While running the stone splitter, one of the machine chisel's carbide teeth broke off sending a metal shard into the operator's neck.

Employee was splitting a rock, the splitter hung up under pressure. Employee was reaching to turn the splitter off to release pressure when the rock split and jumped backwards and hit his left hand middle finger.

After repairing hyd. hose ee slipped on a step while climbing down from the excavator injuring his left shoulder. Sore shoulder continued until he got scheduled for surgery on 5-11-05.
