There was a roof fall above anchorage at the face area of C4butt section. The fall likely occurred due to localized condition in this area and appeared to be made up of layers of laminated shale. The fall was approximately 135 feet long by 18 to 20 feet wide and 4-8 feet above the roof line.

The roof fall occurred at station survey # 17127 in # three entry of M-6 butt. Forty feet long, nineteen feet wide and approximately ten to twelve feet high. The mine roof had a visual defect (mud slip) at the area of the fall.
There was a roof fall above anchorage point in the belt line of 22E at wall 3 to 4. There was one geological feature present in the area. The length of the fall was approximately 80 feet by 20 feet. It was around 10 feet above normal roof.
Hit uncharted borehole in MMU 003 section at the face of number four entry.
THE EMPLOYEE WAS AT THE FACE CRAWLING IN THE MUD WHEN EEE TWISTED AND FELT A PAIN IN EE'S KNEE. THE EMPLOYEE WORKED UNTIL 3/22/2023 BEFORE SURGERY FOR A TORN MENISCUS.
Employee was attempting to locate frozen spot on 10"" pipe while on a ladder. The pipe broke and struck EE off the ladder resulting in a broken foot and wrist.
While roofbolting,the employee knelt down on a rock twisting right knee, employee worked full duty until 02/07/2023 making this a lost time accident.
The H 10 section was moving to a new section, H 11, employee was helping set low drive for new section. While walking stepped in hole twisting left ankle resulting in lost time.
While kneeling down to pick up resin from supply car employee felt a pop in left knee, resulting in surgery to repair torn meniscus on 12/15/2022, making this a lost time accident.
EE was transporting supplies when the scoop drove over a rock that was not seen causing the scoop to lift up towards the roof which EE's head came in contact with the roof, jarring EE's neck. The employee worked until 11/22/2022 making it a lost time injury.
Employee went to get up from being on knees, felt pain in right knee, pain got worse as shift progressed. Employee worked up to Dec. 16th resulting in surgery to right knee meniscus.
DRILL STEEL FELL FROM THE ROOF STRIKING LEFT INDEX FINGER WHICH WAS RESTING ON THE DRILL HEAD. RESULTING IN STITCHES
tramming the bolter out of #2 entry it trammed into a hole in the bottom and swung and bumped EE's arm resulting in a cracked rib.
EE and another employee were throwing belt structure for belt up. EE's right hand got between two bottom rails causing a fracture of the third metacarpal resulting in lost time.
Individual was unbending a 6' bolt and felt a pop and pain in left shoulder. Individual worked till 11/07/2022 to get surgery on left shoulder.
Employee was cleaning rock off of the miner. When they slid one off it hit their leg causing a laceration. They received 11 stitches.
While moving a piece of low structure employee twisted back causing pain in lower back and back of legs, resulting in a lost time accident starting 09/02/2022
Employee was moving the section when they slipped on the conveyor belt twisting their right knee which resulted in a meniscus tear.
While roof bolting, the employee said they got dust in their eye. EE rinsed the eye out and had blurred vision. EE then sought medical treatment on 08/27/2022 and was diagnosed with a detached retina. This resulted in a lost time injury.
The individual was in the number 4 entry which was 40"" high and 18-18.5' wide in wet and muddy conditions. EE was bending and throwing out bolts on the helper side of the twin boom when EE turned to go back towards the back of the machine and the operator trammed forward and EE's toes got under the cats of the machine resulting in two fractured toes and sutures.
Employee was throwing crib block, felt pain in lower back, strain. Section height was approx 39"" to 40"". EE was helping to set crib when EE went to throw crib EE twisted back resulting in a lower back injury.
The individual was driving a 988H front end loader around stacker 3 when the tire went in a hole or ditch causing the machine to shift side to side and EE felt a pain in EE's back. The individual worked until 7-18-2022 until seeking treatment resulting in a lost time accident.
While crawling a rock got stuck under the employee's knee pad. This caused a cut on the right knee of the employee which then became infected.
Employee was moving a detached mobile bridge conveyor when they didn't see a belly in the mine floor which caused the back of the machine to raise towards the roof. The employee was sitting higher than the mushroom post and their head struck the roof causing the roof causing them to jar their neck and resulting in shoulder discomfort and lost time accident.
While bending bolts, the employee felt pain in right shoulder. The employee worked full duty until 02/06/2023 making this a lost time accident.
While belting up, EE's right middle finger got pinched between two top stands, resulting in six stitches
Mixing B-Bond wearing knit backed rubber gloves resulting in cement burns.
While installing six foot roof bolt, felt pop in right shoulder after bending bolt with pain resulting in lost time.
While tramming bolter to freshly mined cut to start bolting, rock fell off canopy striking right shoulder, this resulted in a lost time injury.
While turning to close man door, twisted right knee which resulted in a sprain.
Employee was driving underground transportation through a set of metal doors when the doors proceeded to swing shut which came into contact with EE's middle finger that was resting on the framework of the transportation this causing the laceration.
Individual was assisting a move from room to room when the individual was tearing low belt apart and felt a strain in groin. Individual worked full duty until surgery on 12/12/2022
While lifting up waterline to put back together, felt something pop in left shoulder. Individual had shoulder surgery on 02/17/2023, making this a lost time accident.
The employee was laying out low belt structure when they twisted their knee and felt a pop. This resulted in a torn meniscus and a lost time injury beginning on 9/7/2022.
Individual was getting hose from shelf and slipped backwards and put a hand out to catch themselves and broke their wrist.
Changing out a roller on tight side of the belt and twisted right knee. Employee worked up to date of surgery on 6/29/2022.
The rock truck operator had just dumped a load of refuse on the boney dump. As EE descended down the hill, EE realized EE did not have brake pressure. The employee then drove the truck through a sump to bring it to a stop. EE struck EE's head in the cab which resulted in a laceration which required 12 staples. EE was wearing EE's seatbelt.
When setting up to pull belt off EE dropped the belt pulling arm on left foot just behind the steel toe. The doctor's report said nothing broke but told EE to wear a walking boot, ice it and rest it. EE has been sweeping floors and cleaning outside since that date.
Getting out of scoop, twisted, felt pop in lower back. This resulted in restricted duty.
ind was laying out roof bolts alongside of EE's bolter. When EE felt a pop and burning in EE's shoulder. This resulted in a lost time injury.
When traveling outside as a passenger on a 2 man personnel cart, the driver hit a bump and bottomed out. Employee was holding onto the side of the cart with fingers under the cart. When the cart bottomed out it got the fingers resulting in 5 stitches in little finger and broke the tip of the little finger. No work missed, employee is back 2/4/2022.
Employee was roof bolting when a loose rock came down striking EE's right hand resulting in sutures.
When swinging a hammer to knock over a T3 roof channel, EE swung and glanced the hammer off the bolter canopy which caused EE to hit EE's own hand resulting in stitches in the left middle finger. No time missed and no restrictions. Back to work next shift.
While tearing apart low structure finger got caught between rail and bottom stand. Resulting in needing 4 stitches.
#4 entry, wet bottom. While employee was throwing a crib block out of the way, left index finger got pinched between crib block and bolter resulting in 5 stiches.
using a utility knife to cut the shipping band off a lift of channels, they slipped with the knife and cut left forearm resulting in 3 stitches. NO restrictions or limitations. keep clean and return to work.
ind bent a roof bolt over they were using a hack saw to cut the roof bolts off. while cutting the roof bolt they were pulling on the bolt and the bolt broke coming back and strike them on the chin which resulted in 5 stitches
when taking drill steel out of the bolter head a piece of rock fell and struck left index finger resulting in a broken finger. The doctor said tape it up or splint if necessary and return to work with NO restrictions.
Employee was backing up the mobile, and pulled themselves up in the seat, at the same time the mbc went over a hump, pushing their head down. Which caused their chin to hit off the guard above the levers. This resulted in stitches.
While hanging canvas employee cut their hand on a roof bolt plate. Resulting in 8 stitches.
While loading the bolter a bundle of cable bolt plates fell and hit employee on the hand. Resulting in 8 stitches on pinky finger.
While cutting piece of rubber belt the knife came out and struck employee in forearm resulting in 5 stitches.
ind was building a cement block wall when EE went to place EE's next block into place EE has EE's middle finger between the two blocks and smashed EE's finger which resulted in six stitches
Knocking a block out of a wall another block fell out and struck EE on the left hand. Went to doctor and came back with NO restrictions, small fracture on left hand pointer finger, wearing a splint for a couple days. No work missed.
The injured employee was riding on a diesel mantrip facing the out by direction when the operator of the mantrip turned out of a crosscut too hard and accidently came into contact with a bolt car. The part of the bolt car came inside the mantrip and pinched employee's toes between the support post on the mantrip resulting in the fractured toes and sutures.
Employee was loading the bolter with supplies when EE cut EE's arm on a roof strap that was hanging off the side of the bolter. Employee received 8 stitches
Normal working section, EE was cleaning some rock off the miner. A piece of rock EE was taking off the miner flipped off and struck EE's left thumb and caused a laceration which resulted in 6 stitches. NO work missed and NO restrictions from the doctor. Back to work at full duty.
Employee was trying to stand a scoop tire up by hand when employee lost footing hitting face against the rim. Resulting in 3 stitches inside bottom lip and 1 outside.
Individual was installing roof bolts. When EE went to reach for a roof plate, the plate cut through EE's glove and lacerated EE's hand which resulted in stitches.
The operator was backing out the first side of the cut when they were struck by a piece of coal rib that fell out behind the curtain striking them in the lower back. This pushed them into the miner where they struck their face. The incident resulted in a fractured L-2, broken nose and 6 sutures above their nose.
While cleaning up around the pit area employee cut left hand on a piece of sheet metal. Resulting in 6 stitches.
pulling a T3 strap off the bolter EE cut EE's finger and had to get stitches. Came back with NO restriction from the doctor. full duty.
While belting up employee got EE's middle finger pinched between a low structure rail and a low structure stand. Resulting in 4 stitches
Individual was pulling strap off of the roof bolter when it became stuck. Then when it became free it slid across the bolter striking the individual's right arm resulting in a laceration which required sutures.
Individual was starting to drill a hole for a roof bolt when a piece of rock fell and caught EE's hand between the rock and the head of the bolter. This resulted in 3 stitches.
Individuals hand slid off of the handle while using the belt splice scissors. Which caused the handle to strike the employee on the eye brow causing a laceration. This resulted in stitches.
Repetitive crawling causing inflammation of right knee and employee to go on modified duty on 02/9/2023
Individual feeling pain in left shoulder while installing roof bolts, individual sought medical treatment and was diagnosed with bursitis in left shoulder.
Employee has a sore knee from crawling. Has not returned back to work since 5/4/2022
The individual works in a low seam coal mine which has caused bursitis in EE's left knee from repetitive crawling. The employee had the knee drained on 3/22/2022 and has become a lost time injury.
