Employee was mining in the number 7 right crosscut located on the #2 section when a methane ignition occurred resulting in burns to the employee's face, hands and arms.

Employee was helping remove a broken torque shaft on the CM machine and as it was being pulled out by another employee the broke end struck EE's left hand resulting in a laceration.

Employee was conducting maintenance when they stood up from a knelled position and strained right knee. Employee continued to work until 10/23/25 when they sought medical treatment and took out of work.

Employee was hanging line curtain on the #2 section in the #4 entry when a piece of coal rib gave loose striking them in the lower back and ankle.

Employee states employee was shocked while changing batteries on the scoop. Employee continued to operate the scoop until later when employee informed employee's boss and asked to be brought out of mine to be checked by a doctor.

Employee was mining in the number 7 heading on the Mains section when a methane ignition occurred resulting in burns to the employees face and hands.

Employee was struck on the left forearm by a piece of rock that fell from the mine roof resulting in a laceration.

Employee ran over a solid cinder block causing EE to bounce in the seat resulting in back pain. Employee continued to work for several days and followed up with the ER doctor on 8/22/24 when EE was taken off work.

Employee was using a chain to pull belt and the chain broke striking the employee in the ribs.

On 5/21/24 employee was pulling a piece of mining belt by hand when the edge of the belt flipped over hitting them on the left knee. The employee continued to work until they had surgery to their knee on 9/16/24 and is now off work.

The employee fell and twisted right ankle when exiting the rail bus man-trip.

Employee was using a razor blade knife to cut the knee pad and the knife slipped cutting EE's left index finger.

Employee was struck in the head by small rock while trouble shooting a fouled conveyor on the CM. The employee was examined by EMT and transported to the ER where they determined they may possibly have a concussion. The employee was written off until follow up with family physician who requested the employee remain off work until 2/12/24 for monitoring.

On 1-23-2024, the employee was pulling a piece of line curtain when EE felt a strain in EE's knee. The employee was transported to Clinch Valley Medical Center where EE was treated and released to return to work EE's next shift.

The employee inadvertently got the web of EE's left hand caught between two pieces of roof bolt steel when connecting them together.

Employee was in the process of cutting coal in the number two left crosscut when an unplanned methane ignition occurred resulting in flash burns to the employee's hands, neck and ear.

Employee was using manual ratchet chain hoist to free a stuck flight and the as they released the hoist a hand was pinched between two of the chain links.

Employee was removing the feeder cable that was hanging up with a rope hanger and as EE was letting the cable down the metal hook end of the rope hanger caught EE's nose resulting in a laceration on the left nostril.

Employee was traveling as a passenger on a rail personal carrier mantrip to the Main section. As the mantrip traveled up the slope incline, it lost traction and slid down the incline hitting a parked rail mantrip at the bottom of the slope.

Employee was moving power cables. Picked up on power cable to release rope hanger. Cable dropped and rope hanger cut left hand. Employee had to have stitches.

working at concrete pump around pump lines and employee was exposed to grout when breaking pump line. Employee did not properly wash affected areas.

Employee was dragging wash down hose and valve got caught on something and opened the valve. When pressurized, the valve begin swinging and struck the employee in the face on cheek.

Employee was moving a man door and got a wrist caught between the door and mine roof resulting in a laceration to the wrist.

Employee was pulling miner cable up and the hook broke loose striking the employee on the ankle

Struck against stationary object

Employee was walking across the section and raised EE's head up slightly hitting the mine roof with EE's hard hat and EE's teeth struck together resulting in a possible cracked tooth. The ER doctor released EE after EE's exam, and EE followed up with a dentist and the dentist determined that the best preventative action would be to remove the tooth.

Employee was carrying miner bits, as they dropped them to the ground, felt a pain in shoulder on 8/5/19. Employee refused treatment and on 8/9/19 stated that still had pain in shoulder. EE was taken to hospital, treated and released to return to work the following shift. EE recently brought in excuse beginning on 9/12/19 to be off work 6-8 weeks.

Accident type, without injuries

A borehole was drilled beneath a pre-existing mine that were not shown on a certified map the previous rooms that were mine. The water pressure apparently eroded the borehole and water began entering the mine. All employees were removed from the mine and this was reported to the MSHA hotline for immediately reportable accidents. No injuries occurred from this accident.
