Showing 50 of 152EE was rotating the sand screws manually by grabbing a hold of the V-belts, as the EE was rotating the sand screws the EE's middle and ring finger was caught between the V-belt and Sheeve causing a Turf Fracture to the tip of the ring finger.
EE was involved with installing v-belts when they became twisted. EE was manually rotating the v-belts on the pulley to straighten them when the pulley unexpectedly rotated causing the EE's finger to become trapped between the pulley and the v-belt.
Miner was working inside the Raw Mill on the seal air vertical pipe #3 roller. Employee was disconnecting the pipe and rolled the pipe around out of the way of the work. When they rolled the pipe, the pipe swung around so quickly that it pinned their left index finger between pipe and yoke of the #3 roller. The pipe mashed it at the nail and busted the end of the index finger.
Employee was restacking bags of cement onto a pallet when they felt a pop in their lower back.
Employee removed side port hole to remove rocks and cement rushed out of port hole and got into EE's right eye.
Employee was assisting delivery drive in offloading conveyor to jack stand. Conveyor shifted unexpectedly and crushed employees left hand.
EE was standing on ladder to check progress of the Maintenance work being completed when a co-worker pulled out the screen unaware of EE's hand placement on the ladder. The screen caught EE's finger pulling back EE's left pointer finger nail
Employee was closing a shale gate. The gate did not fully close and EE used the handle lever (detachable square tubing) to strike against the gate. In doing so, EE hit EE's hand on the gate fracturing the tip of EE's left ring finger. Also lacerated the finger the required stitches.
EE was cleaning mill building, after a few hours employee went to cool off and take a break, after removing goggles something went into left eye.
EE was installing a troughing Roller on Cone feed 1 belt and when EE was pushing new roller set into position troughing roller flipped up pinching EE's hand between the frame and the roller causing a laceration to EE's thumb
WHILE EMPLOYEE WAS ASSISTING IN CLOSING THE SCREEN CHUTE GATE, EE PLACED RIGHT THUMB IN A PINCH POINT AND INJURED THE RIGHT THUMB. 3 STITCHES TO THE NAIL BED OF RIGHT THUMB.
EE was hosing out bottom pan of sand screen using fire hose standing on A frame ladder. ladder came out from under them causing them to fall and strike left shin on ladder. left shin received stitches
Employee was picking up a sheet of plywood when EE felt a pop in EE's right elbow. Initial diagnosis is a rupture of the right distal bicep tendon.
Team member was closing a door on the finish mill and caught fingertip between a wire sling causing a laceration.
EE was switching cement lines as gate was stuck, as EE was switching, gate got stuck halfway open which caused cement to spray out and into EE facial area. Causing cement to enter EE's eyes.
Maintenance Tech was relocating empty compressed gas cylinders when the Tech hit self in face with cap of bottle. Tech was pulling bottle to the edge of secure rack to roll cylinder, when pulled on cap to move, the cap came loose and hit left portion of safety glasses causing laceration to eyebrow
Employees were working on a bearing on a belt scale and noticed a fish had come up through the plant and one of the employees went over to move it. When EE picked the fish up, one of the barbs stabbed EE's hand and when EE went to throw it, the barb broke off and got stuck in EE's hand.
EE was replacing drive chain on bucket elevator. As EE was attempting to pull chain together to pin, EE's hand slipped and caused EE to hit bottom of guard causing a small laceration on left hand ring finger second knuckle. Requiring 5 stitches.
While moving a punch top drum, Employee received a cut on their right thumb from a sharp edge. After the thumb was cut, Employee's thumb was glued for closure of the wound, hand and arm were placed in a splint to stabilize and keep the thumb from moving. Employee was prescribed Hydrocodone, Acetaminophen, and Cephalexin, they also received a Tetanus shot and T-Dap Vaccine.
Employee was working on guzzler truck removing back suction hose, when employee loosened clamp hose fell and struck employee in left foot area, just behind steel toe area.
Miner caught R thumb between bridle of rigging chain and storage point. Medical care was provided and prescriptions written.
A group of 4 maintenance technicians were doing a team lift to get a wheelbarrow welder up onto the bed of a truck. One of the technicians felt a very sharp pain in their right forearm.
On 01/09/2024 EE was cleaning under belt and walked into a power box. On 01/24/2024 EE had discomfort in EE's neck went to Dr. on 01/26/2024 Dr. released EE to full duty and recommended PT, this made it a medical
Strained elbow working on kiln baghouse dust collector
Process Attendant was checking the rejects magnet, the PA observed a piece of metal stuck to the magnet. When the PA had the piece of steel outside the plane of the door, PA attempted to remove the steel from the doorway when a rock came in contact with the piece of steel debris and PA's right thumb.
Employee was helping another employee replace the cutting edge on a loader bucket. They were both lifting one end of the cutting edge to get it into place to attach it to the bucket. When they were lifting the cutting edge, it raised up until it hit the bucket then it stopped abruptly. The employee felt pain in EE's back at that moment.
EE thumb was caught between two flanges when assembling.
The EE was improperly loading steel into a drill. They laid the boom down & were adding steel directly into the carousel. One end of a steel fell out while handling it. This caused the other end to fall out & smash their thumb between another steel. Upon seeing a doctor, it was diagnosed the the EE ruptured a tendon in their right thumb.
April 19, 2023, Employee was doing maintenance on a Hydraulic ram when the ram slipped when attaching the ram striking the employees left hand causing injury.
Picking up a busted bag of cement, miner dropped bag and dust went under their safety glasses and got in left eye.
Employee was moving vibrator from railcar when they had stiffness in their lower back.

Employee was lifting door holding bar from finish mill when EE felt pain in EE's lower back.
Miner was lifting empty barrels which had been crushed to load into loader bucket.
Employee was installing new drive belts on a conveyor sheave. As employee was rolling the new belt on the sheave their right thumb was caught between sheave edge and belt causing a laceration to thumb.
While changing a wear liner on the T-400 crusher, EE was struck in the arm by a wear plate that took an unexpected fall during removal.
Employee caught finger between pinch point and caused laceration to middle finger.
Employee was going for a pick on a water pump. The employee was holding the chain for the Telehandler to slide the fork between. As the fork was in position for the chain to slide over the fork employee's finger got caught between chain and fork pinching employee's finger.
EE was removing screw from Skidsteer bucket namely Edge Plate. Head bolt lacerated left middle finger requiring stitches to close the wound.
The EE was respooling the dragline hoist cable. EE had set the wedge in place and was holding the cable. The cable slipped out of EE's hand and caught EE's right index finger between the wedge and the cable. This created a laceration to EE's finger requiring stitches. The EE was wearing leather gloves.
Employee was cleaning inside of a Reed Gunite Pot and got left hand glove caught between feed wheel and rock shear. Severed tip of middle finger and index finger.
Employee was replacing a guard when it slipped through employee's hands and dropped on left foot, just behind the steel toe protection.

On 11/10, EE told supervisor EE was having abdominal pain, thought it was a personal health issue. On 11/15, doctor diagnosed with a groin strain and gave restrictions. EE claims the injury occurred while handling worn shoes on the VSI crusher on 11/09. Worn shoes weigh approx. 50 lbs.
We had two shots today. Halfway through the 2nd shot, I was moving the hose over some drill cutting piles and I felt pain in my scrotum. I went to the ER to be examined by a Dr. After receiving an ultrasound and CT scan, I was told I have 2 Inguinal Hernias that will require surgery to fix.
Employee was working on dozer, when EE noticed a socket under side hatch door. EE attempted to remove socket and when EE did it fell on EE's left hand index finger area.
Employee had removed cover guard to put oil in reservoir. EE set the guard beside the reservoir and after adding the oil, EE and another employee went to put the guard back into place. After lifting it up in tandem they turned to the right to align when employee felt a sharp pain in their lower back.
Employee poked self in the eye with ear-piece of safety glasses on 08/23/2021 while trying to put them back on, and requested medical treatment on 08/25/2021. Was treated with an antibiotic eye drop and released straight back to work. No restrictions.
