Roof fall in the #3,#2,#1 entry at 40 x-cut of the A-butt section. Roof fall was above anchorage and blocking our secondary escape way.

While examining the return, the mine foreman found a roof fall. The fall was at an intersection where spade 2329 is located. The area was bolted with six foot super twist bolts. The fall is approximately six foot high. The men were withdrawn out-by. The area was timbered and danger off.

Cave found on preshift - Fallen material a undected slick & slide seen in roof a small coal rider was also noticed on outby side of cave about 1/2 thick or less. Entry witdth inby cave was 17'6"" wide and outby cave was 18' 6"". The slick & slide would not been seen during morning cycle cave was located in #4 entry of D3A but between last open xcut and next one back.

Roof fall in #3 entry of 3B butt 20 foot inby SS# 16172. The fall is 50 foot in length in #3 entry and continues 20 foot into 3-2 crosscut and is 20 foot in width.

An unplanned roof fall occurred in 1 West Mains break 42 (SS#357) intersection. It was the length of the intersection & was 5' into 4-3 and 15' into 4-5.

An assistant mine foreman found a fall in the return at E-mains station spad 2333. The fall was at an intersection measuring 18' x 55'. The height of the fall was 9'. The area was dangered off and supported.

While employee was performing the preshift of the mine, he came across a roof fall in number 2 entry at spad # 5011 aprox 3 x cuts outby. The section was in the process of moving back away from this area the day before.

While examining the intake, the mine examiner found a cave twenty-five foot outby spad 19893 between four to five entry. The cave measured sixty foot long and six foot high. The cave area was supported with four foot fully grouted bolts. The area was timbered off and abandoned. All work was done according to the approved plan.

While he was helping work on a belt drive he was struck by a man trip on his left hip

While traveling outby in return to get low belt he ran over a rock and roofed out.

EE was bolting when hot lime traveled to the face in the water and EE came in contact with it. This in turn gave him burns on his skin.

While going to restart belts employee tripped on a wire striking right knee on a rock. Continued to work until day of surgery.12-3-2014

While rolling belt up he felt a pain in his left ankle. He had a partial tear of his left Achilles tendon.

Crawling into the MBC and hit knee off the side of it. Got medical attention 10-2-14.

Before installing first row of bolts he examined the roof and saw a loose piece of rock and grabbed his rock bar. While scaling the loose piece a piece of rib/ rock above him came down and he was moving away from it when the piece he was scaling fell and pinched his hand between the rock and the floor.

While operating the hand grinder cut-off wheel to cut the bearing set-ups off, the grinder kicked out of his hands and the grinder fell loose. The grinder came down on top of his left hand cutting through his safety gloves lacerating his left hand.

Due to repetitive crawling, the individual developed an abscess of the left knee.

While crawling in beltline employee hit a hanging roof bolt plate cutting his right ear.

Employee crossed over small supply car tongue and tripped, twisting his knee.

Trouble shooting pump when wire arced causing corneal abrasion

Struck against a moving object

Dropped a pen on the cab floor of front end loader. Kneeling on seat, bent over to pick up pen he felt a pop in his ribs. *Returned to work 8/18/14 fully duty worked all shift. Went to get up 8/19/14 am unable to get out of bed, has not worked since. This is reoccurrence of injury 8/5/14.*

Fall from ladders

While cutting I-beam, the I-beam came loose knocking injured off of ladder and onto the floor.

Bottom conditions turned from dry to wet due to roof water. While getting out of the roof bolter he slipped and twisted his ankle. Resulting in a broken left ankle.

Hit his hand off side of MBC while getting out at the end of the shift. Sought treatment of injury on 6/30/14.

While installing a six foot bolt and plate, the employee pinched his thumb between the roof bolter head and roof bolt plate. This resulted in sutures to the left thumb.

Employee was putting drill steels together when a piece of draw rock fell scaring him. He had his hand on the joy stick and hit the thrust causing drill steels to go thru his hand.

Ind. was mudding a wall with B Bond cement when the cement splashed above his rubber gloves and ran down inside the gloves causing it to burn his arms.

While lifting rails, felt a sharp pain in lower back.

While crawling employee felt something pop in his left knee. Keep on working until 4-7-14 when he went off.

The man was building a cement block wall. The man turned around to get a block from a bundle of block, and part of the wall fell on his lower right leg.

Slipped on wet concrete ramp and twisted his knee not serious however, this will be lost time accident.

Loading structure into scoop bucket smashed thumb between structure and bucket. Became lost time on 3/6/14

He was throwing a piece of belt structure felt something pull in His RT Sholder

Struck by falling object

While bolting, a piece of scale fell and rolled off the canopy striking the employee on the left foot. This resulted in a hair line fracture to the left foot.

Fall to the walkway or working surface

While examining the return, the employee slipped on smooth bottom covered in red boy. This resulted in a labral tear to the left shoulder which will require surgery to repair.

While drilling the roof a kettle pot fell striking employee in right thigh causing a laceration and bruise.

MOBILE OPERATOR TURNING INTO A 3 TO 2 AND WAS PINCHED BETWEEN MOBILE AND RIB

While putting in low belt structure he twisted and felt something pull in his lower back and hips.

While crawling thru a mandoor, hit left knee on frame of door causing pain *did not get medical treatment until 9/17/14*

employee was putting supplies on his bolter, when he turned he felt a sharp pain in his knee

The man and a helper were setting cribs in the belt line. The helper was tossing crib block to him. The man got his finger pinched between two crib blocks when the helper tossed it to him.

While the victim was loading roof straps onto the roof bolter, the straps slid off the machine lacerating his right thigh.

While installing a water pump outby, employee pinched his finger between the pump and a crib block. This resulted in a small laceration of finger tip. Not serious, NO Lost time

He was lifting a starter box with a chain and comealong, when the chain tightened his finger got pinched in it.

While dragging the Miner Cable with the Fork Lift, the Miner Cable came loose and caught his finger between the plug and the propane tank on the fork lift.

While gathering pumps from the mud and water he received burns to both lower legs. He must have hot lime on his coveralls.

A sheet of tin was sliding off roof rack of pickup truck when he went to grab it/it cut his finger

Pinched finger between gear box and frame work of machine.

While crawling past the scoop the employee turned around and hit his head off of the scoop, requiring 3 stitches

The employee was cutting the metal banding off of a bundle of roof mats when one of the bands sprung back lacerating his right forearm. The wound required 7 stitches to close.

While traveling in the belt line on a Johnson stinger, the employee ran into a roof jack which caused a laceration above the left eye. This resulted in seven sutures.

Over-exertion in wielding or throwing objects

While hitting the metal pin with the sledge hammer to remove the pin, a metal chip off of the pin hit him in the upper lip, embedding into his upper lip.

The man was loading crib block into the scoop bucket. The man pinched his finger between the crib block, and the lip of the scoop bucket.

While pulling the low belt tail the employee tried to remove a short rail, when the tail fell off of the blocking pinching his left little finger against a block

Cable bolt steels became stuck in the chuck of the roof bolter. The employee wiggled the steels which were under tension in an attempt to loosen them. Finally the steels sprung free striking the victim in the mouth lacerating his lip which then required several stitches.

While trimming conveyor belt, the knife slipped and lacerated his right thigh, requiring stitches.

While jacking the belt together with a chain come along and belt grab, the come along let loose causing the handle to swing around striking the victim above the right eye. The brow area was lacerated and required five stitches to close the wound.

While throwing cribs under a belt drive his hand got caught between the drive and the crib, breaking his right index finger.

While building a wall, the employee broke the last band holding a pallet of cement blocks together. The blocks fell against a scoop bucket where his right hand was laying, breaking his ring finger

While pulling a piece of rock off of the miner his grip slipped and cut his left forearm requiring stitches.

Scoop bucket slid off drive while applying down pressure causing the scoop to bounce, which slammed the victim's head against the bottom of the canopy.

while unhooking supply cars the employee got his finger pinched between the tongue and car frame. this resulted in a small laceration. not serious, no lost time, returned to work same day

While getting dressed he sat on a chain of a basket, and while pulling his first boot on he must have leaned back and unhooked the chain. Then when he leaned ahead to pull the other boot on, the chain he sat on became free dropping the empty basket down hitting him on the top of his head. He received 10 stitches.

Ind was getting a hacksaw blade out of the pack when the blade slipped and lacerated his thumb.

Employee was going thru a mandoor. He caught corner edge of door with his right eyebrow. Causing laceration.

While plowing snow with the IT-28G, the bucket caught a grate throwing the victim forward, and he hit the steering wheel with his face, result in stitches to his upper lip.

He was installing a 6' roof bolt. He was trying to bend it in the hole. The bolt spun and he drove his shoulder into the bolter.
