Employee was conducting maintenance activities and was using a wrench and felt a pop in left wrist. Was seen at Occupational Medical Center where they were given a brace for their wrist.

EE was cutting a hose with a utility knife. While cutting the hose the EE hit the wire in the hose, causing the blade to jump and cut the employee's forearm.

While reinstalling a head pulley after a bearing replacement, the miner was using a crow bar to loosen the motor adjusting nuts. EE felt a pop in the right shoulder.

Laborer while cutting out new skirting material out of old belting, their grip slipped causing laceration of left hand while using a box cutter.

Maintenance Tech. was repairing the rear step mounting bracket on a loader. It was bent and EE was trying to straighten it out. First, EE had it in a vice and was using a 3lb. hammer. EE then placed it on the ground to get a better angle, when struck, the bracket flew upwards striking them in the upper lip causing a laceration.

While cutting a hose with a box cutter, operator received laceration to left index finger.

Operator was attempting to remove a worn tooth from a payloader bucket using a hammer. A small section of the tooth chipped off striking their upper left arm causing a laceration.

The employee was tightening the nut on the head section of the 7'standard crusher. A suspended weight and flat bar were being used to conduct final tightening. The employee was on the other side of the head and they did not realize the tip of finger was in one of the bolt holes on the bar. When the bar was struck, the bolts broke and the bar shifted cutting the tip of the finger.

Using a sledgehammer to drive in the bucket pin on the loader, they noticed that left wrist and hand were sore. Employee reported but did not see a doc until the 15th.

They were using a come along to pull the motor tension back, this allowed them to replace the drive belt onto to the pulleys. Come along slipped causing the motor to land on EE's hand.

EE had just used a razor knife to cut one piece of rubber and had begun installing it while IE measured out and began to cut the second strip. The knife slipped and cut through the leather glove, into IE's wrist.

Setting legs on the log washer, using a sledge hammer to level the bottom portion of the legs which resulted in the top part tipping on landed on employee.

EE was pitching stone using a hammer and chisel when EE felt a pain in the right arm.

Employee was cutting worn belting for skirts with a utility knife. Employee removed their left glove on the last cut. Employee slipped with the knife and cut a finger.

A conveyor belt that was needing to be replaced, employee cut hand with knife.

Unbinding the chains that had the front of the CAT 345 secured. Chain was tight so a bar was used to loosen the binder. When loosening the binder, it turned and employee was struck in the right check causing a laceration.

While pounding on tooth of loader bucket with a hammer, a small 1/4" piece of the hammer chipped off and lodged in left upper thigh.

Employee was bent over pounding out screens with a 5 pound hammer. When he stood up, he felt pain in his lower back.

Cutting stone with a large hammer, rock split and fell towards employee. Employee strained body/back trying to get out of the way.

Employee removing grease tube from grease gun with knife and allegedly cut thumb.

Cutting Belting, blade fell out of knife and lacerated right thumb.

Loosening bolts on guard to check for oil leak and turned bolts to far and guard fell off pinning elbow between the plate and ground.

Hit tip of right ring finger with hammer while lining up a bolt hole.

EMPLOYEE WAS TRYING TO OPEN A WATER VALVE WITH A PIPE WRENCH AND WRENCHED BACK.

CUT TO RIGHT INDEX FINGER; FINGER PINCHED BETWEEN COME ALONG CHAIN AND HEAD PULLEY GUARD.

Head of hammer broke off and bounced off a belt and struck employee above the left eye.

Employee was prying a rock from under the jaw crusher. The rock came down and hit his leg, cutting the lower leg, requiring stitches.

Employee was scraping the side of the crusher with a putty knife and his hand allegedly slipped cutting 2 fingers on his right hand requiring stitches to his fingers.

EMPLOYEE WAS SPLITTING SLABS WHEN THE HAMMER HANDLE CAME OFF AND HIT HIS FINGER WHICH WAS ON THE ROCK HE WAS SPLITTING.

EMPLOYEE WAS HOLDING A BAR TO BREAK UP FROZEN MATERIAL WHEN ANOTHER EMPLOYEE HIT THE BAR WITH A HAMMER. THE HAMMER HIT THE EMPLOYEE'S FINGER REQUIRING STITCHES.

EMPLOYEE WAS UNPLUGGING A CRUSHER WHEN THE PRY BAR HE WAS USING WHIPPED AND ALLEGEDLY HIT HIS LEFT WRIST. THE DOCTOR SAID IT WAS A SPRAIN.

EE ALLEGES THAT WHILE CHANGING OIL ON A TRUCK HE WAS USING A BREAKER BAR TO LOOSEN THE OIL PLUG. THE BAR SLIPPED AND CUT HIS FINGER WHEN IT HIT THE RAIATOR PIT COCK.

EE ALLEGES THAT WHILE HOLDING A WRENCH FOR A MECHANIC, THE HANDLE BROKE AND HIT HIM ON HIS LEG.
