Struck by... (Not Elsewhere Classified)

Employee was using a utility knife to cut a piece of conveyor belting. He was in a kneeling position and he was cutting toward his knee. The knife slipped out of position and he cut his thigh.

Over-exertion (Not Elsewhere Classified)

Employee was using a toll called a breaker bar to loose a filter he was removing from piece of equip. He pulled on bar very hard, and strained or began to feel pain in groin area.

Struck by flying object

Employee was using a hammer to remove ""build-up"" from #7 screen. A chip from a rock flew upwards and hit him in the left eye. Employee was wearing his safety glasses.

Fall from ladders

Employee was using th extension part of a ladder to complete a project. The ladder slipped because it has no footings to hold it in place. When it slipped, the employee fell to the floor.
