Struck by falling object

While changing a belt on a conveyor, the employee lost control of the guard on the head pulley. The head pulley then struck the employee in the right arm causing a laceration.

He was in a man lift trying to change a gate at the scalpering screen, when he said his back was hurting. he was sent to a medical dr. after complaining of his back hurting. He was at the chiropractor the night before for his back.

Employee smashed his middle finger between a crowbar and manlift handrail as he was trying to dislodge a large rock from a head pulley. The head pulley was locked out.

A shop mechanic used a razor pocket knife to cut a rubber hose from a coupling. He placed the knife on the ground beside him and pulled on the hose. When the hose came loose, his hand whipped to the side and struck the blade that was on the ground.
