Employee alleges EE was using a prybar to get rock out, the prybar slipped and EE hit the left side of EE's cheek. EE received stitches on EE's cheek.

Employee alleges was shoveling and EE heard a pop in the lower back.

Employee alleges EE was climbing a ladder to change screens and later that night EE's left knee started swelling. EE was diagnosed with a sprain and was splint.

Employee alleges self was cleaning skid loader lock arms and ee attempted to set the attachment lock and it slipped off of the machine and cut left shin. Employee received stitches.

Employee alleges he was pulling on a pry bar to loosen a chain and the bar slipped and hit him in the nose. The employee went to the Dr. on 03/06/12 and we were then informed on 03/07/12 that his nose was fractured.

Employee alleges he was breaking up rock out of riprap grizzle and as he was hammering he twisted his lower back.

Employee alleges he was cleaning rollers and went under the conveyor and slipped on some ice.

Employee alleges he was using a propane burner to heat air tank and air liner and the air tank exploded.

Employee alleges he was prying on frozen mud to clear chute and he felt pain in his stomach. Had surgery on 12-13-05 started losing time then.

Employee alleges he was putting in wear bars and hit his left index finger with a hammer.

EE ALLEGES HE WAS CRAWLING OUT FROM UNDER TUNNELBELT AND HE WENT TO STAND UP AND SLIPPED ON WET FLOOR AND WENT DOWN VERY HARD ON RIGHT KNEE.

EMPLOYEE ALLEGES HE WAS WALKING ON CATWALK, THE CONVEYOR STOPPED AND ROCK HAD BUILT UP IN TRANSWER HOPPER AND ROCKS WERE RELEASED HITTING EMPLOYEE'S FINGER.

EE ALLEGES HE WAS DRIVING IN HIS TRUCK AND HE STARTED HAVING PAIN IN HIS PELVIC AREA.
