Employee was working at the secondary plant, Scree #3 middle deck using a cutting torch. The torch cut off, employee bagan to wipe th etip, the torch backfired and caused a burn to their left wrist.
Employee was working with a utility knife fixing a seat cover, the knife slipped from their hand, they tried to catch it and in doing this the blade cut their finger.
Employee was working, trying to remove a ""used shelf"" from a scrap metal unit located in the dumpster at the secondary plant. When the shelf loosened, it moved and their finger got caught.
Employee worked on a loader bucket along with other employees where welding and grinding was required. The claimed when they got home the effect of the welding flash caused their eyes to become irritated to the point they went to the emergency room that night. Note: the employee did not know it was charged to WC until they got the bill.
Customer Truck Driver became engulfed in Stone (DGA) when unloading excess material after being weighed out. Driver at the rear of EE's vehicle when slope of stone pile gave out trapping EE under the pile. EE was removed by Quarry personnel with assistance from Emergency Response Units. EE was evaluated and released from Hospital the same day.

Employee was assigned to ""Quarry Yard Clean-up"" they twisted their R/ankle while walking.
Employee is an Equip Opr./Quarry Truck Driver. EE was operating a Quarry Truck, EE was waiting for EE's truck to be loaded when a rock ""spilled"" or ""fell"" from the Bucket of the WA-900 Komatsu Mach which was loading EE's truck, broke the driver's side window, hit EE on the L/thigh and fell to the floor of the truck EE was operating.
Employee complained of ""lower back pain"" after exiting the loader for a mid morning break.
Employee was helping change screen panels when they lifted one of the panels they began to feel pain in their groin area.

Employee was working on the ""Bucket Wheel Assembly"" which is part of the Wash Plant located in the BBQ Quarry Yard. EE stuck their finger in the hole where a ""bolt"" is supposed to go rather than using the proper tool. EE's finger got caught and cut in the process.
Employee was walking in the primary plant area when they tripped, they did not fall but the awkward step they made to avoid falling caused pain in their right leg groin area,

Employee was working with a ""PRY BAR"" while replacing Screen Cloth at the Secondary Plant. The Pry Bar slipped and employee's L/Pinky finger in the nail area got caught.
Employee was cleaning the inside of a ""CHUTE"" when EE began to feel pain in the left side of EE's back and continuing down EE's L/hip & L/leg.
The bucket on the skid steer loader was stuck in the up position. EE was trying to get the bucket to release and rest on the ground. EE was hitting the machine with a hammer. EE got the bucket to release but was standing a little too close to the machine bucket. When it released, the teeth on the bucket scraped EE's left leg in the shin area.

Employee was exiting the Electrical Room located at BBQ Yard, there is one step to ground level, employee missed the step and fell.
EE was using a ""pry bar"" to loosen screen cloth EE was changing, when EE began to feel pain in lower back.
Employee was cleaning #10 chute. EE noticed a ""clump of dirt"" falling down toward EE's foot. EE made a ""move"" to keep it from hitting EE's foot and in doing so, EE twisted ankle.

Employee was using a torch to cut a section from a steel plate when something blew in right eye. EE reported the irritation to supervisor, ee did not feel it needed medical attention just first aid. Overnight the condition got worse, next day ee R/eye was observed to be red and painful. EE was sent to Concentra for treatment.

EE was holding onto an area where a ""Nut"" was attached with a wrench while another employee was using another tool a ""Power Wrench"" to loosen the nut. The vibration from the ""Power Wrench"" caused ee to loose grip, the wrench ee was holding onto ""slipped"" and the result was a ""Fractured finger"".

Employee was shoveling stone around #7 screen when ee felt pain in back.
Technician was in the process of repairing a broken lift cylinder mounting bracket on a Caterpillar model 777D Truck. As EE was attempting to align or pull together the mount and the frame with the portable hydraulic press in order to weld the broken bracket, the tools on/off button failed causing the c-clamp to pop off striking/lacerating the area above right eye.
Employee was stepping off a portable ladder EE was using to power wash equipment, when EE somehow fell and hurt wrist.

Employee was using a ""portable stair rack"" so he could work on a crusher bowl. When he finished climbing the stair rack he said he felt pain in his right knee.

Employee was working on a piece of steel plate. He and a co-worker were aligning the plate. Employee stuck his finger inside on of the holes, the plate shifted and caught his index finger, left hand.

EE was watering the quarry roads. As he was driving down on of the roads he tried to use the brakes to slow down he says the brakes ""failed"" and he was unable to stop. He managed to turn the wheels towards the embankment was of the quarry and was able to stop the truck that way.
Employee was using a ""hand held"" welding torch to cut ""U bolt"" from the equipment. He failed to turn the torch ""OFF"" while he moved himself to a better position to complete the work. When he reached for the torch, he burned his hand.
EE was working with another employee to remove a ""steel cover plate"" as they were lifting the plate it slipped and hit the EE on the left thigh.

EE and co worker were replacing a ""Jaw Crusher Maganese Bolt"". EE was using a ""Sledge Hammer"" and bar to loosen the bolt, he missed hitting the bar and the hammer motion took him, and the hammer into a steel plate which is part of the jaw. He was still holding on to the hammer handle and his fingers got caught between the plate and handle.
Employee was removing a plate from the Magnet on C-10 Conveyor when the plate ""bounced upwards"" and hit his Left Thumb. Note: RX Prescribed & therapy.

Employee was operating a ""articulate truck"", he was ""dumping a load"" when it. ""jolted"", he said he began to feel pain in his lower back. He stated he had previously injured his back, and was being treated by a chiropractor.
Knocking pins out of a bucket which was attached to a front end loader. A piece of metal broke off and cut his left arm
Employee was on the catwalk located at the c-19 conveyor. He was assisting 2 other employee with the re-spooling of the cable. A section of the conveyor ""dropped down"" the belt went into a ""wave like motion"" causing them to be tossed upward off the belt, and the cables somehow whipped in the air and wrapped around his legs, pulled him upward, then dropped him.
The support cables on c-19 conveyor became twisted. He were working on re-spooling them when a section in the middle of the conveyor frame ""dropped down""due to (4) 3/4"" bolts shearing off. When the section dropped, they were jolted up off the belt due to the ""wave like movement"" on the belt. They they came back on the belt they sustained the injuries
The support cables on c-19 conveyor became twisted. They were working on re-spooling them when a section in the middle of the conveyor frame ""dropped down"". When the section dropped, they who were standing on the belt were jolted upward off the belt due to the ""wave-like motion"" on the belt. When they came back down on the belt, they sustained injuries.
EE was cleaning the tip of his welding torch, by wiping it on the palm of his left hand welding glove; after several tries, the glove caught fire and burned his left hand.
EE was helping to change screen decks at the secondary plant. The handling of the screen deck became ackward and began to tilt, he tried to staighten it up, when it cut his finger.
Employee bent over to disconnect a part from a piece of equipment when he felt pain in his lower back and was unable to straighten himself up. He had this condition and problem approx 2 years ago while he was moving an appliance in his home.
Employee was using a ""Manlift"" to get to the c-9 conveyor. His hand was outside of the cage area. His pinky finger got caught between the manlift and the conveyor crushing it.

Employee was using a hammer to remove ""build-up"" from #7 screen. A chip from a rock flew upwards and hit him in the left eye. Employee was wearing his safety glasses.
Employee was using a utility knife to cut a piece of conveyor belting. He was in a kneeling position and he was cutting toward his knee. The knife slipped out of position and he cut his thigh.
Employee was using a toll called a breaker bar to loose a filter he was removing from piece of equip. He pulled on bar very hard, and strained or began to feel pain in groin area.
Employee was using th extension part of a ladder to complete a project. The ladder slipped because it has no footings to hold it in place. When it slipped, the employee fell to the floor.

Employee was climbing down from his fueling truck when he missed the last step and fell to the ground injuring his left leg. The conditions were wet and muddy and slippery do to the recent rain storms we had in the area.
ee was working with another employee changing a screen. The screen got stuck, ee applied force to get it to move, when the screen moved, his finger was in the way & it got caught between the screen &: the frame.

Employee was trying to remove a nut from C-9 conveyor using a spanner wrench. He realized he was tightening the nut rather than loosening it, so he stopped applying pressure on the handle. When he stopped, the handle flung back and hit him on the right thumb knucle and broke it.
Employee was walking to his work station when he slipped, fell, and twisted his L/ankle.
Ee was stepping down backwards off the komatsu 650 machine. he had been operating, when he misjudged the last step and fell to the ground hard hitting his right shoulder on the ground.

Ee was using a Komatsu WA 30 (small bucket loader) to stack & move ""Screen Cloths"". The Screen Cloths are metal & stack began to tilt & fall. Ee was backing up on foot & lost his footing & stumbled backwards. He fell & in doing so injured his lower back & left knee. He also sustained some minor scrapes & cuts.
EMPLOYEE WAS WORKING, HE SAYS HE LIFTED A ""HEAVY BELT"" & HE BEGAN TO FELL PAIN IN HIS BACK.
Ee was holding a ""striker wrench"" while another employee was hitting the wrench with a hammer. Ee moved his hand and the other employee missed the target and hit ee's third finger left/hand.
EMPLOYEE WAS POUNDING A DENT OUT OF ONE OF THE COMPANY PICKUPS WHEN SOMEHOW HE CUT HIS LEFT THUMB. HE ALSO STRAINED HIS NECK.
EE WAS SHOVELING AT THE SECONDARY PLANT OPERATION WHEN HE MADE AN AWKWARD MOVE AND TWISTED HIS BACK. HE BEGAN TO FEEL PAIN IN HIS LOWER BACK. HE CALLED ON A 2-WAY RADIO AND REPORTED THE PROB LEM TO HIS IMMEDIATE SUPERVISOR.

EMPLOYEE WAS WORKING ON A 650 LOADER. HE WALKED UNDER THE COUNTERWEIGHT AND STRUCK HIS HEAD ON THE UNIT. HE JOLTED HIS NECK WHEN HE BUMPED HIS HEAD.

HE WAS RUSHED TO EMERGENCY WHERE HE WAS DIAGNOSED WITH ACUTE KERATOCONJUNCTIVITIS INBOTH EYES.
EE WAS WORKING AT THE PRIMARY PLANTL. HE AND HIS FELLOW WORKERS HAD TO MOVE THE CRANE OUT OF THE WAY. IN DOING SO THE HOOK EE WAS MOVING GOT CAUGHT ON A GRIZZLY BAR CAUSING IT TO GO UP AND DOW N IN THE AIR. WHEN IT CAME DOWN IT HIT HIM IN THE RIGHT SHOULDER AREA.
