Employee had their right right hand on the power center and was struck by a cable on their right middle finger during a power move, resulting in an abrasion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was sitting on an I-Beam that was stacked on top of another I-Beam with wooden cribbing between the two I-Beams. Their left hand was down near the space between the 2 I-Beams. The wooden cribbing broke, pinching the tip of their left thumb.

Miner suffered 3rd degree burns to the abdomen when a hose popped off the seal pump in the process of removal. Burns required debridement and an allograft.

Employee was staged in designated safe area for blast, along with 2 other employees. When the blast was initiated, a fly rock approximately 5" x 6" x 2" struck an employee in the right hand index finger and knuckle, landing approximately 30 feet behind EE.

EE noticed the vent tube waving and EE's ears began popping and the door on the #5044 haul truck slung open while EE brought EE's truck to a stop. A planned blast took place and the haul route was affected. There were no injuries observed. EE has had multiple doctor visits to diagnose issues with eye and ear discomfort and to explore if there was any damage present.

A secondary blast was carried out while workers were working in the shaft. The blast was performed deep in the 1000 level (abandoned), which normally limits impact on workers. In this situation, the noise from the blast startled the employees in the shaft and one claimed hearing issues which is being followed up on.

A quarry employee was attempting to clear a plugged feeder using a controlled and planned detonation placed inside the hopper; although EE placed themselves in a protected location, a metal piece ricocheted off the tunnel wall, striking them in the lower right leg, causing a bone fracture.

A panel round was shot out of sequence at the end of shift which caused blast smoke to travel up the 685 ramp exposing the miner in the area to the blast smoke. EE was treated at the medic and released. Later that day EE's symptoms worsened and EE sought treatment at ER resulting in being taken off work.

At 12:44 there was an explosion in the quad shredder. One employee was injured and significant damage the facility occurred. The employee received 3 staples for a laceration on EE's head and was given pain medication and antibiotics for two holes in EE's eardrum

The employee was on the third floor of the preheater tower checking an access door to the orifice. A production crew was preparing to shoot cardox onto the preheater tower and were unaware that the employee was checking an access door to the orifice. When the production crew detonated the cardox, hot air burned the employee.

IE was supervising a planned explosive detonation. When the shot was detonated, a stray piece of rock came through the door of the drill, which had been left open, striking the IE in the back of the legs. This resulted in a distal fracture of the left fibula, and an occult fracture of the right 3rd metatarsal.

EE was disposing wet dynamite caps. The caps were put into a designated area, lit on fire, and EE left the barricaded area. EE returned to the barricaded area, leaned down to relight the fire and at that point the caps detonated. EE was wearing safety goggles but EE still received minor burns to the face and a small fragment of steel in right shoulder.

Rock was ejected from the bottom of the face of a blast, traveled 600', over two material stockpiles, and struck a truck parked behind the stockpiles. The rock shattered upon impact with the upper driver's side door frame. Fragments of the rock entered the open window and struck the driver, driver's seat, and back window of the truck.

Mechanic was on a loader when the mine had a blast and a small rock fell through the wire and hit him in the neck. He finish servicing, not thinking much about the cut and when he got to the shop he reported it and it was decided to take him to the Hospital to have it checked. Resulted in a few stitches.

Miner was fin-hoeing blasted ore from top of the muck pile.He created an opening to the backside of the muck pile releasing residual gases trapped from the blast.The miner breathed these gases,became dizzy and retreated from the muck pile and sat down when he momentarily lost conciousness.A co-worker helped him to the man way.The injured miner was given oxygen and left the stope.

An accident occurred when a shot of 37 holes was detonated in the pit. The employee was properly contained inside a steel blast shelter at the time of the detonation. However, the blast shelter was improperly placed and rolled on its side following the detonation. The employee suffered 3 broken ribs, a collapsed lung, and a broken bone in his neck and on his left ear.
