Fall from machine

Ee was using a Komatsu WA 30 (small bucket loader) to stack & move ""Screen Cloths"". The Screen Cloths are metal & stack began to tilt & fall. Ee was backing up on foot & lost his footing & stumbled backwards. He fell & in doing so injured his lower back & left knee. He also sustained some minor scrapes & cuts.

EE WAS SHOVELING AT THE SECONDARY PLANT OPERATION WHEN HE MADE AN AWKWARD MOVE AND TWISTED HIS BACK. HE BEGAN TO FEEL PAIN IN HIS LOWER BACK. HE CALLED ON A 2-WAY RADIO AND REPORTED THE PROB LEM TO HIS IMMEDIATE SUPERVISOR.

Over-exertion in lifting objects

EMPLOYEE WAS WORKING, HE SAYS HE LIFTED A ""HEAVY BELT"" & HE BEGAN TO FELL PAIN IN HIS BACK.

EMPLOYEE WAS POUNDING A DENT OUT OF ONE OF THE COMPANY PICKUPS WHEN SOMEHOW HE CUT HIS LEFT THUMB. HE ALSO STRAINED HIS NECK.

Ee was holding a ""striker wrench"" while another employee was hitting the wrench with a hammer. Ee moved his hand and the other employee missed the target and hit ee's third finger left/hand.
