While running the stone splitter, one of the machine chisel's carbide teeth broke off sending a metal shard into the operator's neck.

Employee was moving a pallet to take inside the saw shed when the employee heard shoulder pop.

EE was pitching stone using a hammer and chisel when EE felt a pain in the right arm.

Working on a hydraulic leak and slipped on some fluid on the ground and twisted knee.

Employee was setting a stone on a feeder conveyor of the splitter and pinched his finger tip between the stone and the conveyor frame.

Employee was splitting a rock, the splitter hung up under pressure. Employee was reaching to turn the splitter off to release pressure when the rock split and jumped backwards and hit his left hand middle finger.

Moving the seat wall stones. Putting slings under the stone. One employee moved his end to get his strap under and that shifted injured employee's end pinching his fingers.

Reported soar neck from operating excavator

Cutting stone with a large hammer, rock split and fell towards employee. Employee strained body/back trying to get out of the way.

Coming down for lunch and slipped and fell. His arm landed on some branches, which cut his wrist.

Loading pallets out and finger got pinched between two rocks.

Putting stones on a pallet. Tried to push the top row in to make tighter. The rock pushed in and arm hit another rock and cut it.

Lifting rock up onto conveyor and back popped out of place/3 ribs

After repairing hyd. hose ee slipped on a step while climbing down from the excavator injuring his left shoulder. Sore shoulder continued until he got scheduled for surgery on 5-11-05.

EMPLOYEE WAS SPLITTING SLABS WHEN THE HAMMER HANDLE CAME OFF AND HIT HIS FINGER WHICH WAS ON THE ROCK HE WAS SPLITTING.

EMPLOYEE WAS CLIMBING OUT OF TRACTOR WHEN HE LOST HIS FOOTING AND TWISTED HIS ANKLE. NOTE: EMPLOYEE WAS SUPPOSED TO START THERAPY ON 12/9/02 BUT HE QUIT WORK FOR OUT COMPANY ON 12/8/02.
