Employee was unloading supplies from the scoop bucket and placing them onto the roof bolter. When the employee was picking up some bolts, the bolting machine moved, and employee jumped into the scoop bucket to get out of the way and accidentally got their foot caught between the scoop bucket and the roof bolter.

An unplanned roof fall occurred above anchorage in the No. 7 entry outby spad 3313 measuring approx. 60 foot long and approx. 35 inches to 45 inches thick.
Employee was performing maintenance on a shuttle car, when a piece of draw rock 11 inches long, 6 inches wide and up to 3/4 inch thick fell and struck EE's left hand causing a laceration. Gloves were worn at time of incident.
EE was letting down cable out of a rope hanger that was connected to a spad nail. The spad nail came out of the rib, hitting EE's left hand against a roof plate cutting EE's hand
Employee was removing a bull line for a pressure pump, the line still had some residual pressure after shutting off the in line valve. As the employee uncoupled the line it blew off and struck the employee in the forehead causing a laceration.
Employee was pulling on a piece of conveyor belt, when the channel lock pliers employee was using slipped striking employee in the face.

Employee was struck on the left knee by a rock while roof bolting.
Employee was tramming roof bolter and a stack of roof bolt plates slid off the deck and came in contact with EE's hand causing injury.

Employee was struck in the face with a crib block.
Employee was using a ladder to build a stopping. EE was carrying block up the ladder and the ladder shifted causing EE to jump and then fall causing injury to EE's shoulder and right side.

While assisting pulling belt, a miner was struck above the right eye by a shive rope when it broke. Injury was not the result of a training or compliance issue.
Employee was tramming scoop when a piece of rib sloughed off, falling into the deck of the scoop striking the employee's small finger ( Pinky finger ) causing laceration.
Employee was pulling down a piece of curtain between the No.1 and No.2 entry, while pulling the curtain from the rib, a nail used to secure the curtain flew in over top of safety glasses striking them in the left eye.
An unplanned roof fall occurred on the West Mains one break inby spad# 1653 in the No. 1 Entry. The fall was approx. 20' wide, 30'long and 14' high.
An unplanned roof fall occurred inby spad #1660 in the No. 2 Entry of the South Mains. The fall was approx. 15' high 20' in length and 20' wide.

Employee was struck on shoulder/back area with a piece of rock, and lost EE's balance falling to the mine floor. The employee felt pain in EE's back and was taken to Logan Regional Medical Center. There EE received x - rays that revealed EE had a compression fracture in one vertebra.
EE was pulling on cable & felt a pain in lower back. EE was taken to a med. center where EE was treated & given full release to return to work next shift. EE did RTW next shift & continued to work until 2/16/21 & has not returned to work since. Ramaco Resources was informed on 4/6/21 that EE's claim was reopened which resulted in a NFDL accident.

An unplanned, non-injury roof fall occurred in the #1 pillar entry on the #1 section. The material fell onto the continuous miner and two mobile roof support systems and had to be removed to get the equipment out and back into service.
A piece of draw rock fell between the primary roof supports and struck the employee on the left shoulder.
An unplanned roof fall occurred in the Right Return just inby Survey Spad #1487. No persons were injured as a result of this accident.

An unplanned roof fall occurred in the right return at break 9 on #6 belt in the vicinity of Spad No. 1011. The fall measured approx. 20' long x 16' wide x 4 1/2' thick.
Employee was walking in front of parked scoop bucket which was carrying a belt head. As the employee walked by the belt head slid out of the bucket onto employee's foot.
Putting tie under track rail & rail slid off slate bar and mashed fingers.

Employee was removing motor from miner and felt a pull in arm / shoulder area. Employee was transported to Logan Regional Medical Center where ee was examined and diagnosed as having a muscle strain. EE was released to return to work the following shift.
Employee was installing metal stopping panel and was pushing against roof and dislodged a piece of rock which struck them on the nose.

Employee was in the process of dropping a cable in the crosscut between #3 and #4 entry. At this time a piece of rock fell from the roof striking the employee in the head and shoulder sliding down their hip and leg.
Employee was pulling hose & wand turned & pressure from water cut employee's face.
The employee was setting up a ladder to plaster on a stopping when a piece of roof material fell and struck employee's left foot causing a fracture.
