Mining in #4 entry in M1 butt encountered an uncharted bore hole.

A roof fall occurred near station survey # 7615 that was 19 feet wide and 310 feet long. The mine roof showed a local cutter in the area of the roof fall.

Troubleshooting to see what was broken on leveling arm. Operator raised head and fingers were caught on boom.

While swinging bolter head, employee tripped and fell forward and put right handout to catch their fall and left hand grabbed joystick which dropped bolter head onto their right hand causing 2 fingertip amputations.

The employee was twisting to throw a piece of low belt structure when EE felt a strain in the low back. This resulted in a lost time injury.

EMPLOYEE WAS PICKING UP THE BELT ROPE AND FELT SOMETHING POP IN THEIR KNEE. EE WAS JUST RECENTLY RELEASED FROM KNEE REPLACMENT SURGERY. THAT WAS THE KNEE THAT POPPED.

Wet conditions on an incline surface which caused the employee to twist EE's left knee resulting with a torn meniscus.

Struck by falling object

Employee set a metal skid on an unseen pin that kept the skid off the ground, EE proceeded to torch a drain hole in the skid when the skid teetered off the pin which fell towards EE striking EE's right foot which caused the fracture.

Struck against a moving object

The employee was traveling down the slope into the pit when they apparently lost control going around a turn and the jeep slid into the side of the highwall. This resulted broken ribs, a punctured lung, and a lost time accident.

EE was spotting low structure and strained shoulder, resulting in a lost time accident as of 12/28/2023.

AT THE FACE DRY CONDITIONS. EMPLOYEE WAS LIFTING THE SECTION BELT UP TO SLIDE STRUCTURE IN AND FELT A STRAIN IN EE'S BACK. FULL DUTY UNTIL 10/23/2023.

Employee was assisting in lifting equipment doors when EE felt a strain in EE's back which resulted in a loss time incident.

Equipment operator claimed that they had back pain from operating a rock truck. They sought medical treatment days later and resulted in a lost time accident on 7-31-2023.

Employee was doing repetitive bolting when employee felt pain in right arm. After doctor appointment had surgery for tendon tear

At the face, Belting up. EE was pulling low structure over. While doing that, EE felt a pain in the stomach area. Later to find out EE got a Hernia.

Employee was standing on the discharge side of the pump while making sure it was primed when the discharge hose came apart striking EE in the right leg causing a bruise. This resulted in a lost time injury.

Employee was roof bolting when a piece of loose roof rock fell striking EE's back. This resulted in fractured ribs.

Individual was lifting up on the low belt to add a top slide when the individual felt a strain in low back. This resulted in a lost time injury as of 3/20/2023.

a johnson mantrip with 2 passengers was coming out from 2 section when a johnson mantrip carrying 1 passenger was going into 1 section. the outgoing mantrip came around a turn and was hit in the left front with the incoming mantrip. this caused the passenger to strike EE's right foot causing a right heel fracture

Struck by falling object

While roof bolting a piece of rock fell from the roof striking EE on right ankle causing it to roll over. Resulting in a bruise/strain to the right ankle. Doctor said time off and rest the ankle.

Fall to the walkway or working surface

Our employee is a truck driver. Employee had mud on boots and slipped on the ground and fell while stepping down out of truck. Employee fractured wrist.

While pulling a spreader out of mainline structure, the spreader got caught between rail and belt causing spreader to stop motion and individual's back to twist causing lower back pain.

Pinched their hand between drill head and roof, which resulted in stitches

EMPLOYEE WAS PULLING A ROCK OFF COAL CHAIN WHEN EE FELT DISCOMFORT IN LOWER ABDOMEM.

Employee felt a pop in knee while crawling. Employee received surgery on 6/30/2023

EE was operating a battery powered scoop and was traveling around a corner when EE put hand on the stop block of the scoop. The scoop pivoted going around the corner until the stop block hit against the frame of the scoop crushing EE's finger tips.

The helper snubbed the cable of twin boom as operator was pulling into the number 5 entry. The helper went toward the off-operator side without notifying the operator. As the operator turned the machine into the cut, the back of the bolter swung towards the solid rib pinning the helper against the rib causing a fracture.

a bundle of roof plates fell on their hand while loading the bolter with supplies.

Individual was trying to straighten a top slide when it slid off of the structure and the individual struck hand on the rail. This resulted in a fractured hand.

building air lock while installing door in main wall the air lock collapsed pulling them down and the wall landing on them resulting in a dislocated shoulder.

Ind was rolling mine belt when EE felt a pop in the right shoulder which resulted in a shoulder sprain.

While changing park brake pad on #5 scoop, hydraulic pressure bled off caliper pinching left index and middle fingers between pad and caliper causing laceration to left index and middle finger.

While loading cable bolt plates on bolter, employee pinched left little finger between a bundle of plates and bolter resulting in fracture of left little finger.

Struck by falling object

Trying to set a pound-up jack, when the jack slipped and fell onto the left hand, which smashed the left index fingertip. This resulted in 2 stitches.

While pulling drill steels out of roof, a stuck drill steel became dislodged and struck individual in upper lip resulting in stitches.

Loading cable bolt plates onto the bolter EE struck a belt chain with elbow which caused EE to hit left little finger resulting in 6 stitches.

THE EMPLOYEE WAS INSTALLING A SIX FOOT ROOF BOLT. EE WAS TRYING TO SEAT THE HEAD OF THE BOLT INTO THE CHUCK WHEN EE BUMPED THE ROTATION TO LINE UP THE CHUCK WITH THE BOLT AND EE'S GLOVE GOT CAUGHT BETWEEN THE HEAD OF THE BOLT AND THE CHUCK CAUSING EE'S HAND TO TWIST AND BREAK EE'S LITTLE FINGER.

Employee was loosening a bolt when the wrench broke and from that force caused employee to fall forward into the roof bolter hitting chin, this causing the laceration.

While bolting employee pinched their hand in between their pointer finger and thumb between two bolter steels receiving eleven stitches.

ind was removing drill steels from the roof when EE's steels got stuck and EE tried to use the drill chuck on the bolter to remove the steels when EE's finger got smashed between the steels and the chuck which resulted in eight stitches

While placing a roller in structure, the roller slid and pinched left index finger against the structure causing a laceration needing 7 sutures.

Struck by flying object

Torqueing a flywheel on incline shaker screen at prep plant. work location outdoors. Weather: light rain. Torque wrench slipped out of grip of employee while on flywheel nut while under tension, spinning the wrench with the flywheel, striking employee on left upper cheek. injury result was a laceration with 5 stitches and multiple facial bruising.

Loading bolter with supplies when a bundle of cable bolt plates fell off and smashed finger. Laceration of the left middle finger.

While loading roof bolt plates on helper side of bolter, elbow came in contact with a pack of roof bolt plates already on the bolter causing a small laceration needing 3 stitches.

While recovering room set up, individuals right hand got caught on a cable support hanger resulting in a laceration needing 9 sutures.

Struck by flying object

Jacking up the dolly employee thought the jack was engaged. When letting go of the handle it hit them in the cheek causing 5 stitches and a lost tooth.

A piece of drawrock fell from the rib striking employee on back and right arm resulting in four stitches to right arm

Hand was fractured from a rail being thrown by another miner while belting up.

installing roof bolt with plate, bending bolt over to bolter head when hand slipped and it sprang back causing a laceration on top of right thumb requiring stitches

The employee was drilling a hole for a roof bolt when the drill steel fell out of the roof, bounced off the bolter head and struck the employee in the chin causing a laceration and required sutures

working on a MBC reducer, the guard slid off and struck their right shin resulting in a hematoma and slight tibia fracture. doctor said no restrictions just be careful for a while.

While exiting the bolter employee cut EE's arm on a roof strap that was hanging off of the bolter receiving 6 stitches.

Employee was bending a roof strap that was broken off, back and forth to get it to snap off, sharp end of strap went thru their glove causing a laceration resulting in eight stitches, no lost time.

Tightening a bolt on the joy stick when the boom came down on the wrench in EE's hand causing a laceration in EE's palm

When installing a roof bolt and plate. The plate spun and cut employee's right middle finger resulting in 3 stitches. Doctor said no restrictions and able to return to work.

Employee developed bursitis in knee from repetitive crawling in section that averages 42-44' high and dry conditions resulting a lost time injury.
