Employee alleges was grinding surface to weld studs and the shrapnel bounced off of the wall and went into EE's grinding hood in EE's left eye.

EE alleges he didn't remove the snow on the chute and he was climbing up on the chute and he slipped on some ice. He broke a bone in his right ankle.
