Employees finger was caught between a sliding section of angle iron and a fixed item on the plant. This resulted in a 1" cut and a fractured finger tip.
Miner was shoveling wet material at the entrance to the Lower Mill.
While lifting a panel (guard) on the side of a trailer section, EE experienced pain in the neck and shoulder area.
Cleaning out chute.
After a wiper blade came loose on the screen discharge conveyor belt, employee and 2 other coworkers were attempting to secure the wiper blade's pivot arm to the belt frame with a wire. During the process, the wire came loose, and employee's hand became pinched between the crusher's frame and the wiper blade's pivot arm, resulting in a cut to the palm of their right hand.
Dust hose to the dust collector pot became full of mud. EE removed the hose at the lower point to to clear it. The hose fell off. employee continued to unclog the hose by elevating it the middle. EE ended up moving hose off to the side to allow it time to dry out. The next day EE returned to installed it. The employee said EE did not need medical attention till 4-22-26
Employee was cutting a plate off a head box above screen when the metal bounced off catwalk and contacted EE's leg. Employee received 2 stitches.
Employee cut a box open with a utility knife and placed the knife on top of the box. When the employee pulled the box open with EE's left hand EE's left index finger make contact with the blade of the utility knife resulting in a cut to EE's left index finger.
Employee was underneath an articulated haul truck setting jacks and cribbing. The job site was very muddy and while twisting and grabbing equipment they felt their left knee pop. Called nurse triage, they advised visiting urgent care. Doctor orders from urgent care stated employee needed to be off of the appendage and work until an MRI can be taken. MRI scheduled through work comp.
Employee was disengaging #2 FM spotter clutch and felt a pop in their right shoulder, diagnosed with a sprain to right shoulder
Several utility workers were loading mill rods into the raw mill. As they were unloading a rod from the forklift forks, their finger was caught in between the hand rail and bottom of rod.
Two MWI employees were moving a 12'L x 6"W x 3/4" Thick steel flat Bar weighing approximately 184 lbs. to a saw horse. Both employees set it down before the injured employee was able to clear EE's right middle finger from getting pinched between the steel flat bar and the steel sawhorse.
Left Hand slipped and hit the side at the screen box causing lacerations, with 3 stitches.
Employee was placing a vibrator on a gyp car, as EE was placing the vibrator on the bracket on the car, EE felt pain in EE's abdomen. Meet reportable criteria on 4/2/26 when EE had hernia surgery.

Employee sprained their left big toe while exiting a screen deck. After three weeks, the injury did not resolve. Follow up with doctor on 03/24/26 resulted in same diagnosis, but new requirement of work boot to stabilize the toe.
Employee working under other employees checking on a dust collector when a small amount of debris fell hitting employee on the top of EE's hard hat causing neck pain.
Employee was shoveling material and felt a pain in EE's left shoulder. EE went in for an MRI and then on 2/25/26 EE was diagnosed with a shoulder impingement and issued PT.
Employee was rigging up the cone ring to remove it from the cone. It got bound up at an angle and would not move. EE reset the rigging then started to descend from cone. The cone ring did not have any pressure from the rigging. Descending from the cone onto the platform EE put EE's hand out for support, the ring fell into its resting position just 2" from bound position.
EE sustained laceration from metal fragment. Clinic prescribed RX antibiotic as preventative measure.
Employee states EE sustained foot injury while removing under grate plates in cooler. X-ray shows possible hairline fracture, RTW same day with restrictions.
EE was throwing a piece of metal in the bed of truck and felt a small strain by EE's groin on right side then a bump came and it is hot to touch now. EE will be out of work for 3 days.
Employee was manually sliding a piece metal off the forks of a forklift when the metal rotated and slid off striking the employees left foot above the steel toe resulting in a hairline fracture of the metatarsal.
EE was trying to climb into the truck and had EE's hands on the handrail. EE had to pull self up to the next step and started to feel sharp pain in EE's right hand going all the way to EE's arm. After miners latest doctor visit, Miner has been put on "Do not Work" status per miners Medical provider until next evaluation.
The incident occurred on 12/1 but employee didn't report it until close of shift on 12/2. Employee was clearing a head pulley when they pulled a rock out another fell smashing their left pinky causing a fracture. Our employee was wearing gloves.
While shoveling pit on VS2, employee felt a pain in EE's right elbow.
Emp was stuck by a angle iron brace that was being used to hold up a roll of belting. The angle moved and the piece struck them in the left shoulder causing them to fall to the ground. They worked the rest of the shift and went to the ER after the shift. They treated them for left shoulder and elbow pain. Employee was placed off work until next Tuesday, 11/18/25.
Employee was injured while replacing belt line when EE was struck by a snatch block that tore away from the structure it was attached to.
Maintenance contractor was changing out conveyor belt when the anchor point they were using on the surge tunnel broke free causing the snatch block to strike the contractor in the left hip fracturing EE's pelvis.
EE had been using a pry bar on a head pully to rotate it and it slipped when they gave it a good tug.
Standing at ground level putting a bar back on grizzly screen. Was trying to guide the bar in to the bolt holes, lost footing and fell back to the ground and the bar fell on employee. Lower back/butt sore.
Employee was pulling in a line out of the river and felt a sharp pain in their elbow.
Lumbar spine discomfort.
While removing an alternator along side another employee, the other mechanic was pulling the detached alternator through the top of the machine, while injured was pushing through the bottom. It slipped out of the other mechanic's hand and fell down smashing injured employee's right middle finger between the alternator and the engine mount bolt.
Employee was adding coolant to a CAT 980M when the hood came down quickly and knocked the employee back into the catwalk /fender support where the hood came down and rested across employee abdomen.
EE was attempting to rotate a rotor shaft manually to grease the bearing housing. EE placed EE's hands on the belt to move the shaft and when EE pulled EE's pinky finger was pulled into the motor sheave causing a laceration and a fracture to EE's pinky finger.
Associate was removing hydraulic pipe from excavator boom. When fitting was loosened, pressurized hydraulic oil (cold) sprayed EE in the face causing EE's hard hat to be pushed back and EE's safety glasses to come off. Associate flushed eyes with eye wash but was taken to Emergency Room for discomfort as a precautionary measure.
Employee was lifting a barrel of grease into position. While doing so their foot slipped on truck surface causing body to reflex and drop the barrel into position.

Employee reported discomfort in RT foot after attempting to load an empty chemical tote into the bucket of a rubber tire backhoe.
EE was trying to clean the metal off of the magnet that leads to the VSI. They grabbed a piece of metal to pull off and strained their arm.
Miner was lifting a belt from the ground when they fell backwards onto the ground.
Employee was assisting in changing out a screen. While pulling on the screen to slide it up, they felt a small pop in right shoulder. Diagnosed with shoulder strain and placed on restricted duty of no push/pull greater than 5lbs and no overhead work.
Employee was cleaning the right side windows on the outside of a Komatsu WA500 front end loader, when they fell from the top of the loader and landed on the ground next to the loader.
EE states EE's hand was smashed by an unknown object when assisting with removal of welder from truck bed. EE stated on 8/7 and 8/8 that EE did not wish to seek medical treatment and no signs of injury were present. EE also stated that EE had previous injury to the same area of EE's hand. EE left site and sought treatment 8/9/25. EE resigned 8/20.
Employee was wearing a full silver hot suit, pants, jacket and hood. While removing a hopper of hot kiln inlet feed from the discharge area, The pant of the hot suit raised above the employees boot due to the seated potion resulting in hot material in their boot resulting in a burn to the left foot arch area. The employee also got dust in eye when they hastily removed hood.
While disassembling a gear box from a super stacker, it was in park postion sitting on a pile of material, went to pull it off the shaft, when it fell it struck the employees foot rather than going into the loader bucket.
