EE was removing scrap metal from a muddy area when EE felt a pain in EE's back. EE visited the hospital on 2-24-26 and received a full release. EE worked until 2-26-26 when EE followed up with a PCP and was given another full release. EE went back to PCP on 3-5-26 when EE was taken out of full-duty work

EE was lifting a timber and felt a pain in EE's groin area

The employee was flipping track rails over to begin installation, when a finger became caught between two rails, resulting in a laceration to right middle finger.

An Employee was assisting in replacing the cutter motor shield on the right side of the continuous miner when their left thumb was caught between the miner frame and the shield, causing a laceration that required four sutures.

During a section move, employee was conducting housekeeping and picking up hinge pins. When employee was on their way to place the pin on the power center, they accidentally tripped on a hinge pin and got their hands caught between the hinge pins and power center causing injury.

Employee was helping remove a broken torque shaft on the CM machine and as it was being pulled out by another employee the broke end struck EE's left hand resulting in a laceration.

The employee was setting standing support when EE turned EE's back on a standing unwedged timber to grab EE's hammer. The unwedged timber fell, striking EE in the head.

On 8/2/25 employee picked up a small air compressor and felt a pain in groin/stomach area. Employee continued to work and sought medical treatment on 8/8/25 and diagnosed with a potential hernia. EE continues to work and has fulfilled all their duties as a dispatcher. The employee is scheduled to see a specialist for complete diagnosis and applicable treatment.

Employee closed EE's finger in the door of EE's pickup truck.

Employee lifted two full oil cans to load onto a ride and felt a pain in their back.

Employee was rolling up line curtain and contacted a curtain hanger with EE's right middle finger causing a laceration.

Employee opened an airlock door, and it blew shut, catching EE's hand between the doors.

Employee was kneeled down and when EE went to stand up felt a pop in EE's knee.

Employee was loading a bag on rock dust into the scoop and injured EE's right shoulder. The incident occurred on 4/12/25, the employee continued to work until 4/15/25. On 4/16/25 the employee sought medical treatment and written off at this time.

Employee was lifting a piece of metal and lost their footing, causing the metal to fall onto their left thumb and causing a fracture.

Employee strained back at the start of employee's shift as employee was pulling themself up to the cab of the excavator.

Employee was unloading supplies from the scoop bucket and placing them onto the roof bolter. When the employee was picking up some bolts, the bolting machine moved, and employee jumped into the scoop bucket to get out of the way and accidentally got their foot caught between the scoop bucket and the roof bolter.

Employee was positioning a ripper motor when it slid onto the side of EE's foot resulting in a laceration.

Employee slipped and fell hitting their elbow and arm resulting a sprain/sprain on 1/15/25. The employee continued to work normally until they were later taken out of work by a physician on 1/30/25.

Employee was moving a hydraulic cylinder, the cylinder rolled over the employees lower left leg causing a fracture.

Employee was picking up a tool/polyester strap and when they stood up, they felt a pop in their back.

On 9/25/24 employee was lifting a 55-pound bag of flobeads (Dry Cat) when EE felt a sharp pain in their back. Employee continued to work until EE was taken out of work by a physician on 10/22/24.

Employee was pulling a cable by hand, when the cable became entangled as EE pulled causing an injury to the lower back.

Employee was dragging a hold down roller and felt a pain in EE's lower back.

Employee was pushing the roof bolter cable over to hang it and while pushing on the cable the employee's left hand slipped causing the left hand to strike a roof bolt plate resulting in a laceration. The employee was wearing gloves at time of incident.

EE was letting down cable out of a rope hanger that was connected to a spad nail. The spad nail came out of the rib, hitting EE's left hand against a roof plate cutting EE's hand

Employee was dropping a cable for a roof bolter and got mud in left eye.

Employee was carrying a roll of line curtain and felt a pain in EE's lower abdomen.

On 5/21/24 employee was pulling a piece of mining belt by hand when the edge of the belt flipped over hitting them on the left knee. The employee continued to work until they had surgery to their knee on 9/16/24 and is now off work.

The employee was lifting a five gallon can of oil off the supply car and felt a sharp pain in right shoulder.

Employee was carrying a bucket with an electric saw inside of the bucket. The saw started to fall out of the bucket and the employee went to catch the saw and contacted the bar of the saw creating an incision. The saw was not operating nor on at time of contact, the action of grabbing the bar as it was falling caused the injury.

Employee walked into a belt hanger injuring neck and shoulder. Employee was injured on 4/22/2024, but continued to work until 4/29/2024 when the employee sought treatment and was written off work at this time.

Employee was replacing a boom jack pin and pinched the tip of right index finger.

Employee was in the process of repairing a damaged plastic water line and got a finger caught between the water line and a belt chain resulting in a laceration.

Employee was working on a belt move and installing structure. A top cradle accidentally fell over causing employee's finger to become caught between a top cradle bracket and the structure rail causing injury.

Employee was conducting repairs on the bird cage/agitator section of the centrifuge when the bird cage rotated and pinched the employee's fingers resulting in lacerations. The employee received sutures to 2 fingers as a result of the injury.

On 1-23-2024, the employee was pulling a piece of line curtain when EE felt a strain in EE's knee. The employee was transported to Clinch Valley Medical Center where EE was treated and released to return to work EE's next shift.

Employee was replacing a pan pin at which time the employee pinched the tip of the left middle finger while placing the pin in the eye.

Employee was struck with a tire that fell striking EE in the lower right leg.

Employee was installing belt structure and felt a pain in the abdomen. Employee finished the shift and was examined by a physician and released to return to work. The employee was later seen by a physician on 1/16/2024 and written off work until further notice.

The employee inadvertently got the web of EE's left hand caught between two pieces of roof bolt steel when connecting them together.

Employee was carrying a roof strap and felt a pain in their neck and shoulders. Incident occurred on 06/28/2023, employee did not seek treatment until 07/05/2023 and was given days away a that time.

Employee was removing rock from the continuous miner, at which time the rock slipped striking EE's left index finger causing a laceration. Employee was wearing gloves at the time of incident.

Employee was hanging cable, the cable slipped causing a rope hanger hook to strike EE's nose causing a laceration.

Employee was carrying a highwall miner cat pad and placed the pad down quickly landing on EE's index finger.

Employee was loading supplies when they felt a strain in their right shoulder. Employee went to LRMC and was x-rayed and released to return to work the next shift. Employee went for follow up on 12/28/2022 and was ordered to see a orthopedic and was written off work effective 01/03/2023 resulting in days away from work.

While breaking down a feeder for belt move, injured employee caught left thumb between crib block and roof bolt, resulting in laceration. Injury was not due to a training or compliance issue.

Bundle of plates fell from the roof bolter striking the back of employee right leg.

Employee was handling roof bolt plates when employee's left little finger was caught between another stack of plates causing a laceration.

The employee was hanging line curtain in the number two entry, and as employee was lifting the curtain felt a strain in groin and back. The initial incident happened on 9/11/22 and the employee sought treatment on 9/19/22 and was taken off work by employee's physician on 9/26/22.
