EMPLOYEE SET A STICK OF 20' ANGLE IRON ON 2 FLOATS WHICH MADE IT 3' OFF THE GROUND TO CUT IT INTO PIECES TO MAKE FLOAT PIPES. WHEN CUTTING OFF A 39" PIECE (THE 20' PIECE) FELL AND LANDED ON THE EMPLOYEE'S FOOT.

Employee was working from truck bed to remove ramp wall off of portable plant. When removing bolt put in for transport, the wall began to fall towards them. As wall was falling employee fell in the bed and injured left ankle. Wall came to rest on truck bed walls.

While cutting a hopper with a circular saw, employee finished the cut and took their safety glasses off. Upon taking safety glasses off, a particle of steel fell into eye. Particle was irrigated out on site, but employee was still seen by physician. Employee received RX for injury.

EE was helping check the finishing plant out during start of shift. EE grabbed a 2 inch water hose and when they bent over they told the foreman they hurt their lower back and could not stand up straight. The foreman brought EE to break room and asked if they need medical assistant. EE stated that they did, and then was taken to the company doctor.

While adjusting the door on secondary crusher, the door fell open and crushed the miner's thumb. Two people were trying to adjust the door when it fell. The door fell open smashing the thumb against a conveyor structure.

The injured employee was positioning an 8" hydraulic check valve onto the forks of a fork truck. The valve slipped on the forks and the employee attempted to catch it before it fell off of the forks. The Valve pinched his left pinky and ring finger between the fork and the valve injuring those fingers.

Miner was pulling electric lines to the primary crusher. While pulling the ground line tight the strap he was using slipped. When the line slipped he grabbed the wire with his left hand and a sharp piece on line cut his hand.

The injured employee was moving a steel beam into position inside a feed hopper with another employee. The beam became temporarily lodged in the hopper. The other employee pulled on one end of the beam to dislodge it. when the beam became dislodged, it pinched the thumb of injured employee and cut the skin requiring stitches to the injured thumb

Employee was replacing a pulley on the primary crusher. He felt something "pull" in his upper right arm, but didn't think much about it. He had some swelling the 2nd day and it was still bothering him, so he notified the plant superintendent. He went to the med center and was diagnosed with 727.62 biceps tendon rupture. He is scheduled for an MRI at a later date.

WHILE PICKING MUD & ROCKS OFF OF CONVEYOR BELT, HE REACHED OUT TO PUSH A ROCK OFF THAT WAS HEAVIER THAN EXPECTED. THIS SPRAINED HIS RIGHT SHOULDER & HE NOTIFIED FELLOW EMPLOYEES & MANAGEMENT IMMEDIATELY. HE CHOSE NOT TO SEEK ANY MEDICAL ATTENTION UNTIL 07-12-2013 AFTER CONTINUED SORENESS PERSISTED.

The injured employee was climbing a fixed 7' ladder to access a conveyor platform. While he was climbing, he felt a sharp pain in his Right Hand. The employee continued his inspection of the platform and climbed back down the ladder. The employee lost sensation in his right hand and approximately 2-3 hours later the employees hand swelled up.

Employee picking mud from conveyor line, S & G plant left shoulder became sore. Reported to supervisor but no medical was immediately sought. On 08/20/2012 the employee requested to see doctor, shoulder was sore. First doctor's appointment 08/22/2012. Shoulder previously injured. There has been no lost time and employee continues regular job. Surgery is being considered.

When grabbing a hold of the grissly's rail the metal had a burr on it and when he moved his hand it sliced open his finger. He received four stitches and restriction until he returns to the doctor on Monday, 8/29/11.

While attempting to set a cable into the eye of a dragline bucket, he pulled back hard on the cable. This caused him to raise up & strike his hard hat on the pulley jamming his neck & back. He did not seek treatment until that evening. He was prescribed medication. After taking medication he had a severe allergic reaction causing a hospital stay & time off with restrictions.

Employee picked up a gravel pan weighing approx. 5 lbs and turned with it to go the opposite direction. When he did this he felt a sharp pain in his right leg. It hurt a lot worse when he tried to stand straight up.

While exiting the barn area, employee started walking down a slight incline. He experienced a sharp pain behind his left knee in the bend of his leg. When he wasn't any better by the following Monday morning, he sought medical attention.

A 55 gallon drum of oil was setting in the back of a pick-up truck. Employee was moving the barrel around to strap it down and sprained the muscle in his right forearm/wrist. He chose not to seek medical attention until Oct. 14, 2006. They put a splint on his wrist and gave him a medication for inflamation (no restrictions).

EE WAS USING IMPACT TO TAKE NUT OFF A SQUARE LEAD BOLT IN WHICH THE BRACKET HAD A PIECE ON IT TO KEEP THE BOLT FROM SPINNING, BUT THE BOLT SPUN OUT AS EE WAS PUTTING PRESSURE ON THE BOLT WITH HIS FINGER. THE BOLT CAUGHT EE'S FINGER, CAUSING A LACERATION.
