Accident type, without injuries

Mine examiner encountered a roof fall on B1 Butt belt line, upon further examination the fall extended into the return of C Main. The area was bolted with 4' resin bolts with 6 12' cable bolts in the intersections.

Unplanned roof fall in the B-Mains at 1 crosscut Inby spad #434. Fall occurred right above anchorage point, which was bolted with 6 FT. fully grouted resin bolts. At the time of the fall, due to being a weekend, the mine was idle.

Found a roof fall in the 1st west mains section at 2:55 PM in the #2 entry 8 X cuts outby section. Fall measured approx. 19' wide - 80' to 100' long 10' high. Area was bolted with 4' bolts.

Over-exertion in lifting objects

Employee strained lower back while loading conveyor structure into a scoop bucket.

Over-exertion in wielding or throwing objects

While changing dolly wheels on an MBC EE felt something in the right shoulder. While swinging a sledge. Had surgery on 12/16/16 to repair shoulder.

While crossing over the low belt ee twisted left knee when ee hit the mud on the other side, causing medial and lateral tears.

Struck by falling object

A slicken-sided piece of rock 78"" long by 20"" wide by 0-11"" thick fell from the mine roof in the #2 entry of the E-mains 001 working section. This rock struck employee on the left hand causing a compound fracture to the employee's fifth finger (little). The area was bolted according to the roof control plan with 6' fully grouted resin bolts and oversized bearing plates.

Employee was just walking when slipped on a slippery spot and fell into a shallow hole.

Over-exertion in pulling or pushing objects

While pulling on the miner cable EE twisted right knee. EE worked up until 6/20/16 when EE had it operated on.

After cut was complete the miner backed out and employee took extendable curtain down and put it on the miner. The canvas fell off of miner, the employee went to put it back on the machine and the miner backed up onto toes.

Employee stepped down off the excavator onto a frozen chuck of mud which EE did not see, twisting EE's ankle

Struck by flying object

The employee was fueling EE's loader at the end of the shift. It was very windy and dust was blowing off the loader. EE apparently had some of the dirt blown into EE's eye. The dirt and rust particles were removed by an eye doctor. There was no permanent damage to EE's eye.

Victim crawled under miner tail and when EE thought EE was clear EE started the miner to swing and raise the tail EE hit the wrong function and set the tail down on EE's leg.

Fall from machine

Employee was climbing ladder onto equipment. EE lost EE's handle and fell to the ground.

Drill steel got caught under drill head, when lowering the head it caused the steel to recoil out of tray and struck EE in the bicep.

Fall to the walkway or working surface

The loader operator was getting off the CAT 992G loader at the end of the shift. EE stepped off the counter weight of the machine with EE's right foot onto a level berm. EE stepped onto a piece of #3 stone causing EE's ankle to roll and EE fell to the ground. EE was diagnosed with a sprained ankle.

Employee was bending bolts by the kitchen of the twin boom when a piece of rock 22"" x 25"" x 9"" fell onto EE's lower leg.

The underground miner notice a piece of rock hanging out by the face, employee went to get a rock bar and heard a pop. Employee went to spin away and the rock hit foot.

Pushing roof bolt up to roof with roof bolter, roof bolt slipped off head catching thumb and over bending wrist back

Was operating a lathe when a curl of steel cut through glove lacerating finger on right hand requiring 4 stitches.

Individual was taking a pump off of a pallet. when the pump went to fall off the pallet the individual tried to prevent the pump from falling off the pallet which in turn fell on hand and fracturing middle and index finger

While washing a scoop with the pressure washer something flew into left eye. Thus causing a large corneal abrasion.

Moving a 6 foot ladder when a 10lb. ratchet that was on the ladder fell and struck ee above the left eye.

While removing a belt trainer from the dolly the employee smashed left ring finger when EE put it in the tray of the bridge. This action broke the bone on the end of this finger. there was no missed days.

Employee hit self in the mouth with a drill steel as employee was removing it from the hole.

Employee was crawling on the bottom turned head and caught left ear on the end of a roof mat.

Fall to the walkway or working surface

While putting main line in EE slipped and fell striking EE's right elbow on the ground. EE's elbow swelled up and EE went to hospital. EE received shot.
