Employee alleges that EE was retrieving a tool from EE's truck toolbox and a strong gust of wind blew the toolbox lid shut, the lid hit a wrench causing it to fly up and cause abrasion to employee's upper lip

Employee alleges was in process of cleaning screens and fractured finger handling screen cleaning materials.

Employee was pulling wire by hand and heard a pop in their left wrist. Injury was reported to employer 2/7/25. Injury is musculoskeletal, muscle strain and tendon tear. Injury occurred in AM. Injury initially recorded as an OSHA injury, error identified 7/2/25 and reported to MSHA.

Employee alleges was installing some grease lines and pulled a utility knife out of their pocket. It slid in EE's hands cutting EE's left palm. EE received stitches.

Employee alleges employee was getting ready to install a plate on a service truck and it fell and cut left pinky finger.

Employee alleges they were changing a drill bit and the drill fell from the hammer and pinned the index and ring fingers under the drill bit causing injuries to their right hand. Employee also received stitches.

Employee alleges they were adjusting the teeth on a loader and their right hand got caught in between the fork and frame cutting the palm of their right hand. They received 5 stitches.

Employee alleges they were lifting a piece of belt unto the back of a truck and they heard their left shoulder pop straining their shoulder.

Employee alleges they were moving cable wires off of a truck and they heard their left shoulder pop.

Employee alleges employee was holding a liner in employee's hands and a sharp object cut left hand/fingers. Employee received sutures.

Employee alleges EE was removing a guard from a water pump the guard behind EE fell on EE's left foot. EE received a fracture to EE's foot.

Employee alleges EE was removing a belt from the primary crusher and EE smashed EE's left thumb. EE has to wear a splint while working.

Employee alleges was closing the gate to the plant and left arm got caught in the gate cutting EE's left arm. EE received 23 stitches.

Employee alleges EE was lifting a screen motor to get the pin back in the hole and EE started feeling pain in EE's lower back.

Employee alleges was unloading a truck and the co-worker threw a pipe off of the truck. The pipe hit employee on their left foot.

Employee alleges the day before they were rolling some belting around but they didn't feel any pain until the next day employee wasn't sure if they injured themselves at work. Later after seeking medical attention they stated they did hurt their lower back at work. This is late reporting do to just receiving Dr. notes.

Employee alleges they were unwinding a electric pump and when they stood up, they hurt their lower back.

Employee alleges EE was removing a rock on crusher and it slipped and cut EE's middle finger on EE's left hand

Employee alleges was lifting and pulling some screens at the plant and heard right wrist pop. Employee started losing time on 09/29/22 and then had surgery on 10/18/22. (Late Reporting due to waiting on Dr. notes and information).

Employee was doing maintenance at the plant, removing a section of catwalk. While doing so, the catwalk fell hitting EE's wrist pushing it into the hood of the scalping screen causing laceration on the outside of EE's rt wrist.

Employee alleges employee was removing a drive belt guard on a screen and it popped out and struck employee in right arm and smashing employee's middle finger. Employee received stitches in arm. *Employee had surgery on Nov 14, 2022 and is losing time and is still off work.

Employee alleges was lifting a hydraulic pump and felt pain in employee's back.

Employee alleges they were removing loader tooth from magnet and they smashed their right thumb between the loader tooth and box beam. Employee received 9 stitches.

Employee alleges the impact roller got caught and EE tried to release it and EE's left thumb got caught in the roller and it smashed EE's left thumb.

Employee alleges was shoveling and EE heard a pop in the lower back.

Employee was placing fuel hose nozzle back in cradle and felt discomfort in EE's side, shoulder and neck. Lifting nozzle from filler neck to hose nozzle cradle.

Employee was changing out a troughing roller when a speck of rust/metal got into employee's right eye. Employee thought employee got metal out of eye, but eye swelled up the next morning, and sought treatment to remove piece of rust/metal.

Employee alleges EE was climbing a ladder to change screens and later that night EE's left knee started swelling. EE was diagnosed with a sprain and was splint.

Employee alleges after feeling discomfort in abdomen area EE went to a physician. Dr. scheduled EE to see a surgeon. Employee recently had a hernia operation not job related. Surgery was scheduled for 08/31/20 and employee started losing time then.

Employee alleges was removing a bolt from the cutting edge and the sharp edge of the bolt cut the middle palm of left hand.

Employee alleges they were cleaning off the belt on the loader and they turned sharply and felt immediate pain in left lower rib area. Employee didn't seek medical attention at first then later decided to be seen.

Employee alleges they were removing a skid steer bucket pin and a finger got jammed in between the bucket and the tire. Employee smashed the left index finger and received 4 stitches.

The employee alleges they were working at the plant with some flopping gates and screens and they hurt their lower back.

Employee alleges ee was moving some large culver pipes and one came off and ee tried to stop it from falling and ee cut left hand receiving 7 stitches.

Employee alleges was holding back the middle deck rubber and turned on the hose and it slipped out of employee's hands and hit employee in the mouth breaking front tooth in half.

Employee alleges ee was hooking up chains to move a conveyor and ee stood up and heard knee pop. Late reporting employee just notified employer on 11/10/17. Employee went to Dr. on 10/23/17 and is now loosing time.

Employee alleges ee was unloading a portable pump from a truck and ee tossed the chain around the front end of the loader and it got caught and came back and hit ee above right eyebrow. EE received stitches.

Employee alleges self was cleaning skid loader lock arms and ee attempted to set the attachment lock and it slipped off of the machine and cut left shin. Employee received stitches.

Employee alleges ee was aligning the pulleys and the bolts up and ee was pushing forward on the bearings with left arm and ee heard shoulder pop. Late Reporting due to employee just had surgery on 02/28/17 and ee is now losing time from work.

He was installing new hydraulic control levers on a loader and while reconnecting the electrical wiring had to push hard to connect wiring harness. He heard a pop in his left hand at the base of his index finger. His hand was swollen and hurting the next morning.

Employee alleges he and a co-worker were attempting to reposition a I-Beam assembly which was being worked on and supported by jack stands. They both had a hold of one end of the I-Beam and it slipped from both of their grips striking the employee on the top of his foot behind the steel toe of his shoe.

Employee alleges he was putting liners in a load hopper and it became too heavy and it tipped over on him. Causing a fracture to his left ankle.

Employee alleges he was cleaning out a grizzly and he reached down to remove a small rock and then a big rock fell over and smashed his finger between the grizzly and the rock causing a laceration to his right index finger. He received 6 stitches.

Employee alleges he was lifting and pulling rocks out of the crusher and some rocks fell on him hurting his neck, left shoulder and hip. Employee just started losing time on 7/28/14.

Employee placed a 4x4 underneath a portable pump to be able to pick the pump up with a forklift. He signaled the operator to raise the forks. The 4x4 snapped pinching the end of his little finger causing a small fracture.
