Fall from machine

Ee was stepping down backwards off the komatsu 650 machine. he had been operating, when he misjudged the last step and fell to the ground hard hitting his right shoulder on the ground.

Employee was trying to remove a nut from C-9 conveyor using a spanner wrench. He realized he was tightening the nut rather than loosening it, so he stopped applying pressure on the handle. When he stopped, the handle flung back and hit him on the right thumb knucle and broke it.
