Employee was clearing rocks from the dump site going to the primary plant. Was struggling with loading a rock, so EE picked the rock up and carried it over to the dump. Rock slipped from the employee's grip landing on EE's right foot.

EE was cutting a hose with a utility knife. While cutting the hose the EE hit the wire in the hose, causing the blade to jump and cut the employee's forearm.

Operator was cleaning snow around the shop. EE damaged a cardboard box of bolts that was on the ground and waterlogged. EE parked the skid loader to go into the shop to find a dry box and slipped on transition from gravel to pavement. EE fell on their side fracturing their hip. It was snowing with freezing rain on this day.
Operator was jack-hammering frozen material in L-7 hopper. When the Hilti tool slipped, EE's left hand contacted a scraper bolt which cut through the works gloves resulting in a laceration.

Employee was removing chute covers from Ode wash plant when the cover rotated and struck their forearm.
EE tripped and fell on a rock causing a cut to EE's knee. The cut is deep but not very wide. I sent EE to the occupational health site to see if stitches are required

Employee was sliding a 30 lb pillow block bearing onto a shaft. Employee was wiggling it when it popped into place, pinching the tip of employee's right little finger between the bearing and the frame.
Employee was loading heavy pieces of steel into the back of a pickup when EE felt pain in the lower back.

Employee was doing yard clean-up when the wind blew something into the left eye. Foreign object removed next day at clinic.

Employee was changing the cutting edges on the bucket of the front end loader. While loosening one of the bolts, it spun in the hole and cut palm and middle finger of left hand. Twelve stitches.
Employee was using a chain ""come-along"" to raise a pipe into position to bolt it onto the collar of another pipe. The chain slipped off the pipe and cut right middle finger. Five stitches.
A group of employees were using bars to get rocks unstuck from a cone-shaped bin. A rock fell on employee's bar. Employee's hand flew up against the bin and employee's finger was cut. Six stitches.

Employee and a coworker were going to splice two ends of a power cord for a hand grinder together. They were stripping the wire when the coworker's knife slipped and poked into the employee's forearm. Three stitches.
Employee was climbing up and out of a rock crusher. The place he grabbed to pull himself out was sharp and he cut his left little finger. Four stitches.

Employee was climbing down a ladder from a platform next to a conveyor. He missed the last two steps and fell about two feet (landing on his feet) to the ground jarring his lower back.
Employee spent about six hours working outdoors that day. Wind-chill was about -20 degrees. Some of his work required removal of his heavy gloves and working with lightweight ones. Went to doctor on 4-29-14, frostbite on tip of right index finger.

EE was loading haul trucks, his loader bucket was full of material. A large rock came over back of the bucket, falling into the cab, braking the windshield, and hitting his right hand and forearm.
Employee was climbing down from his haul truck. He slipped and fell about seven feet to the ground.

THE EMPLOYEE INVOLVED WAS HELPING ANOTHER EMPLOYEE WORK ON A DRILL AT THE QUARRY SHOP. THEY WERE USING A 48"" PIPE WRENCH, WHICH SLIPPED & STRUCK HIM ON HIS LEFT KNEE. IT REQUIRED (2) STITCHES.
IT WAS VERY WINDY ON 3/7/05 AND THE EMPLOYEE HAD WORKED OUTSIDE ALL DAY. THAT NIGHT HIS EYES STARTED HURTING, SO HE WENT TO THE EMERGENCY ROOM. THE DOCTOR SAID THAT BOTH OF HIS CORNEAS WERE SCRATCHED, POSSIBLY FROM SAND.

EMPLOYEE WAS WORKING ON A PIECE OF EQUIPMENT WHILE STANDING ON A MAN-LIFT. HE WAS RAISING THE LIFT WHEN IT JERKED AND HE HIT HIS HEAD ON THE PLATFORM ABOVE HIM.
