Struck by falling object

Employees were trying to lower the gates inside the rolling rock box. To lower the gate, 15 bolts needed to be removed. When the last bolt was removed, the gate slipped and landed on the foot of one of the employees.

Over-exertion in pulling or pushing objects

Employee was conducting maintenance activities and was using a wrench and felt a pop in left wrist. Was seen at Occupational Medical Center where they were given a brace for their wrist.

Laborer while cutting out new skirting material out of old belting, their grip slipped causing laceration of left hand while using a box cutter.

Caught in, under or between a moving and a stationary object

The employee was tightening the nut on the head section of the 7'standard crusher. A suspended weight and flat bar were being used to conduct final tightening. The employee was on the other side of the head and they did not realize the tip of finger was in one of the bolt holes on the bar. When the bar was struck, the bolts broke and the bar shifted cutting the tip of the finger.

An employee was assisting in re-welding a section of the mainframe liner, during the process, employee was exposed to arc-flash.

Employee was cutting worn belting for skirts with a utility knife. Employee removed their left glove on the last cut. Employee slipped with the knife and cut a finger.

Struck by flying object

EE was standing behind the employee trying to remove a worn tooth on the loader bucket. A small piece of the tooth came off hitting EE in the left cheek area below the safety glasses.

While pounding on tooth of loader bucket with a hammer, a small 1/4"" piece of the hammer chipped off and lodged in left upper thigh.
