Fall to the walkway or working surface

Employee was washing out under the cone crusher to prepare for maintenance. While performing the task, employee slipped and fell landing on EE's ribs. Was seen at local ER and prescribed pain medicine. Was able to return to work with zero restrictions.

Struck by flying object

Cable and hook was wedged between two rollers. Miner pulled the cable out and the hook struck EE in the mouth resulting in a chipped tooth.

Struck against a moving object

Employee was operating the 345 excavator clearing in front of the face. The bucket dropped off a ledge causing the machine to jar. The shudder of the machine resulted in the operator hurting their lower back. Was seen by medical professional who restricted them to resting for one week.

Employee bent over to open a bag of mansand to empty it out. When EE straightened their back they felt a pop. Injury is a result of ergonomic nature. Employee initially injured back on 10/08/2024 and has been trying to care for injury at home. Activities completed on 11/22/2024 aggravated the injury. Employee went in to ER on 11/24/2024 & was kept overnight for treatment to back.

Fall from machine

Miner was dismounting equipment and tripped landing on the ground, knees first. Was placed on restrictions and physical therapy.

Inhalation of radiations, caustics, toxic and noxious substances

The Employee informed us has Dust Disease of the Lungs (Pneumoconiosis) - Silicosis.

Driller was climbing into the cab of the drill, which was parked at a slight angle, when the door swung shut pinching a fingers on left hand causing a laceration.

Iron worker was tied off on a beam with a retractable lanyard. Walking when lanyard suddenly locked. This caused the worker to fall onto surface of the beam in an awkward position. They received a contusion to the right side of their back.

Employee was moving rock around and out of a 7' standard crusher after it had plugged. Rock shifted and pinched finger between rock and metal cone.

EE had just used a razor knife to cut one piece of rubber and had begun installing it while IE measured out and began to cut the second strip. The knife slipped and cut through the leather glove, into IE's wrist.

EE was unhooking chains attached to an item that had been picked and set in place by the crane. As EE was pulling the chains, EE said EE felt a strain in EE's back. EE's back was sore and stayed home to rest for two days following the incident before going to the clinic.

EE was going up the stairway and EE scraped finger, on the top behind the fingernail, but did not think it was serious and did not report the injury at that time.

Two employees were lifting a piece of garbage metal approximately 80LBS off a service truck while doing this employee felt a pulled a muscle in stomach which then became a hernia

Employee was torching a steel plate on the Return Deister deck. This area has a 2 deck screen bed where screens are changed out weekly/bi weekly when worn. Torching and welding when needed. Conditions were windy that day and while torching a piece of slag went into employees ear. Plant was down for maintenance and all PPE was worn.

Employee was carrying a piece of steel 4ftx2ftx3/8 thick up a flight of stairs. EE turned on the landing of the stairs and felt like ee may have pulled a muscle. EE wasn't too concerned about it at that time. EE texted the next day that EE couldn't put any pressure on leg.

Struck by falling object

A flat rock approx 6"" wide and 20"" long bridged across the step box end. The employee attempted to dislodge the rocks. EE's left hand was struck by a rock from up above in the chute. The natural reaction was to pull EE's and back which caused the fall. There was no property damage.

Unbinding the chains that had the front of the CAT 345 secured. Chain was tight so a bar was used to loosen the binder. When loosening the binder, it turned and employee was struck in the right check causing a laceration.

The employee was removing a u-clamp from a vehicle when his finger was pinched between impact and jack stand. He was taken to a clinic and had two stitches placed in his finger.

ON 3/23 HE TALKED TO ANOTHER SUPERVISORAND SAID HE WOULD NOT BE PAID FOR HIS TIME OFF WORK SO HE WOULD NEE THIS INJURY TO BE WORKMAN'S COMP IN ORDER TO RECEIVE PAY.
