Over-exertion in lifting objects

Was going to help another employee lift an oxygen bottle into the back of a service truck and felt a pull in EE's abdomen. Reported to supervisor and denied observation. Stated EE would let them know if the condition worsens. Came to supervisor at a later date and requested to be seen by the doctor. Doctor placed on restrictions of not lifting more than 25 pounds for 7days.

EE attempted to start the truck, but the battery was dead. EE went to grab the jumper cables lying on the ground and felt a sharp pain in their lower back.

Struck against a moving object

After replacing the tail pulley on the bridge belt, the operator, in the basket with fall protection attached, was moving the manlift with the basket approximately 4 feet above the ground. The jib boom failed and the basket dropped to the ground. This caused the operator to strike the basket resulting in a compression fracture of a vertebra and sprained knee.

Shoveling the Tunnel and was shoveling material back onto the belt when felt a pop in their back.

Over-exertion in wielding or throwing objects

Using a sledgehammer to drive in the bucket pin on the loader, they noticed that left wrist and hand were sore. Employee reported but did not see a doc until the 15th.

They were using a come along to pull the motor tension back, this allowed them to replace the drive belt onto to the pulleys. Come along slipped causing the motor to land on EE's hand.

Employee was lifting a section of pipe, employee was lifting with another employee when back started to hurt. Also moved some 5 gal buckets which made it worse.

Over-exertion in lifting objects

A tube was being fed through the catwalk with a loader, with two employees guiding it on the catwalk. While lifting the tube on shoulder, one employee felt pain in shoulder. Upon investigation, the employee felt employee put too much weight on one shoulder and was not lifting properly.

Working on conveyor frame when employee's finger was pulled in between the two frame flanges by their gloves.

Caught in, under or between a moving and a stationary object

EE and co-worker were working on the tail pulley shaft for the mainline stacker. They had taken a come along and attached it to the shaft of the tail pulley and pulled the shaft backwards. When EE went to remove the bearing from the shaft the bearing turned and crushed EE's finger in between the bearing and the tail pulley adjuster.

A conveyor belt that was needing to be replaced, employee cut hand with knife.

While attempting to loosen the adjuster jam nut, EE placed both feet against the crusher frame in an attempt to increase leverage. Before I even realized what ee was doing, the nut loosened causing ee to fall to the deck.

Struck by falling object

As EE was putting on harness below the cat walk of the McCloskey R155. Co-worker was climbing into the hopper from the cat walk, to shovel. Co-worker threw the shovel towards the hopper and missed. When co-worker missed, the shovel fell back down and hit EE on the brim of hard hat and glanced off of neck.

Right foot got caught by concrete debris on levy. Slipped on mud when stepping over other debris with left foot. This caused his right knee to twist and hyper-extend.

Struck by falling object

Using left arm to activate switch and right hand to hold open the access panel and the panel slipped and struck his left arm.

Over-exertion in pulling or pushing objects

Pulling hydraulic assembly out of backhoe when it allegedly slipped out of hand and dropped on finger.

The ee allegedly injured his right shoulder when he slipped on ice and fell down landing on his shoulder. He has not lost any time from work but has been put on restricted duty.
