Employee was operating the 345 excavator clearing in front of the face. The bucket dropped off a ledge causing the machine to jar. The shudder of the machine resulted in the operator hurting their lower back. Was seen by medical professional who restricted them to resting for one week.

Employee bent over to open a bag of mansand to empty it out. When EE straightened their back they felt a pop. Injury is a result of ergonomic nature. Employee initially injured back on 10/08/2024 and has been trying to care for injury at home. Activities completed on 11/22/2024 aggravated the injury. Employee went in to ER on 11/24/2024 & was kept overnight for treatment to back.

Miner was dismounting equipment and tripped landing on the ground, knees first. Was placed on restrictions and physical therapy.

The Employee informed us has Dust Disease of the Lungs (Pneumoconiosis) - Silicosis.

Driller was climbing into the cab of the drill, which was parked at a slight angle, when the door swung shut pinching a fingers on left hand causing a laceration.

Iron worker was tied off on a beam with a retractable lanyard. Walking when lanyard suddenly locked. This caused the worker to fall onto surface of the beam in an awkward position. They received a contusion to the right side of their back.

Employee was moving rock around and out of a 7' standard crusher after it had plugged. Rock shifted and pinched finger between rock and metal cone.

EE was using a boring bar and cut left finger.

While unloading acetylene tank from truck the tank slipped and fell on EE's finger causing a fracture.

Lifting a gas tank into the back of the pickup.

EE had just used a razor knife to cut one piece of rubber and had begun installing it while IE measured out and began to cut the second strip. The knife slipped and cut through the leather glove, into IE's wrist.

EE was unhooking chains attached to an item that had been picked and set in place by the crane. As EE was pulling the chains, EE said EE felt a strain in EE's back. EE's back was sore and stayed home to rest for two days following the incident before going to the clinic.

Using a sledge hammer to break apart rocks that were lodged in the grizzly rails

EE was going up the stairway and EE scraped finger, on the top behind the fingernail, but did not think it was serious and did not report the injury at that time.

Lifting a turbo using an A frame hoist, while picking up the shut felt a pain in EE's arm.

Adjusting nut on crusher tramp release cylinder bolts. Trying to loosen net and felt a pain in arm.

Walking and twisted knee.

Two employees were lifting a piece of garbage metal approximately 80LBS off a service truck while doing this employee felt a pulled a muscle in stomach which then became a hernia

Employee was torching a steel plate on the Return Deister deck. This area has a 2 deck screen bed where screens are changed out weekly/bi weekly when worn. Torching and welding when needed. Conditions were windy that day and while torching a piece of slag went into employees ear. Plant was down for maintenance and all PPE was worn.

Employee was carrying a piece of steel 4ftx2ftx3/8 thick up a flight of stairs. EE turned on the landing of the stairs and felt like ee may have pulled a muscle. EE wasn't too concerned about it at that time. EE texted the next day that EE couldn't put any pressure on leg.

A flat rock approx 6" wide and 20" long bridged across the step box end. The employee attempted to dislodge the rocks. EE's left hand was struck by a rock from up above in the chute. The natural reaction was to pull EE's and back which caused the fall. There was no property damage.

Repair work was being done to an HP400 Crusher. EE was working inside crusher cavity and climbed out to help work on the top side. While climbing out EE felt a pull/prick on left side of EE's lower back and leg

Doing mainframe liner change hit left knee on cateye when moving to climb out of crusher

Unbinding the chains that had the front of the CAT 345 secured. Chain was tight so a bar was used to loosen the binder. When loosening the binder, it turned and employee was struck in the right check causing a laceration.

The employee was removing a u-clamp from a vehicle when his finger was pinched between impact and jack stand. He was taken to a clinic and had two stitches placed in his finger.

Repetitive use of hand tools and air tools and heavy lifting have resulted in bilateral carpal tunnel syndrome.

Reinstalling a truck set under a railcar - when pushing on wheel received a steel sliver in the palm of left hand.

Employee was lifting metal up on a screen bed. As he was lifting the metal, he pinched his finger between the metal in his hand and an I-beam for the deck above.

Employee was bent over pounding out screens with a 5 pound hammer. When he stood up, he felt pain in his lower back.

Driving 35 ton haul truck and as he drove, his back became stiff and eventually went into spasms and became sore.

Employee performing maintenance on screen bed and went to dismount and feet caught on air line and he lost his balance and fell.

Heat Exhaustion and Dehydration

Cutting a piece of expanded metal and cut right knee.

Torching metal and in doing so, slag splattered and got behind his safety glasses and went in eye.

Employee was torching bolts off of the discharge plate of a screen bed. There was built up pressure between the plate and screen causing the plate to pop off, hitting him in the right forearm, causing a laceration.

Changing screens on the west 3D screen.

Retreiving parts from upstairs and when turned to leave, tripped over a box and fell on a piece of steel and busted face open.

Kneeling down, setting up wheel lathe and went to stand up and had pain in right leg. Wasn't able to put any weight on it.

Changing a screen on 3D Deister and screen fell down on palm of right hand.

Loosening bolts on guard to check for oil leak and turned bolts to far and guard fell off pinning elbow between the plate and ground.

Helping put back screen plate and the plate got stuck and while trying to move the plate, it shifted and smashed his ring finger on the right hand.

Rubbed metal partical from hand or face into right eye.

Pulling metal shavings away from wheel on locomotive and one of the shavings cut through glove and cut finger tip.

A spec of welding slag in right eye while welding.

Strained right knee while putting oil in 50 ton Cat truck

Hit tip of right ring finger with hammer while lining up a bolt hole.

Caught a spark in the eye.

Squatted down in short cab to check sanding valve and when stood up there was extreme pain in the right knee

Right knee injured when working on jaw crusher in primary.

Walking past angle iron and hit knee against it.

PUTTING ON RUBBER BOOTS TO BEGIN WORK OUTSIDE.

Employee was climbing down a ladder and his knee popped.

EMPLOYEE FELL THROUGH AN ACCESS DOOR THAT WASN'T PROPERLY SECURED.

EMPLOYEE WAS TRYING TO OPEN A WATER VALVE WITH A PIPE WRENCH AND WRENCHED BACK.

Worker was pulling on a tube for the rolling rock box, when his hand slipped and his little finger went under one of the rollers.

Small puncture wound on right forearm due to contact with a sharp object.

Moving railroad ties, saw a splinter, went to remove splinter of wood when a tie slipped and cut skin of finger.

CUT TO RIGHT INDEX FINGER; FINGER PINCHED BETWEEN COME ALONG CHAIN AND HEAD PULLEY GUARD.

CLEANING OUT THE GRIZZLY SHE PINCHED HER FINGERS BETWEEN TWO ROCKS.

Head of hammer broke off and bounced off a belt and struck employee above the left eye.

Employee was allegedly shoveling rock in tunnel from floor on to belt and developed a hernia.

EMPLOYEE WAS ALLEGEDLY GETTING ROCKS OUT OF THE TAIL PULLEY OF A RAILCAR.

Electrical cable at entrance interior walls burned.

EE WAS WELDING ON A CONVEYOR IN A TUNNEL. WHEN FINISHED, EYES BEGAN TO HURT AT THE END OF THE DAY

EMPLOYEE WAS HOLDING A BAR TO BREAK UP FROZEN MATERIAL WHEN ANOTHER EMPLOYEE HIT THE BAR WITH A HAMMER. THE HAMMER HIT THE EMPLOYEE'S FINGER REQUIRING STITCHES.

EE ALLEGEDLY SLIPPED ON ICY STEP ON THE LOCOMOTIVE AND PULLED HIS SHOULDER WHILE TRYING TO HANG ON TO THE HAND RAIL. ON FEBRUARY 20, 2004 EE HAD SURGERY ON HIS SHOULDER DUE TO THIS INJURY.

EE WAS CLEANING SNOW OUT OF RAILROAD CARS WHEN HE COMPLAINED OF PULLED MUSCLES IN HIS LOWER BACK.

EE WAS CUTTING A BOLT WITH A CUTTING TORCH WHEN HIS FLANNEL SHIRT STARTED ON FIRE BURNING HIS STOMACH AND HAND.

EE INJURED RIGHT HAND WHEN THE PUMP HOUSING HOOKED TO A CHAIN TIPPED AS HE WAS UNLOADING IT AND HIS HAND HIT THE CRANE HOOK.

EE ALLEGES THAT WHILE CLEANING AND CLOSING RAILROAD CAR DOORS HE PULLED BACK MUSCLE.

EMPLOYEE WAS FOLDING UP A CONVEYER WHEN HE SLIPPED AND FELL ABOUT FOUR FEET TO THE DECK BRUISINGHIS LEG AND HIP. THE DOCTOR FELT THE EMPLOYEE JUST HAD BRUISES. NOW THE EMPLOYEE STARTED HAVING MORE PAIN SO THE DOCTOR PRESCRIBED NO WORK FOR ONE WEEK. FORST DAY OFF WORK WAS 1-7-02.

ON 3/16/01 EE SAID HE WAS GOING TO A DOCTOR FOR AN UNKOWN ARM PAIN NOT WORK RELATED. ON 3/21 HE CALLED HIS FOREMAN AND SAID HE HAD SURGERY ON HIS NECK BUT DID NOT KNOW WHEN HE COULD COME BACK TO WORK. ON 3/23 HE TALKED TO ANOTHER SUPERVISORAND SAID HE WOULD NOT BE PAID FOR HIS TIME OFF WORK SO HE WOULD NEE THIS INJURY TO BE WORKMAN'S COMP IN ORDER TO RECEIVE PAY.

EMPLOYEE PUSHED A HEX BELT INTO A HOLE AND HAD HIS FINGER BETWEEN THE HEX HEAD AND THE METAL WHEN THE NUT GOT TIGHTENED HIS FINGER GOT PINCHED.

EE WAS GETTING OUT OF THE HAUL TRUCK WHEN HE SAID HE TWISTED HIS LEFT KNEE AS HE TURNED AROUND TO CLIMB DOWN THE STEPS OF THE TRUCK. HE HAS HAD NO PROBLEMS UNTIL TODAY.

EMPLOYEE ALLEGES HE TWISTED HIS ANKLE WHEN HE STEPPED ON A ROCK.
