Contact with hot objects or substances

Employee was working at the secondary plant, Scree #3 middle deck using a cutting torch. The torch cut off, employee bagan to wipe th etip, the torch backfired and caused a burn to their left wrist.

Struck by flying object

Employee was working with a utility knife fixing a seat cover, the knife slipped from their hand, they tried to catch it and in doing this the blade cut their finger.

Caught in, under or between a moving and a stationary object

Employee was working, trying to remove a ""used shelf"" from a scrap metal unit located in the dumpster at the secondary plant. When the shelf loosened, it moved and their finger got caught.

Flash burns (welding)

Employee worked on a loader bucket along with other employees where welding and grinding was required. The claimed when they got home the effect of the welding flash caused their eyes to become irritated to the point they went to the emergency room that night. Note: the employee did not know it was charged to WC until they got the bill.

Struck by rolling or sliding object

Customer Truck Driver became engulfed in Stone (DGA) when unloading excess material after being weighed out. Driver at the rear of EE's vehicle when slope of stone pile gave out trapping EE under the pile. EE was removed by Quarry personnel with assistance from Emergency Response Units. EE was evaluated and released from Hospital the same day.
