Employee alleges that a wrench slipped from their hand and injured their foot.

Employee alleges was grinding surface to weld studs and the shrapnel bounced off of the wall and went into EE's grinding hood in EE's left eye.

Employee alleges was holding back the middle deck rubber and turned on the hose and it slipped out of employee's hands and hit employee in the mouth breaking front tooth in half.

Employee alleges EE was cutting a piece of steel on a truck frame with a grinder and the wheel slipped from EE's hand and cut EE's left arm. EE received stitches.

Employee alleges EE was coming off of stairs and EE step of on some cement base falling forward unto EE's left knee. Late Reporting due to alleged injury not being documented by employee, and confusion with insurance company compensability because of the lapse time from the original date of injury. Employee started loosing time on 08/09/2016.

EE alleges he didn't remove the snow on the chute and he was climbing up on the chute and he slipped on some ice. He broke a bone in his right ankle.

Employee alleges he was trying to remove the top of the hand cleaner with a utility knife and the knife slipped and cut his left index finger at his knuckle. A medical procedure was done by using glue to glue together his finger.

Employee alleges he was tightening a bolt and the wrench slipped off and his hand struck the conveyor frame at full force fracturing the tip of his left pinky finger - he received 7 stitches.

EMPLOYEE ALLEGES HE WAS WALKING OVER FROZEN MATERIAL AND HE TRIPPED AND TWISTED HIS LEFT ANKLE.

EMPLOYEE ALLEGES HE WAS WALKING ON CATWALK AND HE STEPPED THROUGH THE CONVEYOR AND SLIPPED, HE TRIED TO CATCH HIMSELF BY USING HIS HANDS AND ARMS AS HE FELL TO THE GROUND.

EE ALLEGES HE WAS USING A SLEDGE HAMMER TO KNOCK A RETURN IDLER OVER THE BELT AND THE HAMMER GOT CAUGHT BETWEEN THE BELT AND THE RETURN IDLER PULLING HIS RIGHT ARM INTO THE CONVEYOR.

EE ALLEGES HE WAS PICKING UP CONVEYOR BELT AND HE STEPPED OFF THE END OF THE CATWALK AND FELL ABOUT 3 FEET HURTING HIS NECK, RIGHT ARM, LEG AND CUTTING HIS HEAD RECEIVING STITCHES.

EE ALLEGES HE WAS LIFTING WEAR PLATES WEIGHT ABOUT 70LBS. ON THE DRY PLANT WHEN HE FELT A POP IN HIS BACK.

EE ALLEGES HE WAS JUST FINISHING UP GREASING A 30 GALLON BARREL AND HE FELT SOMETHING PULL IN HIS STOAMCH AREA. (UMBILICAL HERNIA)

EE ALLEGES HE WAS USING A PRYBAR TO CLEAN OFF BUILT UP MATERIAL FROM THE PRIMARY CONVEYOR AND THE PRYBAR SLIPPED SMASHING HIS LEFT MIDDLE FINGER.

EE ALLEGES HE WAS GRINDING PIPE WITH A GRINDER AND HE STARTED HAVING PAIN IN HIS LEFT WRIST.

EE ALLEGES WHILE WALKING TO HIS TRUCK HIS LEGS AND FEET SLIPPED FROM UNDER HIM, THE GROUND WAS COVERED WITH ICE AND HE FELL ON HIS RIGHT ARM AND SHOULDER.

EE ALLEGES HE WAS BENDING OVER SHOOT DIGGING OUT MUD AND HE STARTED FEELING PAIN IN HIS LOWER BACK.

EE ALLEGES HE WAS BEATING ON BIN WITH A PIECE OF METAL TRYING TO UNPLUG THE BIN AND HE FELT PAIN IN HIS RIGHT WRIST.

EE ALLEGES WHILE LOADING HAUL TRKS & OPERATING LOADER THE FLOOR WAS ROUGH & THE MACHINE KEPT TWISTING & TURNING, HE STARTED FEELING PAIN IN HIS BACK
