Employee was replacing a tail pulley on the pea gravel belt when the shaft of the tail pulley slipped and causing a finger to get caught between the shaft and conveyor belt frame. Employee stated they pulled the finger back in a jerking motion causing the hand injury.

Two employees were picking up a 6 inch hard suction hose that weighs approximately 80 pounds. EE says foot slipped and EE pulled a muscle in the back and requested to see a doctor.
The employee was replacing a 6 inch 40ft discharge hose on a water pump. After connecting the new hose, the employee felt a sharp pain in the groin area.
Employee experienced shoulder pain while opening and closing a window in the office. Employee was sitting down while opening the window, overextending shoulders. The pain progressed throughout the week. Employee requested to go to the doctor on 12/18/2017.

EE was working inside a material chute. He was using a cutting torch to re-fabricate the shoot. A piece of slag fell down his shirt. He moved to quick and twisted his knee. EE has a strained MCL.
Employee lifted a 10 inch pipe into bed of truck and while setting it down the pipe rolled and struck his left wrist causing fracture. employee was released to restricted duty.
The employee was greasing the plant and rock fell between the screen and guard. The rock struck him on the lip.
Employee was standing under overhead hopper when a 2.5 inch rock fell and struck on left shoulder, employee was examined by physician and was release to regular duty, employee followed up with physician on 3/28/13 and was put on restricted duty due to soreness until 4/11/13 follow up visit.
EE was cutting steel I-beams into 8' lengths. After making cut, slag was still holding the cut together, EE picked up the opposite end and dropped it to break the slag. As he did this, the I-beam caught his glove and smashed his right ring finger between two pieces of I-beam.
Employee was helping rotate a suction hose to connect to the a de-watering pump when he said he felt a pop in his right shoulder, he was examined by a physician and was returned to work with restrictions, and will follow up with doctor on May 20th.
EE and two other employees were installing new chains on an Eagle Crusher. The bar the chains were on slipped out of the bracket they were being attached to and struck EE's left thigh causing pain. EE was examined by a physican and returned to restricted duty. He will follow up with physican.
Employee dropped water bottle in Cat D30 Haul unit air condition vent hole, while retrieving the bottle, employee injuried right ring finger tip when finger made contact with blower fan. While getting first aid treatment in the office, the employee fainted and broke his front tooth when he hit the desk.
Employee hurt left knee while placing Boring Bar into hole on an Excavator.
While checking front windshield wiper, the wiper became stuck, emplyee got out and found a 1.5 inch diameter rock stuck at the base of the wiper, he grabbed the rock with his left index finger and thumb and pulled back on it, the rock and the wiper started to move back to the start position, catching the left thumb between the wiper arm assembly, caused a fracture on left thumb.
EMPLOYEE ALLEGES THAT HE HURT HIS LEFT SHOULDER WHILE ROLLING A HYDRAULIC HOSE. EMPOLYEE WAS TREATED BY AN M.D. AND RETURNED TO WORK ON RESTRICTED DUTY AND WILL FOLLOW UP WITH HIS DOCTOR IN TWO WEEKS.
Employee atempted to use rope to stop barge. While pulling up to dredge to re-fuel, employee wrapped the rope around the handrail on the barge while it was still moving, causing a cut on his right ring finger when his hand became entangled in the rope.
employee was swiping dirt off of trailer ramp with his right hand and a sliver of metal cut through his glove and little finger. Employee received stiches and was released to light duty. Employee will have stiches removed in 10 days.
Employee was placing wheel hub on water truck axle and felt pain in his lower back. The employee sought medical care the following day and was told he strained his back. He was restricted to light duty work until 11/01/2007.
While unwinding grease hose from hose reel, employee felt a burning sensation in his abdomen, thought he had just pulled a muscle, however the pain persisted. The following day he told his supervisor. The employee went to the doctor, who took him off work due to a possible hernia and was refered to a specialist.
Employee was greasing WA 500 Komatsu Front-end lader, when the grease gun hose detached from the grease fitting and sprayed employees face and right eye. He recieved medical treatment the following day for swelling in his right eye, the doctor prescribed eye drops and was released to full duty on September 29, 2006.
Employee was cutting out a wall on a classifier with a cutting torch when he heard a popping sound, he then stopped cutting and looked around to see where the popping sound had come from, he then heard a second pop, and felt a burning sensation on his right forearm and thumb. the plant manager drove the employee to hospital, where he was treated and release to work.
Employee was hooking welding trailer to pick-up and was not lined up with trailer and tried to pull trailer to align with pick-up and strained his back. He was seen by a doctor on 5/3/06 and was told he strained his back. He was returned to work with restrictions and wil follow up with doctor on 5/12/06.
On 3/25/06, EE told his supervisor that his back was aching. On 3/27/06, EE wanted to see a Dr. He stated that his back had been hurting for a couple of weeks and believed pain was caused from a previous back injury several years ago. The Dr. asked EE what he had done and EE said his normal job duties, and on Saturday he had removed a pulley from a motor shaft.
Employee hurt right shoulder while adjusting tail pulley on feeder belt. He reported injury to plant manager but did not want to see a doctor at this time. On April 4, 2006 he requested to see a doctor and later was referred to an orthopedic doctor and is waiting to be scheduled for minor corrective surgery.
Employee was tightening air line on Euclid b-30 haul unit when coupling broke and air blew dirt in left eye. Employee went to the doctor and was prescribed ointment for eye and returned to regular duty.
