Welding h-beams and channel inside hoppers when employee picked up a beam and felt pain in left shoulder.
Shoveling snow out from under conveyor and began to have right elbow pain. Employee did not report injury till 11/27/2019.
While changing a belt on a conveyor, the employee lost control of the guard on the head pulley. The head pulley then struck the employee in the right arm causing a laceration.
EMPLOYEE SET A STICK OF 20' ANGLE IRON ON 2 FLOATS WHICH MADE IT 3' OFF THE GROUND TO CUT IT INTO PIECES TO MAKE FLOAT PIPES. WHEN CUTTING OFF A 39" PIECE (THE 20' PIECE) FELL AND LANDED ON THE EMPLOYEE'S FOOT.
Employee was working from truck bed to remove ramp wall off of portable plant. When removing bolt put in for transport, the wall began to fall towards them. As wall was falling employee fell in the bed and injured left ankle. Wall came to rest on truck bed walls.
Employee was sandblasting and stopped to take a break. Employee took off PPE and rubbed their eye. During that process it is believed that a piece of debris was transferred into the employees left eye.
Employee states that over the past 2 years of operating a loader they have developed a cyst on their left thumb from repetitive motion. Employee will have surgery to remove the cyst.
While cutting a hopper with a circular saw, employee finished the cut and took their safety glasses off. Upon taking safety glasses off, a particle of steel fell into eye. Particle was irrigated out on site, but employee was still seen by physician. Employee received RX for injury.
While an employee was carrying waterhose up the platform the nozzle came off and struck the injured employee, who was below, on the hand.
EE was helping check the finishing plant out during start of shift. EE grabbed a 2 inch water hose and when they bent over they told the foreman they hurt their lower back and could not stand up straight. The foreman brought EE to break room and asked if they need medical assistant. EE stated that they did, and then was taken to the company doctor.
While adjusting the door on secondary crusher, the door fell open and crushed the miner's thumb. Two people were trying to adjust the door when it fell. The door fell open smashing the thumb against a conveyor structure.
The injured employee was positioning an 8" hydraulic check valve onto the forks of a fork truck. The valve slipped on the forks and the employee attempted to catch it before it fell off of the forks. The Valve pinched his left pinky and ring finger between the fork and the valve injuring those fingers.
The injured employee was clearing rock out on the discharge end of a conveyor belt. He freed the obstruction and injured his left Pinkey Finger against a skirt board.
The injured miner was removing 6-10" limestone rock from a feed chute. He moved one rock and another fell and impacted his left middle finger, fracturing the tip bone.
Employee was installing belts on a screen while rolling belts on, he pinched his middle finger.
Employee was pulling on a water hose. It got caught on an I-beam. He pulled harder to get it to break free and it did not. The last pull hurt his right Shoulder
He was in a man lift trying to change a gate at the scalpering screen, when he said his back was hurting. he was sent to a medical dr. after complaining of his back hurting. He was at the chiropractor the night before for his back.
Miner was pulling electric lines to the primary crusher. While pulling the ground line tight the strap he was using slipped. When the line slipped he grabbed the wire with his left hand and a sharp piece on line cut his hand.
The injured employee was moving a steel beam into position inside a feed hopper with another employee. The beam became temporarily lodged in the hopper. The other employee pulled on one end of the beam to dislodge it. when the beam became dislodged, it pinched the thumb of injured employee and cut the skin requiring stitches to the injured thumb
Employee was cleaning rocks from a plugged screen.
Employee was replacing a pulley on the primary crusher. He felt something "pull" in his upper right arm, but didn't think much about it. He had some swelling the 2nd day and it was still bothering him, so he notified the plant superintendent. He went to the med center and was diagnosed with 727.62 biceps tendon rupture. He is scheduled for an MRI at a later date.
WHILE PICKING MUD & ROCKS OFF OF CONVEYOR BELT, HE REACHED OUT TO PUSH A ROCK OFF THAT WAS HEAVIER THAN EXPECTED. THIS SPRAINED HIS RIGHT SHOULDER & HE NOTIFIED FELLOW EMPLOYEES & MANAGEMENT IMMEDIATELY. HE CHOSE NOT TO SEEK ANY MEDICAL ATTENTION UNTIL 07-12-2013 AFTER CONTINUED SORENESS PERSISTED.
The injured employee was climbing a fixed 7' ladder to access a conveyor platform. While he was climbing, he felt a sharp pain in his Right Hand. The employee continued his inspection of the platform and climbed back down the ladder. The employee lost sensation in his right hand and approximately 2-3 hours later the employees hand swelled up.
EE was working on East Pump and using right hand to put test cover on incoming power, when power flashed back and burned his right hand.
Employee picking mud from conveyor line, S & G plant left shoulder became sore. Reported to supervisor but no medical was immediately sought. On 08/20/2012 the employee requested to see doctor, shoulder was sore. First doctor's appointment 08/22/2012. Shoulder previously injured. There has been no lost time and employee continues regular job. Surgery is being considered.
Employee was removing a 2"x10" board after splicing a conveyor belt, he reached out with his left arm grabbing the board to pull it toward him. Employee said his left shoulder popped when he pulled on the board.
They were running gravel to the surge and a shut plugged up. They were cleaning up the rock and gravel. EE twisted his back.
When grabbing a hold of the grissly's rail the metal had a burr on it and when he moved his hand it sliced open his finger. He received four stitches and restriction until he returns to the doctor on Monday, 8/29/11.
EE and another EE were working @ the #3 bin. They had a piece of steel on sawhorses. Injured EE went to flip the steel over and the steel hit a 2' level causing it to fly up and hit EE in his left cheek.
Employee picked up piece of steel to weld on top of dragline bucket. Holding piece at chest level twisted slightly to install piece and his back popped.
Building new hand rail, another IMI EE heated the hand rail to form it. EE walking up stairs, grabbed rail and burned right palm, not knowing that the hand rail had been heated up.
EE was at the store to pick (2) 12V equipment batteries when he picked it up, he said his back popped, and took him to his knees.
Employee was replacing belts on crusher motor, when he lacerated the middle finger on his right hand.
While attempting to set a cable into the eye of a dragline bucket, he pulled back hard on the cable. This caused him to raise up & strike his hard hat on the pulley jamming his neck & back. He did not seek treatment until that evening. He was prescribed medication. After taking medication he had a severe allergic reaction causing a hospital stay & time off with restrictions.
Doing oil change and maintenance on 980H Loader. EE was putting the fender on back of loader, and when he twisted to set it in place, he felt a sharp pain in the center of his lower back.
Climbing up ladder of #4 bin of stone plant. When he was near top, ladder twisted around. He climbed down back side of the ladder by his hands. His left knee hit something when the ladder twisted around, and has been hurting ever since.
Two employees were loading two hydraulic cylinders onto a forklift, when one cylinder slid into the other smashing my left little finger.
Employee picked up a gravel pan weighing approx. 5 lbs and turned with it to go the opposite direction. When he did this he felt a sharp pain in his right leg. It hurt a lot worse when he tried to stand straight up.
Employee was pulling steel up out of wet bins on stone plant and pulled/strained his right wrist. He thought it would get better but it is still hurting. He reported it on 44-23-07 and went to medical center the same day.
While exiting the barn area, employee started walking down a slight incline. He experienced a sharp pain behind his left knee in the bend of his leg. When he wasn't any better by the following Monday morning, he sought medical attention.
A 55 gallon drum of oil was setting in the back of a pick-up truck. Employee was moving the barrel around to strap it down and sprained the muscle in his right forearm/wrist. He chose not to seek medical attention until Oct. 14, 2006. They put a splint on his wrist and gave him a medication for inflamation (no restrictions).
EE was exiting haul truck when his wedding ring became caught on the lid of the truck bed cutting his ring finger on his left hand. The laceration required three stitches.
EMPLOYEE WAS USING FORKLIFT TO BRING A PIECE OF METAL INTO THE SHOP, THE METAL SLID OFF ONE OF THE FORKS, AS HE WAS LIFTING THE METAL BACK ON THE FORKS HE STRAINED HIS BACK.
WHILE PREPARING TO DRILL A PIECE OF FLAT STOCK STEEL STRAPPING, EMPLOYEE PLACED STEEL ON A TABLE. THE STEEL SHIFTED & FELL ON THE TOP OF EE'S LEFT FOOT, WHICH CAUSED BRUISING ON THE TOP.
EE hurt back by moving 100 pound anvils out of VSI crusher. He attached a hook to the anvils and pulled them up and out.
EE IS A MECHANIC AND WAS WORKING ON A TAILGATE OF A HAULING UNIT AT STONEY CREEK. HE WAS WORKING UNDER IT AS WELL AS ALL AROUND. HE WAS WEARING SAFETY GLASSES. AT SOME POINT METAL FLEW BEHIND THE GLASSES INTO HIS LEFT EYE CAUSING IRRITATION. IT GREW MORE IRRITATED AS TIME WENT ON.
LIFTING SAMPLE PAIL OF STONE INTO BACK OF PICKUP TRUCK, PAIL WEIGHT IS ABOUT FIFTY POUNDS WITH STONE IN IT.
EMPLOYEE WAS CLEANING BEARINGS AND GOT METAL IN RIGHT INDEX FINGER.
EE was working with the dredge operator securing the locking ring on a ball and ball connection on the 14" pipeline while rotating the ring EE elbow popped after three medical visits to examine ee elbow it was determined that his bicep tendon its separated and will require surgery to correct.
EE WAS USING IMPACT TO TAKE NUT OFF A SQUARE LEAD BOLT IN WHICH THE BRACKET HAD A PIECE ON IT TO KEEP THE BOLT FROM SPINNING, BUT THE BOLT SPUN OUT AS EE WAS PUTTING PRESSURE ON THE BOLT WITH HIS FINGER. THE BOLT CAUGHT EE'S FINGER, CAUSING A LACERATION.
