift. Water was pressent in the roof and a layer of sandstone was evident in the fallen rock at 3' and 6'.
Roof fall occurred on the #2 main belt. The 42"" full grouted and 48"" full grouted rods failed to hold the questionable roof.
Employee was mining a cut of coal when a horseback fell out causing fatal injury.
Employee moved roof bolter outby face to rebolt in the section. Horse back fell out causing fatal injury.
EMPLOYEE WAS ROOF BOLTING WHEN HIS STEEL GOT STUCK IN THE ROOF. EMPLOYEE STARTED TO DRILL A HOLE RIGHT BESIDE THE OTHER ONE AND HAD HIS HAND ON THE DRILL HEAD. VIBRATION WHILE DRILLING THE OTHER HOLE CAUSED THE STEEL THAT WAS STUCK IN THE ROOF TO COME OUT STRIKING EMPLOYEE ON LITTLE PINKY ON LEFT HAND, CUTTING IT REQUIRING STITCHES.
EE INJURED HIS BACK MOVING A WATER PUMP
Employee was bolting, putting in 6' bolts when while pushing up a bolt it got stuck. While trying harder to push it up, his left shoulder popped, causing pain and swelling
Ee postioned himself along L rib, where came in contact with moving tail of continous miner. Miner tail hit employee causing blunt force trauma.
Employee was going outside to get supplies. He was pulling mantrip into crossuct and front tire hit the rib spinning the steering wheel. Steering ball on the wheel went between the middle and index fingers.
WAS LIFTING FIBERCRIBS WHEN MINER FELT PAIN IN LOWER BACK
EE WAS HOOKING UP SKID TO SCOOP WHEN HE GOT HIS LEFT PINKY FINGER SMASHED/BROKE
MAN WAS MOVING SCOOP, TURNED, SCOOP CAUGHT LEFT HAND BETWEEN RUBBER STOP AND OUTSIDE OF OPERATOR'S COMPARTMENT.
MAN WAS INSTALLING ROOF BOLT IN HIGH SPOT, FELT PAIN IN LOWER BACK. 1ST DAY OF MODIFIED DUTY AND PRESCRIBED MEDICATION WAS 4/26/05.
While changing drive roller on conveyor belt slipped on snow conditions on belt and fell 6-8"" to ground breaking colar bone and a rib.
EE was attempting to kick a low structure rail out from under rear wheel of structure jeep. rail came loose and jeep raw over top of left foot. panic bar was not kicked.
Worker was operating mbc when backing out of cut was to close to rib-was slow at hitting panic bar and pinched his foot between control box and rib.
EE WAS HELPING MECHANIC LIFT A SPEED REDUCER ON M.B.C. AND HURT SELF. LOWER HERNIA REQUIRES SURGERY. ESTIMATED WILL BE OFF WORK 6-8 WEEKS
WAS LIFTING 8"" SEAL BLOCKS AND HANDING TO ANOTHER WORKER WHEN EE FELT BURNING IN BACK.
EE TRIPPED ON A PIECE OF ROOF ROCK AND FELL AGAINST TAIL OF MINER, FRACTURING A RIB. (OFF 1 WEEK - ?)
Ee was tightening tail jack he let go of jack handle latch on jack was not latched and handle came back and struck man on head.
TWO EMPLOYEE'S WERE IN THE PROCESS OF LOCATING BREAKS IN ROOF BY DRILLING 6' TEST HOLES. INJURED WAS DUCK WALKING THROUGH A WET AREA ALONG SIDE THE DRILL STATION OF BOLTER. AS CO-WORKER REPOSITIONED THE BOLTER FOR NEXT HOLE, INJURED TRIED TO POINT TO WHERE HE WANTED THE NEXT HOLE. HE LOST HIS BALANCE GRABBING THE ATRS CANOPY AT THE SAME TIME THE CANOPY CONTACTED THE ROOF.
Injured man was operating continious mining machine when piece of draw rock fell from between roof bolts striking him on right ankle and foot.
While roof bolting employee's right glove got caught between head of roof bolt and drill chuck. This resulted in his right hand-wrist and thumb to spin around causing a sprain.
EMPLOYEE WAS ROOF BOLTING, PUTTING IN 6'RESIN BOLTS WHEN HAVING TO BEND BOLT TO GET INTO ROOF. BOLT TURNED & TWISTED CAUSING ROOF BOLT PLATE TO HIT EE IN THE NOSE CAUSING A CUT.
Employee was roof bolting putting in cable bolts when taking steel out of the roof put his left hand between two steels and the top one let go hitting left middle finger causing a fracture to that finger.
Employee pinched left index finger between drill steels.
While drilling roof hole, hand was pinched between the pot and the canopy of the roof bolter.
While installing a roof bolt, the employee's glove caught on the bolt when spinning. This caused a fracture, dislocation and lacerations on the left hand. Specifically, the third and fourth fingers.
EMPLOYEE WAS BUILDING CRIBS WHEN SOME LOOSE ROCK FELL ON HIS HEAD
The man was loading 72"" posts into a scoop, from a pile on the ground. He moved one of the posts, and the pile shifted. One of the posts struck his middle finger of his right hand.
EMPLOYEE HIT HIS BACK ON A ROOF BOLT PLATE WHILE MOVING ABOUT THE SECTION. THIS CAUSED A LACERATION REQUIRING STITCHES, NO LOST TIME, MEDICAL ONLY.
THE EMPLOYEE WAS REMOVING A DOOR FROM THE SIDE OF THE MINER. THE WEIGHT OF THE DOOR WAS OVERBURDEN. THE EE DROPPED THE DOOR ON HIS RT. RING FINGER, SPLITTING & BRUISING THE TIP OF HIS FINGER, REQUIRING (6) STITCHES.
EMPLOYEE WAS TREATED FOR AN INFECTION UNDER HIS FINGERNAIL DUE TO A MINE RELATED INJURY. ON 4/13/05 HE HIT HIS FINGER WITH A HAMMER, SMASHING HIS FINGERNAIL.
EE WAS MOVING BOLTER CABLE. HIS RIGHT RING FINGER WAS SQUEEZED BETWEEN THE CANOPY POST AND THE CABLE, CUTTING HIS FINGER. EE RECEIVED 4 STITCHES.
EMPLOYEE WAS PULLING LOW BELT STRUCTURE ON SIGHTS. STRAINED BACK; PHYSICIAN GAVE EMPLOYEE PRESCRIPTION.
A small piece of sharp roof slate fell out of roof slate fell out of roof and cut ee L forearm, while roof bolting medical only no lost time.
WHILE EE WAS INSTALLING A ROOF BOLT A PIECE OF ROCK SLID DOWN THE BOLT, PINCHING EE'S FINGERTIP BETWEEN THE ROOF BOLT PLATE AND THE HEAD OF THE BOLT. THIS RESULTED IN AN AVULSION OF THE LEFT INDEX FINGER WHICH REQUIRED SEVERAL SUTURES.
WHILE ROOF BOLTING, A SMALL PIECE (6""X6"") OF ROOF SLATE WITH A SHARP EDGE FELL OUT OF ROOF HITTING EE ON LEFT SIDE OF FACE CAUSING LACERATION. MEDICAL ONLY, NO LOST TIME
Employee got hand pinched between roof bolt plate and roof while installing bolt.
