Unplanned roof fall in 2nd North Butt Room, #12. The fall is 80' long, 20' wide and 5' high. Inby spad #3119. The place was bolted with 42"" fully grouted resin bolts.
A roof fall occurred in the D-Mains section of the mine in a x-cut between the intake & belt entry. Approx 30' long x 6' high. Area was resupported. Location is 1 x-cut inby station #3701.
A roof fall was discovered in the #5 entry of the R Butt section. It did not impede the normal route of travel to and from the section. Timber was set in various locations in the #5 entry.
Face of #2 entry 25 ft to 30 ft long, 18 ft wide, off high was the size of the fall.
Fall happened in crosscut of #4-#5 in first southeast section seven crosscuts outby face. Fall was 30 ft long, 18 ft wide & 42"" high.
Unplanned roof fall in 6th NE rooms 23 and 24. Spad #2698. The fall is 80' long and 20' wide and 3 1/2' - 4' high. The roof was bolted with 3' fully grouted resin bolts.
Unplanned roof fall in 6th NE butt in Rooms 17, 18, 19 and 200' at spad #2672. The fall is 150' long and 70' wide and 3 1/2' - 4' high. The rooms were bolted with 3' fully grouted resin bolts.
Unplanned roof fall above anchorage zone in the 2nd north belt #3 room. Roof fell at spad #3141. The roof fall is 65' long, 20' wide and 7' high. The entry was bolted with 42"" full grouted resin bolts with 6' full grouted resin bolt spotted for extra roof support.
A roof fall was found on a weekly examination in a worked-out area inby section approx. 1000' from station 3931 about 60' long, 20' wide, 5' - 6' high.
Roof fall in 3 heading of 4 SE Mains at center of 16 break going from crosscut 3-2 and into 2-1. Fall is 18' wide, 80' long, and 12-15' high at highest point.
A roof fall was found on a preshift at approx 9:00 pm. 1 X cut off of the intake travelway. It covered a complete intersection, it was approx 5' high. 1 Xcut inby station #3030. No escapeway or any ventilation was effected.
During normal mining operations, miner cut into an 8"" borehole that wasn't on any mine maps. Water was coming from borehole. No CH4.
During normal weekly examination of return, two roof falls were discovered. Neither of them impeded travel or air movement. They were at the following locations: 1 crosscut outby spad #5525 in K Butt return and in the intersection of spad #8525 in the O Butt return.
Roof fall in #3 entry at 22 break of 5 SE mains. 30' long, 40' wide, and 6' high.
About 10:00 pm afternoon shift mined into an unplanned 6"" drill hole. Overburst of water came out then a small dripper 02ch4. Built a 24-24 Solid concrete piller under hole ater hole was plugged top and bottom.

Roof fall in 5 heading at 16 break of 2 belt 20' long, 25' wide, 5' high.
Roof fall in #4 heading at 40 break of 4 SE Mains 20' long, 25' wide, 4' high.
Unplanned roof fall above anchorage zone in the 5th north east butt #3 #4 room. The roof fall started at spad 2878 and was 120' in length, 5-6' high. The rooms were oblted with 42' fully grouted resin bolts. All entry to the roof fall cribs were built.
Unplanned roof fall above anchorage zone in the 5th nor east butt #5 entry. The fall was about 40' inby spad #2877. 65'l x 20' w. The entery was bolted with 42"" fully grouted resin bolts.
A roof fall occurred in the 2nd east mains, #2 belt. The fall was located approx 75' inby ss 3569 in #3 entry.
Roof fall happened between shifts in the E2-Butt intake about 40' off of the main travelway, it was an intersection. Fall measured 19' wide X 40' long X approx 5' high. Area was cribbed and dangered off. Area was bolted with 42"" & 6' fully grouted bolts.
A roof fall occured in the return of old workings of k butt 1xcut outby spad #6261 at spad #6744. No equipment was in area of the fall.
Unplanned roof fall above anchorage zone in 5th North East Belt #1 Entry & Crosscut 2-1 Entry. 20' x 40' in length. The fall was 55' outby the face of the #1 Entry & inby Spad #2883 25'. The roof was bolted with 42"" fully grouted resin & some 6' fully grouted resin spotted. The anchorage zone was 42"". The roof fall was 6'-7' high.
Roof fall @ 6 break on #2 belt in #5 entry - 30' long - 4' high. No injuries, damages, or impairments.
Unplanned roof fall above anchorage zone in 6th nebutt. Between room #27 & #26 in last x cut inby spad #2730, 50 ft long and 20 ft wide. 4'-41/2' high. Anchorage point was 36"" fully grouted resin bolt.
Unplanned roof fall above Anchorage Zone in 1st North Butt Room #12 outby Spad #2514 and inby Spad #2508. Two cross cuts in length 4'-4 1/2' high. Anchorage Zone was 36"" fully grouted resin bolts.
A roof fall occurred in the return of old workings. No equipment was in the area of the fall.
The crew noticed that their belts went down. Upon investigation they saw that the roof was working and left the area. When they came back they found an intersection containing the lo-belt drive had caved. The area was bolted with 42 inch fully grouted resin bolts. The fall is approximately 20 ft x 60 ft x 4-6 ft high. The area has been cribbed and dangered off.
Roof fall in #3 entry of #4 SE Mains at #41 break in intersection. Fall was 20' long, 20'wide, and 4-10' thick. No injuries. Some structure was damaged.
Unplanned roof fall above qanchorage zone in 1st North Butt Room #11 outyb spad #2513 and Inby spad #2504. Two crosscuts in length, above anchorage zone. Anchorage zone was 36"" fully grouted resin.
Employee's left arm was caught in the tailpiece and dimembered from his self.
Gradual oncoming knee problem from crawling on knee surgery 11-14-2007.
Bolter operators were loading 13' Long metal straps(used for roof support). While turning to load more straps on the bolter the victim's arm hit the corner of the other straps being loaded lacerating the victim's left arm in the elbow area.
Roof fall in #3 heading of 4 left in the 1 E Butt. Fall was 30' wide, 50' long, and 12-15' high. Miner operator was turning crosscut 3-4 when a roof fall occurred. Operator dove under boom for protection, however some rock fell on his right leg.
Knee was twisted when operating a Long Airdox MBC. While loading coal, the belt was shut off and employee was crawling on the low belt while backing out of a cut. Someone started the belt by accident and his knee was injured.
Backing miner out of right X-cut, stopped and was swinging miner over when left tram lever on remote stuck, causing miner to swing downhill. The auger of the miner rolled over EE's leg, causing a break.
Stooped over onto knee's, pushing chain reducer from one pallet to another. Experienced pain in back.
Installing Cat pad pin with hammer when piece of pin came off striking man in left eye.
Injured was attempting to attach sling to dolly, slipped and fell against dolly, striking head on same.
Employee was setting a main tail; the tail was pulled into place and hooked into the rail jacks when a simplex jack slipped and let the main tail kick sideways striking him in the upper arm. He had shoulder pain but continued to work until 1-22-07.
Miner was throwing blocks to a co-worker to build a wall when he felt a pain in hip area. He worked the rest of the shift. On Monday morning, pain was worse and he went to the emergency room.
Roof rock fell out and hit drill steel. This caused the drill steel to bend. The bent drill steel hit employee's left hand on the back side. Returned to work 11-28-07.
Injured man was replacing return roller of beltline. Lifted roller & felt pain in lower back.
Knee was twisted while running a Long Airdox MBC while loading coal.
MBC tramming over roll in bottom causing EE to hit should on roof. Accident was reported 3/26/07 first day missed work 6/27/07.
While operating a Johnson personal carrier, he was hauling a spill board for a belt drive on top of jeep behind operator seat, while tramming the jeep down the belt line. The spill board got caught on an airlock door and swung around and hit him in the back of the neck, causing his neck to break.
While crossing a narrow bridge on his johnson cart, a hoe being carried on the cart caught on the structure of the bridge. The handle of the how flipped striking the victim on his right hand positioned on the cart's steering wheel causing a fracture to the 4th & 5th metacarpal of the right hand.
EE was installing a return belt roller. He lifted roller into place and felt pain in his right shoulder.
While walking bent over in low coal, EE caught his (R) ear on a roof bolt plate. This required 4 stitches, returned to work same day. No lost time, medical only.
Handling low belt structure when thumb got smashed between a rail and bottom structure.
While scaling roof rock down, EE accidently stuck himself in the (R) thigh with pinted end of rock bar. No lost time, medical only, 4 stitches.
While in the process of retrieving belt structure from the last room set up the victim pinched his right pointer finger between the roof and the structure piece as he attempted to load it onto a skid for transport. The resulting laceration required 15 stitches to repair. ""no lost time""
Man was pulling conveyor belt together with a come-along. The handle slipped out of the holder striking him in the face. Man received sitches; no lost time.
The victim was bending a piece of sheet steel measuring 4' x 10"" X 3/16"" which had been placed in a vise. At some point during the operation the piece of steel worked loose and the sprung free from the vise striking the victim lacerating his forhead.
Employee was installing roof bolts with a Long Airdox bolter when the bolter wrench slipped off the bolt head striking his hand.
Victim was crossing the low belt after it had been shut off and locked out. As he was crossing the belt he tripped and fell lacerating his right knee on the belt structure.
While loosening a bolt the wrench slipped causing an abrupt jerk to the victim's left arm after which he had to catch himself from falling.
The victim was holding a steel rod (1.5"" diameter) while it was being struck with a sledge hammer to penetrate a cinder block wall. At some point he was struck with a glancing blow from the hammer when it ricocheted off the steel rod.
Employee was crawling in low coal and turned his head into a conveyor belt hanger on the roof, hitting himself above (L) eye. This resulted in 6 small stitches in (L) eyebrow. Not Serious. No Lost Time.
Employee was removing a high voltage coupler when high voltage wrench slipped off and hit him in mouth. This resulted in 3-stitches inside upper lip. NO LOST TIME not serious.
The victim had lifted a scoop wheel/tire with a cable style come-along. Upon lowering the tire the victim's left ring & little fingers were pinched between the cable and pully on the hook end lacerating both fingers & seriously injuring the left little finger.
Over a period of time, crawling in low coal, EE has developed Miner's Knee. Occupational Ilness. Left knee has to have cartlidge removed/torn medial meniscus.
No incidence of injury. Man's right knee was sore and began to swell from crawling. First day off work 7-9-07.
