Employee lifted two full oil cans to load onto a ride and felt a pain in their back.

Employee was standing at the rear of a roof bolting machine when the bumper was lowered onto their boot.

Struck by falling object

Employee was operating a Continuous Miner in the No. 8 Right, when a piece of rock 9'x3'x6"" fell from between the outside roof bolt and mine rib striking their right leg.

Employee was pulling a cable by hand, when the cable became entangled as EE pulled causing an injury to the lower back.

Employee was putting EE's tools away off of EE's ride, when the ride went into forward tram, forcing EE's leg into EE's toolbox.

Employee was cutting belt and the utility knife slipped out causing an incision to the right leg.

An unplanned roof fall occurred on the No. 2 Section 0030/0040 MMU in the No. 7 entry located at spad no. 3684.

Struck against stationary object

Employee walked into a belt hanger injuring neck and shoulder. Employee was injured on 4/22/2024, but continued to work until 4/29/2024 when the employee sought treatment and was written off work at this time.

Struck by falling object

Employee was struck with a piece of draw rock on the right shoulder and right arm causing a laceration to the right forearm.

Over-exertion in lifting objects

Employee was installing belt structure and felt a pain in the abdomen. Employee finished the shift and was examined by a physician and released to return to work. The employee was later seen by a physician on 1/16/2024 and written off work until further notice.

Caught in, under or between a moving and a stationary object

Employee was prying with a slate bar and the bar slipped pinching employee's left index finger causing a laceration and a fracture. Employee was wearing gloves at time off accident.

Employee was installing ventilation curtain and tripped and fell injuring right knee in the process.

Employee was walking in the No.4 to No.5 crosscut and tripped over the C/M cable and fell injuring left and right knees.

Struck against stationary object

Employee was tramming a scoop down No. 3 entry on the 0030/0040 section and ran over a block, causing the scoop to bounce striking the employee against the canopy.

Fall from machine

As employee was conducting pre-op check on scoop, EE climbed up on top of scoop to move material from top of scoop and stated EE lost footing and fell to the ground. The employee completed shift and did not seek medical treatment & continued to work, without any complaints, until 4/19/23. On that date, EE went to doctor & was written off work until further notice.

Struck by falling object

Bundle of plates fell from the roof bolter striking the back of employee right leg.

Struck against a moving object

Employee was using scoop to assist with moving shuttle car, the bucket of the scoop slid upon the shuttle car. The shuttle car rolled forward causing the scoop bucket to drop causing the scoop to bounce and the employee then struck head in the canopy.

Employee lost EE's balance and fell on EE's left arm.

Employee stated that EE was packing timbers, and lost EE's balance. Straining EE's lower back

Employee stated was loading an object onto a pallet. At this time EE felt a strain in the groin area.

On 1-21-2021 a non injury roof fall occurred in the area of spad 183.This area was cribbed off in all approaches. This fall was not immediately reportable.

Employee was struck with material from the mine rib.

Struck by falling object

Employee had right hand laid upon bolter, draw rock fell between bolts striking employees index finger.

A non injury roof fall occurred inby spad 1563 in the # 3 entry in the active pillar line. This fall resulted in equipment being covered up.

Employee stepped off of the roof bolter & lost balance. As employee was attempting to regain balance, employee twisted an ankle. The employee was taken to a medical center & x-ray indicated employee had a sprain & employee was released to return to work. The employee went for a second opinion the following day & it was determined employee had a fractured fibia in leg.

Employee turned to leave the operators station and as they turned, they twisted an ankle causing them to fall to the ground. The employee was taken to Logan Regional Medical Center where they were x-rayed and it was determined that they had a fractured lower leg.

Struck by falling object

Employee was closing battery lid on fork lift and dropped lid on EE's hand causing a laceration.

Struck by flying object

Employee was using allen wrench to remove bolts from wheel unit. While attempting to loosen the bolt, they struck it with a hammer and the allen wrench came out of the bolt and went under their safety glasses and struck them just above the left eye causing a laceration.

Employee was operating shuttle car and hit corner of rib with canopy and material struck them in the face causing laceration above right eye.

Employee was pulling downward on drill steel during roof bolting operations when they felt a strain in their shoulder.

Employee was removing debris from machinery when a piece of material fell and pinched their hand between material and machine resulting in a laceration on their finger.

Struck by powered moving object

Employee stated that EE was bolting top. The bolt bent and EE went to realign bolt to drill pot, bumped rotation causing the bolt to strike employee in hand and arm.

While removing a piece of rock employee pinched finger causing laceration.
