While changing a belt on a conveyor, the employee lost control of the guard on the head pulley. The head pulley then struck the employee in the right arm causing a laceration.

The employee was getting off of a manlift into muddy conditions and slipped and twisted knee.

He was in a man lift trying to change a gate at the scalpering screen, when he said his back was hurting. he was sent to a medical dr. after complaining of his back hurting. He was at the chiropractor the night before for his back.

Employee smashed his middle finger between a crowbar and manlift handrail as he was trying to dislodge a large rock from a head pulley. The head pulley was locked out.

A shop mechanic used a razor pocket knife to cut a rubber hose from a coupling. He placed the knife on the ground beside him and pulled on the hose. When the hose came loose, his hand whipped to the side and struck the blade that was on the ground.

The employee was walking through the plant area and was struck by a falling rock. The rock struck the hard hat causing the hard hat liner to cut his forehead.

Employee struck his head when his forklift tipped to its side.

Employee was striking a steel bearing case with a sledge hammer and when a piece of metal seperated from the case and struck injured in right arm.

EMPLOYEE WAS CRAWLING FROM UNDERNEATH A CONVEYOR AND FELT A ""POP"" IN HIS LEFT KNEE.

EMPLOYEE WAS CLIMBING DOWN THE LADDER OF A LOADER AND STEPPED ON A ROCK. THE ROCK CAUSED HIS ANKLE TO TWIST.

EE WAS ATTEMPTING TO PUT A PIN BACK THE TAILGATE HINGE WHEN IT SHIFTED AND FELL, STRIKING HIM ON THE RIGHT ANKLE.
